As part of the Ending Eclampsia Project and supported by the Pre-eclampsia Technical Working Group, the Population Council developed High Impact Practice (HIP) briefs for three strategies that show promise for improving the prevention, detection, and management of Hypertensive Disorders in Pregnancy (HDPs). HDPs affect 10 percent of women globally and are major contributors to maternal and newborn mortality, morbidity, and disability. Providing essential services and health education for women at all levels of the health system is necessary to improve survival rates and quality of life for those who experience HDPs and their newborns. Implementers of sexual and gender-based violence programs in refugee settings are typically eager to learn about the extent to which their programs are making a difference in the lives of those they serve. Basic monitoring information can help address this imperative without unduly burdening program implementers with time-consuming evaluations. The Sauti/VOICE project is guiding adaptation and implementation of evidence-based SGBV interventions in refugee contexts. These interventions are implemented by UNHCR partners in eight countries in the East, Horn, and Great Lakes region of Africa. Health care providers collect an array of documentation and specimens to support criminal investigations. Such documentation and specimens are referred to as “medico-legal” or “forensic” evidence, and in the case of rape, include a survivor’s documented injuries and emotional state at the time of examination, as well as samples and specimens from the survivor’s body or clothing. For survivors of rape and defilement who want to obtain legal justice, medico-legal evidence is critical. In African countries, health providers and the police are depended upon to ensure such evidence is collected, but medico-legal evidence collection can present challenges. The ZAMFAM program aims to improve the care and resilience of orphans and vulnerable children (OVC) as well as young people living with HIV in Zambia by providing support and protection and by strengthening the capacity of children, families, and the community to better manage the socioeconomic shocks of the HIV/AIDS epidemic. A case study approach was employed to understand actors and perceptions, and to document best practices by the program. This brief summarizes the case study findings. There are substantial gaps in essential services and numerous missed opportunities to engage children in care and provide effective HIV treatment. Although not new, family-centered care models that provide HIV services comprehensively to families as a unit, rather than providing separate services to children and adults, have the potential to address family needs and break down social, physical, and emotional barriers to accessing care. The family-centered care study (FAM-CARE) aims to contribute to our understanding of the role of family-centered care models (FCCM) in improving pediatric HIV outcomes in a sub-Saharan African setting.

Zambia’s HIV prevalence is estimated at 11% among adults aged 15–49 years and 1% among children younger than 15 years. An estimated 10% of Zambia’s population is at high risk of being orphaned or vulnerable due to the HIV epidemic. The Zambia Family (ZAMFAM) project aims to improve the care and resilience of vulnerable populations while supporting HIV epidemic control. ZAMFAM used a case management approach that tracks beneficiaries from identification to graduation. Breakthrough RESEARCH, in collaboration with De La Salle University’s Social Development Research Center (DLSU-SDRC) in Manila, conducted a targeted qualitative research study to facilitate social and behavior change approaches for out-of-school ages 15 to 19 for positive reproductive health and family planning behavior change. Research in three regions of the Philippines generated evidence for the design of intervention approaches to be most effective for four priority behaviors: delay of sexual debut, use of modern contraceptive methods, care-seeking in the first trimester for pregnant youth, and birth spacing of three to five years to prevent short birth intervals.

While access to and uptake of modern family planning (FP) in Ghana has steadily risen over the last decade, the modern Contraceptive Prevalence Rate (mCPR) among all women reached only 22% in 2019 with 30% of women still reporting unmet need. To increase FP uptake via mitigation of cost barriers among women with unmet need, the Government of Ghana is seeking to integrate claims-based FP services into the National Health Insurance Scheme benefits package. This program brief documents lessons for social and behavior change programmers implementing multi-health, community-based programs with integrated health services. An iterative process by Breakthrough ACTION Zambia, from October 2018 through March 2019, identified four design concepts with strong feasibility and potential for impact and expansion. These four concepts were refined in Breakthrough ACTION Zambia provinces and districts in collaboration with implementing partners and the Ministry of Health Department of Health Promotion, with final intervention design and implementation processes determined in close collaboration with the Ministry.

The Population Council has a longstanding partnership with indigenous communities in rural Guatemala through the Abriendo Oportunidades program. In compliance with COVID-19 prevention protocols, as of mid-August 2020 government restrictions allowed for inter-district mobility at the national level, classes remain suspended, and businesses were allowed to operate. To better understand indigenous knowledge, perspectives, and evolution of needs related to the COVID-19 pandemic in Guatemala, we conducted a second round of phone interviews with 144 key informants in ten municipalities. Many women in the developing world already rely on private sector providers for various types of treatment, as well as for family planning (FP) services, due to ease of access, shorter waiting times, and a perception of better service. Drug shops and pharmacies are also important for improving access and basic information about contraceptive products. Private pharmacies and shops are key when public sector facilities face frequent shortages and stock outs.

Pakistan's family planning (FP) program remains more or less exclusively aimed at women, yet evidence from both international as well as programming from within Pakistan suggests that including men in FP could improve results. Given the large youth cohort entering reproductive life in Pakistan, there are compelling grounds for also including younger men and adolescent boys, especially those about to wed, within the country's reproductive health (RH) and FP strategies.

Additional financing for family planning (FP) in Pakistan is strongly justified, not only to increase contraceptive prevalence, reduce unmet FP need, and avert millions of unwanted pregnancies, but also to reduce maternal mortality and effect improvements in other spheres of development. As a safeguard against unintended fertility, FP contributes to national savings in multiple ways, and additional annual funding of $93 million—or 40 cents per capita—is a relatively minor investment that promises huge dividends.

Integration of trained, equipped, and supported community health workers (CHWs) within a health system is recognized as a proven high impact practice for family planning (FP) services, with evidence from around the world confirming the need to complement facility-based FP services with CHW programs, particularly in areas where people’s access to health care is limited. Although sexual violence against children is increasingly prevalent in Kenya, documented and tested interventions for proactively identifying child survivors and providing them with care are currently nonexistent. This study aimed to determine the feasibility of screening children for SV in both school and health facility settings, and of providing child survivors identified through this process with access to care. The four-component intervention, implemented from January through April 2017, involved parent dialogues; student sensitization; provider training to administer a child-friendly screening tool and respond to child survivors; and SV screening, paired with SV service provision. In Haiti, where an estimated 3,000 children and youth are receiving antiretroviral therapy (ART), Caris Foundation, with funding from the United States President’s Emergency Plan for AIDS Relief (PEPFAR) through the United States Agency for International Development (USAID), provides hospital-linked psychosocial support groups, known as Kids Clubs, for young people ages nine and older. This brief summarizes the results of a behavioral diagnosis that reveals “Facility-based providers do not consistently follow best practices for the detection and management of complications during childbirth.” Low perception of risk of postpartum hemorrhage, limited review of compliance with best practices, consequences of current practices, and resource scarcity can negatively affect provider decision-making and clinical practice.

This brief presents responses from the first round of data collection among adults, adolescents, and community leaders in indigenous municipalities of Chiapas and Yucatan, Mexico, June 1–30, 2020. In total, 295 people from 17 municipalities were surveyed in Yucatan, and 83 people from 15 municipalities were surveyed in Chiapas. Adults and adolescents in both states identified the groups most at risk of suffering complications. Food insecurity may be a consequence of the pandemic. Regarding livelihoods, 51% of women and 63% of men in Yucatan have lost their work and/or their primary source of income since the start of the pandemic, …

The Sustainable Development Goals target the elimination of all forms of harmful practices, including female genital mutilation/cutting (FGM/C) by 2030. Kenya has made progress in addressing the elimination of FGM/C through ratification of global instruments and enactment of the Prohibition of FGM Act in 2011 and other supportive laws. The Act created the foundation for the establishment of the Anti-FGM Board, which provides policy leadership and coordination of anti-FGM/C programs in Kenya. The containment measures implemented by the national and state governments, particularly the nationwide lockdown, have helped keep the COVID-19 pandemic at a relatively low level in India. Many migrants from metropolitan cities in India began to return to their native homes with the rise in COVID-19 cases in the cities and the extension of the lockdown. With relaxation of travel restrictions, Bihar started receiving migrants who were returning home. The government made arrangements for them to be quarantined before they traveled to their villages and homes. The key objectives of this study are to understand the extent of return migration, the impact of lockdown on migrant household livelihoods, and the opportunities for household economic revival through social protection schemes.
