Historically, only men went to war. The winners took the losers' women and could rape, enslave, or kill them. As we moved into a more modern civilization, the rights of the winner over the women of the loser became less clear and were only vaguely documented. Even for modern war historians, what happens to the men on and off the battlefield is the main preoccupation. After all, women do not face combat and what happens to them seems unimportant. A critical review of current epidemiological trends and social science research demonstrates that there is an urgent need for expanding the range of female-controlled HIV prevention methods. Existing efforts to control the spread of HIV infection primarily through the encouragement of a reduction in the number of sexual partners, widespread condom promotion, and the control of other sexually transmitted infections are inadequate for many of the world's women. This issue of Quality/Calidad/Qualité describes the experience of one adolescent sexuality program, Gente Joven in Mexico. Gente Joven began by acknowledging reality: that the problems related to teenage sexuality will not go away simply by denying their existence. Along the way Gente Joven learned, and is continuing to learn, many lessons about how to build a successful program for adolescents. Here they share their experiences and the Afterword explores in greater depth one particular issue that Gente Joven has identified as its next major challenge: re-examining their work with young people from a gender perspective.

Finding Hiv-Infected Women - The Clinician's Role, Mary Guinan

An estimated 100,000 women are currently infected with the human immunodeficiency virus (HIV) in the United States, and a great majority of them are unaware of their condition. Approximately 20,000 HIV-infected women were identified through publicly funded HIV screening programs in 1989 and 1990, and an unknown number through private screening. Because most HIV-infected women are believed to be in the lower socioeconomic strata, it is unlikely that a significant number were identified in the private sector. Therefore, up to 80% of HIV-positive women may not know they are infected.

The studies in this issue of Quality/Calidad/Qualité describe three efforts to involve women in the development of reproductive health care materials in different settings in the developing world. In Cairo, a women's health collective produced a comprehensive reference book for women; in Peru, a woman's group, with the extensive involvement of their nonliterate audience, developed a series of illustrated teaching materials; and in the South Pacific, an all-woman production crew produced three motivation and teaching videos developed in response to the expressed needs of Pacific Island women. After five years of national programmatic use of Norplant® contraceptive implants, in 1992 the National Population and Family Planning Board (BKKBN) plans to implement a follow-up Use-Dynamics Study of implant users in West Java and West Sumatra Provinces, Indonesia. A Diagnostic Study was implemented in 1991 to prepare for that study prior to final approval, and to get some early insights into implant program operations in advance of final results from the main study. As of February 1991, more than 16,000 cases of AIDS in women had been reported in the United States, but the ". tidal wave of cases in women is yet to come. World Health Organization (WHO) estimates of the number of women infected with the human immunodeficiency virus (HIV) range from 1.5 million in Africa to I 00,000 in the United States. Within the next ten years, the majority of these women will develop AIDS and die. Some will live a bit longer, but all are eventually doomed to die prematurely. Most will not live to see their 40th year.

The health-promoting behaviors, perceived health status, and perceived health education needs of older women were identified in this non-experimental, descriptive, correlational study. Fifty women over 60 years of age, living in three housing complexes for the elderly, disabled and handicapped in two southeastern United States cities were asked to complete The Health-Promoting Lifestyle Profile, The Perceived Health Status Questionnaire, The Health Education Needs Questionnaire, and Demographic Data Sheet. On December 26, 1990, Nancy Cruzan died, 12 days after her feeding tube was removed. She had been in an irreversible coma for seven years and her parents had sued for the right to remove the feeding tube that was keeping her alive. The pain and suffering of her family in coming to this decision was certainly compounded by those who disagreed with them and tried to prevent the extubation. Whether one agrees with this particular decision or not, most of us will not be faced with such clear alternatives, ie, leave the tube in or take it out in …

In a prospective study we compared two different dosage regimens of IV chloramphenicol succinate (100 mg/kg/day and 75 mg/kg/day) in children with culture proven typhoid. Trough and peak blood samples, obtained at 48 hrs, were analysed for free chloramphenicol by high pressure liquid chromatography (HPLC). Although the mean trough (8.8 +/- 7.7 versus 5.4 +/- 2.6 mcg/ml) and peak (19.9 +/- 12.2 versus 15.4 +/- 6.1 mcg/ml) chloramphenicol concentrations were comparable in both groups, a significantly wider range was found in the group receiving 100 mg/kg/day. Potentially toxic levels (greater than 30 mcg/ml) developed in two patients with liver dysfunction. …

The purpose of this study was to investigate the differences in physiological and subjective perceived exertion levels at maximal and submaximal treadmill exercise between three phases of the menstrual cycle in eumennorheic women.

Nine females of varying fitness levels were subjects in the study. None of the subjects were using oral contraception or hormone altering medications at the time of the study.

The subjects performed three separate maximal graded exercise tests (GXT) using the Balke protocol. During each GXT, oxygen uptake (VO₂), heart rate (HR), blood pressure (BP), respiratory quotient (RQ), rate of perceived exertion, and total time …

Infants born to adolescent and young adult mothers who abuse substances, are at high risk for chemical dependency and its sequelae. There is a need to identify such infants and their mothers so that intervention will be available to them. The purpose of this study was to develop a systematic approach to evaluate mothers and infants for potential drug use and improve the quality of the nurses decisions.
