A 62-year-old male farmer was killed inside his barn on a Saturday afternoon after starting his tractor from a standing position. The victim was demonstrating recent repairs to the tractor for his brother-in-law; a new battery and starter had been installed. Standing beside the tractor, the victim reached for the ignition switch to turn the tractor on. The tractor was in gear; when he turned the key the tractor lurched forward, running over the victim, who died moments later. The KY FACE investigators concluded that, to prevent similar occurrences, farmers and other tractor operators should:

Log Loader Runs Over Worker At Stave Mill, Kentucky Injury Prevention And Research Center

A 56 year-old-male stave edger was killed when he entered the path of a moving log loader at a saw mill. At 4:30 pm the day of the incident, the victim had completed an 8-hour shift in the mill. He walked out of the metal building toward the employee parking area. About 18 feet from the building he was struck by a loader, which was backing up. A witness and the loader driver ran to the victim and helped move him to some nearby logs. The victim was alive and conscious. He removed his ear plugs while waiting for the …

Because of the enormity of the HIV-AIDS epidemic and the urgency for preventing transmission, HIV prevention programs are a high priority for careful and timely evaluations. Information on program effectiveness and efficiency is needed for decision-making about future HIV prevention priorities. General characteristics of successful HIV prevention programs, programs empirically evaluated and found to change (or not change) high-risk behaviors or in need of further empirical study, and economic evaluations of certain programs are described and summarized with attention limited to programs that have a behavioral basis. HIV prevention programs have an impact on averting or reducing risk behaviors, particularly …

This issue of Quality/Calidad/Qualité tells the story of a group of Brazilian women who came together in 1981 with the shared belief—not as widely understood then as it is now—that conventional approaches to women's health and family planning services were overly medicalized, incapable of dealing with the relationship between sexuality and contraception, and disempowering to clients. They started the Coletivo which runs a clinic, a training institute, and a public education program that has “given voice” to thousands of women, provided leadership to the Brazilian government and women’s health movement, and has been active in research and international policy affairs. …

At the time of the incident, the victim was mowing a pasture along a creek bed. The tractor was not equipped with a Roll Over Protective Structure (ROPS) or a seat belt. Operating a farm tractor with a side sickle bar mower attached to the right side, he proceeded around the perimeter of the field. One edge of the field dropped off steeply to a ravine. While making the second pass around the field the ground under the left wheels gave way causing the tractor to roll over. The tractor came to rest on the victim causing massive internal injuries. At approximately 12:30 pm, the victim was about 1/4 mile from the main road mowing blackberry briars along a creek bed. This area was between the bean field and a creek embankment. A 38-year-old part-time farmer (landowner) was killed after the tractor he was operating turned over on him. The victim was mowing his property using a tractor and rotary mower attachment. The tricycle-type tractor was not equipped with a rollover protective structure (ROPS) or a seat belt. After completing an upper field, he drove the tractor down a hill toward the public roadway. A 48-year-old male tree trimmer (the victim) died from a fall while trimming a tree from a bucket truck. The victim was the owner/operator of a tree trimming service. He operated the business for about 23 years, employing various part time helpers. On the day of the incident, two sons-in-law were operating chain saws at the scene when they heard a loud "pop." As they looked up, the witnesses saw the bucket carrying the victim slam against the truck. The sons-in-law removed the victim from the bucket. He was not breathing. The witness and helper were unloading 30' sections of 4" irrigation pipe from a flat bed wagon at a tobacco field. The procedure was to unload and connect the pipe in the field, then pump water from a creek approximately 300' away. As the operation began, the victim parked the tractor and wagon along the edge of the tobacco field and remained seated on the tractor. The victim, along with a neighbor (the witness), was loading the neighbor's tractor on a flatbed trailer to transport it to the victim's tobacco field for plowing. The trailer, attached to the pick-up truck, was parked on a gradual downward slope in preparation for tractor loading. The victim was standing on the front of the trailer while the tractor was being driven onto the trailer by the witness.
