On 22 November 1995, a 52-year-old farmer died when he was caught in the corn picker he was operating. He was a full-time farmer and had been involved in the family farming business all of his life. On the day of the incident, the victim and his 20-year-old son had been working in the field together operating the corn picker. After obtaining a full load, the son transported the load back for storage while the farmer stayed in the field to finish a small section of corn. A 32-year-old male was killed when he was caught in a rotating power take-off (PTO) shaft. He had been involved in farming for many years and often helped his friends who owned a dairy farm. The farm worker was helping one of the owners of the farm load corn from the silage wagon into the feeding trough. Both the farm worker and the owner were experienced in operating the equipment on the farm and had worked together on this type of task before. Cumulative trauma disorders are musculoskeletal problems such as carpal tunnel syndrome, rotator cuff tendinitis, and epicondylitis which can affect assembly line workers. The injuries are thought to result from repetitive motions performed under excessive loads in anomalous postures. The project aimed to design, build, install, and evaluate worksite accommodation at the Saturn plant for a worker with right lateral epicondylitis. The goal was to make her fully functional in her original team which required addressing the injurious aspects of the task that caused or aggravated her injury.

Chaque année, près de 1 adulte sur 1, 000 résidant à Montréal, subira une intervention chirurgicale pour le syndrome du canal carpien (SCC), ce qui représente aux alentours de 1, 000 personnes. Le ratio femme: homme est de trois pour un et l'incidence chez les femmes de plus de 45 ans est plus du double de la moyenne. Les personnes opérées pour le SCC rapportent des symptômes typiques du SCC, aux mains et aux poignets, qui évoluent depuis 2 ans en moyenne au moment de l'opération. This study examined the effect of the implementation of a risk management program tool on worker safety knowledge in a composite building material fabrication facility whose workers suffered from an increasingly high injury (incidence) rate in recent years. Risk assessment codes (RACs) and job safety analyses (JSAs) were the primary tools used to introduce workers to the concept of risk. Because employee involvement is a crucial element in JSA development. employee input was frequently solicited throughout JSA development.

Industrial workers who have sustained hearing losses often wear hearing aids on the job in order to hear their co-worker's speech. However they risk damaging their hearing further by amplifying the high levels of background noise typical of such environments. The Occupational Safety and Health Administration (OSHA) has established guidelines to protect workers' hearing. A maximum allowable level of 90 dBA averaged over a period of eight hours is considered safe. Wearing hearing aids on the job may expose an individual to considerably higher levels, however no guidelines as to maximum allowable levels of amplified noise exist at this time. …

A 52-year-old male logger was killed when he was struck by a log rolling off of his truck during the unloading process. The victim was a self-employed logger who had at least 25 years of experience. He was using the family-owned truck to haul a load of logs to the mill. The vertical stakes on the flatbed truck were approximately three feet high with the logs stacked another 3-4 feet above the stakes. A 21-year-old farm worker was killed in a tractor rollover incident. The victim was driving a tractor on a secondary public road, pulling an unloaded 18-foot flatbed trailer, when he lost control. After the tractor wheels went off the right side of the road, the victim overcorrected, which caused the vehicle to become unbalanced; he was thrown from the tractor as it overturned into a ditch. It was not equipped with an appropriate rollover protective structure (ROPS) or a seatbelt. A 22-year-old logger was killed after being struck on the head by a falling snag (dead standing tree). The victim was working with one co-worker, who was selectively cutting trees which were approximately 22 inches in diameter. Standing approximately three feet from the tree being cut, the victim was not wearing a hard hat. The cut tree broke a snag about 35 feet up, causing the snag to fall and strike the victim on the head. A 41-year-old self-employed logger was killed when the endloader he was driving down a steep mountain road rolled over. Recent rainfall had made the dirt road wet and muddy. Tire marks left in the mud indicated that as the vehicle began to slide downhill, it hit the right embankment, causing it to veer to the left side of the road and roll over several times before coming to rest at the bottom of the dropoff on its left side. A 32-year-old farmer was killed when he was crushed in a hay baler. The victim was operating a tractor, towing a round baler in an open field with gradually sloping terrain. Although the victim had been doing farm work since he was young and had experience operating square balers, his experience with this particular round baler was limited. The victim was found caught in the baler headfirst up to his waist. There were no witnesses to the incident. The victim was pronounced dead at the scene.
