Gastrocolic Fistulas Due To Cancer: Report Of A 16-Year Survival And A Review Of The Literature, Susan Adelman, Joseph L. Ponka

A case of 16-year survival following resection of a malignant gastrocolic fistula is reviewed. The patient's post-gactrectomy anemia is discussed. The world literature of 92 cases of malignant gastrocolic fistulas reported since 1925 is reviewed.

Prevention Of Recurrent Systemic Embolism With Anticoagulants In Patients With Rheumatic Heart Disease, Sudarsan Misra, Irwin J. Schatz

Henry Ford Hospital Medical Journal

Results are reported of long-term anticoagulant treatment in a group of 90 patients with rheumatic heart disease who had systemic embolism. Compared to the reported incidence of recurrent systemic embolism in patients not given anticoagulant drugs, such therapy remarkably lowers the risk of such occurrences. Complications from this treatment in the reported patients were minimal.

Use Of A Cooling Jacket During Kidney Transplantation, Stanley G. Dienst, Mohammed R. Ansari

The model of renal autotransplantation, with immediate contralateral nephrectomy in the dog, is used to demonstrate that continuous cooling by means of a cooling jacket during vascular anastomoses is effective in preserving renal function acutely and over a two-month follow-up period.

This review of pre- and post-operative management of six cases of small bowel perforation secondary to blunt abdominal trauma provides the opportunity to analyze the problems involved in prompt diagnosis and adequate care. Some of the possible mechanisms producing the perforation are discussed. In order to prevent undue delays in diagnosis, changes in clinical picture should warrant early surgical exploration. This avoids increased rates of mortality and prolonged morbidity.

Newer diagnostic techniques, including radioisotope liver scans, bacteriologic culture procedures for anaerobes, serologic tests for amebiasis, ultrasonic echograms, and hepatic arteriography, permit more specific identification and localization of hepatic abscesses. Large pyogenic abscesses are of particular Importance to surgeons; are frequently caused by anaerobic organisms; and require early, direct, adequate, and sufficiently prolonged surgical drainage. The clinician should be alert also to recognition and management of the primary lesion responsible for the hepatic abscess, associated abscesses elsewhere, and the possible presence of an underlying liver neoplasm.

Effects Of An Eeg "Seizure Discharge" On Operant Responding, Richard M. Lee, Julia A. Lee, Hendrik Van Den Ende, Lorne D. Proctor

A post-stimulus "seizure discharge" recorded in the amygdaloid area of monkey brains was investigated primarily by studying its effects upon behavior. Three different operant conditioning schedules were used with animals implanted with multiple depth electrodes. The schedules involved the factors of number discrimination, attention, sequential responding, and self-stimulation. Although the electrical brain stimulation caused a disruption in responding, no behavioral effects could be attributed to the recorded discharge itself. This result, and previous findings, lead to the tentative conclusion that the discharge results from a physico-chemical, brain-electrode interface phenomena and is not neurophysiological in nature.

Dissecting Aneurysm Of The Aorta: Diagnosis, Treatment And Prognosis, Hahn J. Lee, Ellet H. Drake

The records of 13 patients with proven dissecting aortic aneurysm were studied with particular attention to the diagnosis, treatment, and prognosis in each case. The three essential steps in reaching an early correct diagnosis included (1) history and physical examination, (2) serial chest x-rays and (3) aortography. Electrocardiogram and SCOT and LDH measurements seem to be of value only when incompatible with the diagnosis of acute myocardial infarction. Significant differences between Type 1 and Type 3 dissections were observed in their clinical manifestations, results of treatment and natural prognosis.

Rubella Immunization Of Adult Females Using Hpv-77 Dk-12 Live Attenuated Rubella Virus, Donald A. Romig, E. L. Quinn, Frank Cox, Robert G. Brackett

This study demonstrates the serologic response as measured by the HAl test and the side reactions of the HPV-77 DK-12 live rubella vaccine in a small group of adult females. One hundred percent seroconversion was obtained using this vaccine. The mean titers obtained in two separate time periods post-vaccination are higher than those reported for several other rubella virus vaccines. A 66% occurrence of joint symptomatology was recorded post-vaccination with a mean duration of 11.6 days; 24% of women who received placebo reported joint complaints which had a mean duration of 2.0 days. Observations On Uric Acid Transport In Man, The Dalmatian And The Non-Dalmatian Dog, Howard Duncan, April S. Curtis

The urinary output of uric acid from the purebred Dalmatian is similar in amount to that in man. Also, this breed of dog has a higher plasma uric acid level than other dogs and, like man, this hyperuricemia is accompanied more frequently with renal and bladder lithiasis. Allopurinol is effective therapy in both man and dog. Study of the fate of uric acid in the Dalmatian shows that the liver does not oxidize the available uric acid completely although it is capable of doing so when liver homogenates are studied. Atrial flutter sounds are occasionally audible on bedside examination especially in patients with slow ventricular rates. A patient is described with audible flutter sounds and an illustrative phonocardiogram is presented. The value of careful auscultation in patients with atrial flutter is re-confirmed.

Gastrocolic And Duodenocolic Fistulas: An Experimental Study, Joseph L. Ponka, A. Karim Shaalan, P. C. Pesaros

In an experimental study of gastrocolic and duodenocolic fistulas, it was clearly seen in 10 animals that duodenocolic fistulas result in the most serious loss of nutrition. Death from starvation occurred in all 42 animals in 30 days. Gastrocolic fistulas similarly resulted in weight loss and death of the animals in an average of 42 days. Lengthy fistulas between the stomach and colon caused no obvious disturbances in the physiology of the gastrointestinal tract. The significant microscopic changes included destruction of the mucous membrane and a decrease in size of the villi. The other noteworthy observation was that fistulous aggregations …

Mycosis fungoides is best viewed as a cutaneous lymphoma. Over a period of years, it progresses through three stages—the first consisting of red scaly patches, the second of pruritic plaques, and the third of brownish-red tumors. The therapy of each stage is outlined, and several selected modalities are discussed in detail, including topical steroids, systemic steroids, localized superficial irradiation, topical nitrogen mustard, electron beam, and chemotherapy (alkylating agents and antimetabolites.

The various methods for diagnosis of gonorrhea are reviewed. Gram-stained smear of urethral exudate in the mate is appropriate, but for the female emphasis is placed on the use of culture on Thayer-Martin medium. Currently effective modalities of treatment are detailed, pointing out that higher doses of antibiotics are generally required for females with pelvic inflammatory disease than for males with acute gonococcal urethritis. The necessity for follow-up is also discussed.

An Improved Method Of Staining Paraffin-Embedded Bone Marrow Sections, Wanda Pusey, A. R. Villanueva

Henry Ford Hospital Medical Journal

Cytological detail is well demonstrated in an improved method of staining aspirated bone marrow sections embedded in paraffin. The tinctorial patterns of the stain resemble some of its predecessors; thus, cell counting and identification involve little difficulty. The method has proven reliable, reproducible and a valuable addition in the diagnosis of bone marrow disorders. It is a useful tool in research problems in which objectives are the study of the bone marrow.

The Clinical Course Of Duodenal Ulcer Associated With Hyperparathyroidism, Melvin A. Block, Boy Frame, Thomas A. Fox Jr.

In 98 patients with primary hyperparathyroidism, 15 had objective evidence of duodenal ulcer disease. Following operative correction of the hyperparathyroidism, improvement of duodenal ulcer disease occurred usually, but not uniformly, if unassociated with non-beta islet cell tumors. Correction of hyperparathyroidism in patients with the Zollinger-Ellison syndrome does not significantly influence the peptic ulcer which should be given primary aggressive surgical treatment.
