Healthy amounts of sleep is vital for normal human functions such as daily learning, memory, emotional state and cardiovascular function. Studies conducted previously have shown sleep deprivation to be associated with increases in sympathetic activity contributing to autonomic nervous system (ANS) dysregulation. HRV biofeedback (HRVB) training induces HRV coherence, a condition that maximizes HRV and facilitates autonomic and cardiorespiratory homeostasis. This randomized, controlled, intervention trial will test the hypothesis that HRVB can improve HRV coherence and increase overall sleep quality. OBJECTIVE: This study examined the relationship between cardiorespiratory fitness (CRF) and age to determine if the decrease in CRF associated with advancing age is greater among those with type 2 diabetes mellitus (T2DM) as compared to those without T2DM. This study also assessed if differences in CRF between persons with T2DM and those without are consistent across CRF percentiles.

The purpose of this dissertation project is to add to the growing literature about the multi-faceted aspects of colorectal cancer (CRC) prevention and related disparities. We focused on the spatial distribution of facilities performing screening services to identify areas that underutilize colonoscopy screening. Next, we examined how the food and physical activity environment affects precursors of CRC by considering plausible pathways. Finally, we explored if access to health insurance reduced the racial disparity of receiving a timely surveillance colonoscopy after a CRC diagnosis.

Over-modulation of the sympathetic nervous system and reduced heart rate variability (HRV) are commonly overlooked components of pain, poor cognition (decreased attention, recall, and cognitive processing), depression, stress, and fatigue. HRV Biofeedback (HRVB) training induces HRV coherence to balance the autonomic system. Paced breathing (~6 breaths/minute) increases HRV coherence. This randomized, controlled intervention trial tested the hypothesis that HRVB would improve HRV coherence, pain (severity, interference, and catastrophizing), cognitive performance, and reduce depressive, stress, and fatigue symptoms and pain medication use in veterans. Despite strong evidence for the use of statins for patients with atherosclerotic cardiovascular disease (ASCVD), statin prescription is still suboptimal. We aimed to determine the rates and factors that influence statin prescription using national survey data. This is a cross-sectional retrospective study on 8,468 patients with clinical ASCVD who were drawn from the National Ambulatory Medical Care Survey and the National Hospital Ambulatory Medical Care Survey from years 2011 to 2015. Introduction: The burden of tick-borne disease (TBD) in Maine has steadily increased since the first case of Lyme disease was reported in the late 1980s. The emergence of five different agents of TBD in Maine has been challenging and confusing for clinicians and the public.

Methods: We reviewed the ecology of emerging of tick -borne disease, then reviewed risk factors for tick bites and tick-borne disease in Maine. We then compared the burden of TBD versus community-acquired comparison infections in terms of hospitalizations, deaths, and media attention.

The objective of this study was to examine whether there was a gender difference in the occurrence of hypertension in Indonesian people, after controlling for individual and environment factors. Data were ob tained from the Indonesia Basic Health Research 2007 and a cross sectional design was used. Samples consisted of 13.262 men and women age 15 years or more. A multilevel logistic regression was used to ana lyze the data. There was a statistically significant association between gender and hypertension. Women were less likely to have hypertension than men (OR 0.86 P value < 0.0033). There was an interaction between variable of gender and variable of age. It suggested that the probability of having hypertension in women and men was varied by the strata of age. In the strata of age more than 60 years, women were more likely to have hypertension than men (OR 1.25, P value 0.0065); in strata of age 30-59 years, there was no difference of having hypertension between women and men (P value > 0.05); in strata of age <30 years, women were less likely to have hypertension than men (OR 0.67 Pvalue 0.0000). Among women, there was an increased of having hypertension with age. Age of e” 60 years was the highest risk of having hypertension. Among men, there was an increased of having hypertension with age, but at in the age of e” 60 years the increased risk of having hypertension was not as high as in women.

Determinan Yang Meningkatkan Risiko Terinfeksi Hiv Pada Wargabinaan Lembaga Pemasyarakatan Dan Rumah Tahanan Di Indonesia: Studi Tinjauan Pustaka 2007-2017, Mondastri Korib Sudaryo

Wargabinaan pemasyarakatan (WBP) diduga berisiko terinfeksi HIV selama tinggal di lembaga pemasyarakatan (lapas) dan rumah tahanan (rutan) akibat menyuntik narkotika dan perilaku berisiko lainnya. Studi ini bertujuan mengetahui determinan yang meningkatkan faktor risiko terinfeksi HIV di kalangan WBP di lapas/rutan di Indonesia. Rancangan studi adalah tinjauan pustaka sistematik (systematic literature review) yang merangkum dan membandingkan hasil studi dari 12 artikel yang diterbitkan sejak 2007 2017 di jurnal nasional/internasional berbahasa Inggris/Indonesia mengenai karakteristik dan determinan infeksi HIV pada populasi WBP yang tinggal di lapas/rutan di Indonesia. Karakteristik demografis WBP: umumnya laki laki; usia rata-rata 30 tahun (rentang: 21-51 tahun); sebagian besar <=SMU; setengahnya menikah. Prevalensi HIV di kalangan WBP di beberapa lapas: 10%-56%. Tiga survei nasional tahun 2011, 2013, 2015 menunjukkanprevalence rate antara 1,2% – 3%. Proporsi perilaku berisiko: menyuntik narkotika (antara 5% - 66%), berbagi jarum suntik (28%-47%), kontak seksual tidak aman dengan sesama WBP (2%-13%), atau dengan WPS (96%), pakai tattoo (2%-56%), atau tindik (5%-13%), minum alkohol (47%). Dari 3 survey yang sama tingkat prevalensi Sifilis antara 2,1%– 5%. Determinan/faktor risiko terinfeksi HIV: menggunakan narkotika suntik (OR antara 11,4 - 104,8), pakai tatoo (OR: 1,7 -2,4), pakai tindik (OR = 4,4), seks tidak aman (OR=1,9), terinfeksi Sifilis (OR=4,2). Faktor-faktor penting yang meningkatkan risiko terinfeksi HIV di kalangan WBP di lapas/rutan adalah penggunaan narkotika suntik, berbagi jarum suntik tidak steril, kontak seksual dengan sesama WBP &/ WPS, terinfeksi Sifilis dan memakai tato/tindik.

Angka Kematian Ibu (AKI) merupakan indikator kesejahteraan masyarakat suatu negara. Sasaran RPJMN 2015-2019 di Indonesia AKI ditargetkan menjadi 306 pada tahun 2019. AKI di Kabupaten Serang tahun 2017 adalah 195/100.000 KH, atau 58 kasus dengan jumlah kelahiran hidup sebanyak 29.787 jiwa. Dari 58 kasus kematian ibu, 14 orang persalinan ditolong dukun (24,14%). Inflammation and antioxidant capacity have been associated with colorectal and breast cancer. We computed the dietary inflammatory index (DII®), and the total dietary non-enzymatic antioxidant capacity (NEAC) and associated them with colorectal and breast cancer risk in the population-based multi case-control study in Spain (MCC-Spain). We included 1852 colorectal cancer and 1567 breast cancer cases, and 3447 and 1486 population controls, respectively. DII score and NEAC were derived using data from a semi-quantitative validated food frequency questionnaire. Despite existing reports on differential DNA methylation in type 2 diabetes (T2D) and obesity, our understanding of its functional relevance remains limited. Here we show the effect of differential methylation in the early phases of T2D pathology by a blood-based epigenome-wide association study of 4808 non-diabetic Europeans in the discovery phase and 11,750 individuals in the replication. We identify CpGs in LETM1, RBM20, IRS2, MAN2A2 and the 1q25.3 region associated with fasting insulin, and in FCRL6, SLAMF1, APOBEC3H and the 15q26.1 region with fasting glucose. The number of people living with HIV (PLWH) ≥65 years old is increasing in the United States (US) as PLWH live longer. In 2015, there were nearly 1 million people living with diagnosed HIV in the US and under 10% were age ≥65. By 2035, the proportion of PLWH in this age group is projected to be 27%. Periodontitis is a chronic inflammation that destroys periodontal tissues caused by the accumulation of bacterial biofilms that can be affected by environmental factors. This report describes an association study to evaluate the relationship of environmental factors to the expression of periodontitis using the National Health and Nutrition Examination Study (NHANES) from 1999–2004. A wide range of environmental variables (156) were assessed in patients categorized for periodontitis (n = 8884). Multiple statistical approaches were used to explore this dataset and identify environmental variable patterns that enhanced or lowered the prevalence of periodontitis. OBJECTIVE: Early psychosis is an important window for establishing long-term trajectories. Involuntary hospitalization during this period may impact subsequent service engagement in people with newly diagnosed psychotic disorder. However, population-based studies of involuntary hospitalization in early psychosis are lacking. We sought to estimate the proportion of people aged 16 to 35 years with early psychosis in Ontario who are hospitalized involuntarily at first admission, and to identify the associated risk factors and outcomes.

Background: Socioeconomic status is associated with differences in risk factors for cardiovascular disease incidence and outcomes, including mortality. However, it is unclear whether the associations between cardiovascular disease and common measures of socioeconomic status-wealth and education-differ among high-income, middle-income, and low-income countries, and, if so, why these differences exist. We explored the association between education and household wealth and cardiovascular disease and mortality to assess which marker is the stronger predictor of outcomes, and examined whether any differences in cardiovascular disease by socioeconomic status parallel differences in risk factor levels or differences in management.
