Analisis Survival: Hubungan Konversi Sputum Dengan Keberhasilan Pengobatan Pasien Tuberkulosis Resistan Obat Di Indonesia, Shena Masyita Deviernur, Nurhayati Adnan

Angka keberhasilan pengobatan Tuberkulosis Resistan Obat (TB RO) di Indonesia masih rendah. Konversi sputum dapat menjadi monitoring awal untuk memantau keberhasilan pengobatan. Penelitian ini bertujuan untuk melihat hubungan antara konversi sputum <3 bulan dengan keberhasilan pengobatan. Penelitian ini menggunakan desain studi kohort retrospektif dengan data yang bersumber dari sistem informasi tuberkulosis Kementerian Kesehatan RI. Populasi pada penelitian ini adalah pasien TB RO yang memulai pengobatan tahun 2020-2021 di Indonesia dengan jumlah sampel sebanyak 8.025 kasus dan participation rate sebesar 88,6%. Kurva Kaplan Meier dengan uji log-rank dibuat untuk analisis deskriptif, sedangkan analisis regresi Cox digunakan untuk melihat hubungan antara konversi sputum dengan keberhasilan pengobatan. Hasil analisis menunjukkan bahwa 45,9% pasien konversi sputum < 3 bulan pengobatan dengan 74,6% memiliki keberhasilan pengobatan. Secara keseluruhan median time untuk keberhasilan pengobatan berdasarkan konversi sputum < 3 bulan adalah bulan ke-18. Hasil analisis multivariat menunjukkan konversi sputum < 3 bulan 1,25 (95% CI 1,18-1,34) lebih tinggi peluangnya untuk mencapai keberhasilan pengobatan dibandingkan pasien yang tidak konversi < 3 bulan setelah dikontrol oleh status HIV dan paduan pengobatan. Berdasarkan hasil penelitian ini dapat disimpulkan bahwa monitoring konversi sputum dapat menjadi cara untuk melakukan monitoring keberhasilan pengobatan sedini mungkin. Konversi sputum merupakan standar yang dilakukan untuk pemantauan pengobatan pasien TB RO. Namun jumlah laboratorium yang dapat melakukan pemeriksaan untuk konversi pada pasien TB RO masih terbatas. Perlu adanya perluasan layanan dan peningkatan mutu layanan untuk melakukan pemeriksaan konversi sputum pada pasien TB RO.

Prevalensi global stroke dan sindrom metabolik, sebagai kumpulan faktor risiko utama yang mempengaruhi stroke terus mengalami peningkatan. Penelitian ini bertujuan melihat hubungan antara sindrom metabolik dan kejadian stroke di Indonesia. Desain studi ini adalah cross-sectional dan menggunakan data survei skala nasional. Responden berusia >15 tahun yang mengalami stroke dengan data lengkap terkait pengukuran lingkar perut, tekanan darah, kadar glukosa darah, trigliserida, dan kadar high-density lipoprotein akan masuk dalam studi ini. Model regresi logistik digunakan dalam analisis kami. Dari 24.451 responden, studi kami menunjukkan mayoritas berusia <55 tahun (73,9%), perempuan (62,2%), dengan proporsi penderita stroke serta proporsi sindrom metabolik adalah 1,2%, dan 24,4%. Studi ini memperlihatkan adanya hubungan antara sindrom metabolik dan kejadian stroke (adj POR: 2,4; 95% CI: 1,9-3,1). Lockdown measures enacted in 2020 to control the spread of COVID-19 led to increases in the prevalence of mental health problems. Due to their high-risk status, individuals with chronic diseases may be at increased risk and disproportionately adversely affected by the COVID-19 pandemic. The investigators examined associations between having a high-risk chronic condition, social connectedness, and general distress and COVID-19-specific distress among U.S. adults during the COVID-19 lockdown. Parasomnias, such as sleepwalking and nightmare disorder, are abnormal behaviors and emotions emanating from sleep or wake to sleep transitions. Parasomnias can lead to significant adverse outcomes – sleepwalking can result in injuries and hospitalizations, frequent nightmares are associated with self-harm and suicidal thoughts, and rapid-eye movement (REM) sleep behavior disorder (RBD) is linked to neurodegeneration. Background: Oncotype DX assay, a multigene molecular test, has been widely used to stratify relapse risk and guide chemotherapy treatment in breast cancer. However, the optimal threshold of the Oncotype DX score in predicting chemotherapy benefit and its racial variation has not been investigated.

Methods: In this study, we apply a random forest survival model to the SEER-Oncotype cohort data (Surveillance, Epidemiology, and End Results with Oncotype DX test information for breast cancer patients) and determine chemotherapy benefit thresholds in early-stage, estrogen-receptor-positive (ER+), and HER2-negative (HER2-) patients of different races.

The USA is the third-leading cotton-producing country worldwide and cotton farming is common in the state of Georgia. Cotton harvest can be a significant contributor to airborne microbial exposures to farmers and nearby rural communities. The use of respirators or masks is one of the viable options for reducing organic dust and bioaerosol exposures among farmers. Unfortunately, the OSHA Respiratory Protection Standard (29 CFR Part 1910.134) does not apply to agricultural workplaces and the filtration efficiency of N95 respirators was never field-tested against airborne microorganisms and antibiotic resistance genes (ARGs) during cotton harvesting. Background: This hospital-based cross-sectional study aimed to examine the association between diabetes-specific quality of life (QoL), diabetes-related clinical characteristics and glycemic control among type 2 diabetes (T2DM) patients.

Methods: 335 T2DM patients receiving treatment at the diabetes clinic of a tertiary hospital in Bangkok, Thailand, were recruited into the study. The Audit of Diabetes-Dependent Quality of Life (ADDQoL19) was used to assess QoL. The ADDQoL scores were split into two groups using quartiles. The group in the lower quartile was considered as “having low QoL.” Hemoglobin A1c (A1C) was used as an index of glycemic control.

I consider two key areas in the growing field of human microbiome research: improving the quality of study reporting and the impact of antibiotics on participants in human gut microbiome research studies. In the first chapter, a team of evaluators used the Strengthening the Organization and Reporting of Microbiome Studies (STORMS) checklist to assess recently published microbiome literature. Background: The prevalence of Post-Covid Conditions (PCC) is estimated to affect 10-35% of infected individuals, with upwards of 85% of previously hospitalized individuals reporting prolonged effects. Literature indicates deficiencies in communication between healthcare providers which can negatively affect patients. Due to limited interprofessional communication, there was a scarcity of individuals with PCC receiving comprehensive therapy treatment on an outpatient basis. Differential methylation plays an important role in melanoma development and is associated with survival, progression and response to treatment. However, the mechanisms by which methylation promotes melanoma development are poorly understood. The traditional explanation of selective advantage provided by differential methylation postulates that hypermethylation of regulatory 5'-cytosine-phosphate-guanine-3' dinucleotides (CpGs) downregulates the expression of tumor suppressor genes and therefore promotes tumorigenesis. We believe that other (not necessarily alternative) explanations of the selective advantages of methylation are also possible. BACKGROUND: Mortality from cervical cancer has declined steadily in the United States over the past several decades due to widespread screening for precancerous and early-stage cervical cancer (ECC), which are significantly easier to treat compared with late-stage cervical cancer (LCC). Unequal screening access continues to cause significant racial/ethnic disparities in cervical cancer diagnosis stage. The kidneys operate at the interface of plasma and urine by clearing molecular waste products while retaining valuable solutes. Genetic studies of paired plasma and urine metabolomes may identify underlying processes. We conducted genome-wide studies of 1,916 plasma and urine metabolites and detected 1,299 significant associations. Associations with 40% of implicated metabolites would have been missed by studying plasma alone. We detected urine-specific findings that provide information about metabolite reabsorption in the kidney, such as aquaporin (AQP)-7-mediated glycerol transport, and different metabolomic footprints of kidney-expressed proteins in plasma and urine that are consistent with their localization and function, including the …

INTRODUCTION: The COVID-19 pandemic had significant impact on clinical care and clinical trial operations, but the impact on decentralized pragmatic trials is unclear. The Diuretic Comparison Project (DCP) is a Point-of Care (POC) pragmatic trial testing whether chlorthalidone is superior to hydrochlorothiazide in preventing major cardiovascular (CV) events and non-cancer death. DCP utilized telephone consent, data collection from the electronic health record and Medicare, forwent study visits, and limited provider commitment beyond usual care. Through the experiences gained by accelerating new vaccines for both Ebola virus infection and COVID-19 in a public health emergency, vaccine development has benefited from a 'multiple shots on goal' approach to new vaccine targets. This pilot study aimed to evaluate the effects of extremely hot ambient temperatures on the total number of fatal injuries. Data were collected from a population-based mortality registry of Thanh Hoa, a province in the North Central region of Vietnam.
