Relationship conflict for the psychiatric patient can have significant detrimental effects. There are specific types of interactions that can increase conflict and predict the potential for relapse; these have been identified by research and designated as components of Expressed Emotion (EE). Cognitive Behavioral Therapy (CBT) and Interpersonal Therapy (IPT) have been very effective when targeting specific psychiatric diagnoses, but less effective when addressing relationship conflict. The majority of studies addressing relationship conflict have taken place in an outpatient, long-term setting. In policy and practice, communities should implement support programs for rural veterans, and veteran families should educate themselves about the risk factors for suicide and support veterans in their families; policymakers should increase VA outreach programs in rural areas, as well as programs educating rural veterans about the importance of mental health treatment for those at risk for suicide. The American Psychiatric Association and academic psychiatry in the United States have two conflicts of interest that may affect their assessment of psychiatric drugs and their development of diagnostic and clinical care guidelines: payments from pharmaceutical companies and guild interests. Until recently, the proposed solution to industry-academic relationships has been transparency. However, cognitive dissonance research reveals that disclosure is not a solution because cognitive biases are commonplace and difficult to eradicate. What is the interaction between a psychiatric patient and their diagnosis? How do they respond to being classified? A number of philosophical theories attempt to explain the interaction between the diagnosed patient and their classification. Ian Hacking develops an account of interaction which holds that objects of human science classification are influenced by the awareness of the classification in a way that changes both the classification and its object. Hacking thinks that psychiatric patients are “interactive kinds” whose awareness of their classification causes changes in the individuals' experience of themselves, and thus changes in their classification. Borderline Personality Disorder (BPD) is a disabling condition characterized by chronic emotion dysregulation and behavioral impulsivity. Prospective studies that test proposed mechanisms of within-person change in BPD hold the key to improving symptom predictability and control in this disorder. A small body of evidence suggests that fluctuations in estradiol such as those occurring naturally at ovulation during the monthly female reproductive cycle may increase symptoms in women with BPD (DeSoto et al., 2003). Furthermore, there is preliminary evidence that both self-esteem and feelings of social rejection are highest at ovulation, when estradiol peaks (Durante and Hill, 2009; Eisenlohr-Moul et al., …

This brief is about the effect of the Seeking Safety program's services on improvement in PTSD, psychiatric symptoms, and social support among homeless female veterans. In policy and practice, clinicians should be trained on how to use the program to better serve homeless female veterans, and policymakers should push for clinician training for those who work with homeless veterans. Suggestions for future research include applying this study and the Seeking Safety program to non-VA healthcare systems, assessing substance use within the program, and assessing the long-term effects of the Seeking Safety program.

Objective: Although bipolar disorder has high heritability, the onset occurs during several decades of life, suggesting that social and environmental factors may have considerable influence on disease onset. This study examined the association between the age of onset and sunlight at the location of onset.

Method: Data were obtained from 2414 patients with a diagnosis of bipolar I disorder, according to DSM-IV criteria. Data were collected at 24 sites in 13 countries spanning latitudes 6.3 to 63.4 degrees from the equator, including data from both hemispheres. This study examines the relationship between psychiatric status and work impairment among OEF/OIF veterans enrolled in VA healthcare. In practice, mental health services are a high priority for the Veterans Administration, which is now focused on integrating behavioral health services into primary care so OEF/OIF veterans face fewer obstacles in obtaining and sustaining psychiatric care. In policy, policymakers should integrate veterans’ health programs with systematic assessment functioning, and develop treatment geared towards addressing impaired job performance. Background: Persons with a severe mental illness (SMI) prematurely lose up to 25 years of life when compared to the general population. This patient population has increased morbidity and mortality due to higher than normal rates of obesity, hypertension, diabetes, and cardiovascular disease. Treatment of SMI often includes the use of atypical antipsychotic (AA) medication which has been associated with the development of cardiometabolic illnesses. In response to the higher rates of co-morbid, chronic physical illness, monitoring guidelines for cardiometabolic illness have been published. Objective: To describe and characterise treated psychotic disorders in the Indigenous populations of Cape York and the Torres Strait.

Design: Cross-sectional analysis of patients with a psychotic disorder identified by treating psychiatrists.

Setting and participants: Indigenous patients aged 􏰀 15 years in Cape York and Torres Strait communities receiving treatment for a psychotic disorder over 3 months in 2010.

Main outcome measures: Prevalence of psychosis diagnoses, intellectual disability, and substance use comorbidities.

Results: 171 patients were included. The prevalence rate in this population was 1.68%, higher for males (2.60%) than females (0.89%), and twice as high in the Aboriginal (2.05%) …

Clinical practice guidelines are used increasingly across medical specialties and settings, making evaluation of their utility and validity a critical public health issue. In this paper, we describe some of the challenges that specialty organizations face as they try to ensure that their guidelines are trustworthy and useful. We examine the practice guidelines for Major Depressive Disorder recently published by the American Psychiatric Association (APA), identify five sources of potential bias that may affect the guideline development process and offer suggestions based on our review. In the mental health field, corporate sponsorship bias can affect psychiatric taxonomy and clinical Practice Guidelines (CPG). Financial conflicts of interest (FCOI) can occur when there are financial associations between researchers, authors, or panel members developing psychiatric diagnostic and treatment guidelines, and the pharmaceutical industry, or when randomized clinical trials (RCTs) are industry funded. OBJECTIVE: To validate the effect of gender on age at onset of schizophrenia in a developing country

METHODS: Medical records of 252 patients diagnosed with schizophrenia according to DSM-IV criteria at a tertiary care hospital in Karachi, Pakistan between 2002 and 2008 were reviewed using a structured questionnaire. Age at onset was defined as age at onset of psychotic symptoms, age at first contact with a health care provider and age at first hospitalization. Socio-demographic variables were reported using descriptive statistics and all measures of age at onset were compared across gender using t-test.

DBT has also been adapted for inpatient settings and demonstrates effectiveness with this population. To date no published literature examines the effectiveness of the standard outpatient model implemented in an inpatient setting. Self-injury behavior (SIB) includes, among other behaviors, "intentional carving or cutting of the skin and subdermal tissue, scratching, burning, ripping or pulling skin or hair, swallowing toxic substances, bruising, and breaking bones" (Cornell Research Program, 2006). This study investigated the efficacy of current medication treatment procedures for chronic schizophrenics on neuropsychological measures. A total of twenty participants from the Coles County Mental Health Center, who met DSM-IV criteria for chronic schizophrenia, volunteered to be administered the Wisconsin Card Sorting Test (WCST)-64 card version as a means of assessing neuropsychological executive functioning. A total of twenty participants free of any diagnosable psychological disorder also volunteered to be administered the WSCT-64, which served as a control group. This paper is an attempt to raise awareness of the presence of self-injury in Western societies. Causes, symptoms, related diagnoses, and treatment of self-injurious behaviors will be discussed. Most importantly, however, this paper is meant to challenge and, ultimately, decrease the stigma surrounding SD and increase empathy and compassion for the self-injuring person.

Due to constraints of this research paper, the author focuses on female self-injurers and excludes information on self-injury in mentally retarded individuals and prisoners.

An Exploration Of The Relationship Between Loneliness And Differential Diagnosis In A Rural Mental Health Center, Kathryn Dunscomb

The review of the literature discussed loneliness in general and specified three seperate forms of loneliness; social, psychic, and existential. The relationships between loneliness, social interaction, and pathology were discussed. 139 clients were given the Belcher Extended Loneliness Scale - Revised prior to therapy. They were assigned one of six diagnostic categories to determine loneliness score (LS), diagnostic, and sex interactions. An analysis of variance found a significant difference (p <.0001) between the diagnostic categories’ LS. The schizophrenic’s and the neurotic’s LS s were significantly higher (<.05) than the remaining categories. The males had significantly (p <.036) higher LS than the females. It was also hypothesized that the degree of loneliness would increase the lower the diagnostic developmental level. The hypothesized order was not consistent with the results. Further research was recommended.

The purpose of this study was to evaluate a token economy program at Oregon State Hospital in terms of patients’ behavioral improvement during the first seventy days of the program. As the study progressed, it became apparent that a study of the problems involved in initiating this program would be significant. Thus an attempt has been made to evaluate both the patients’ behavioral improvement and the problems involved in setting up an effective token economy program.
