The opioid crisis remains a critical public health concern, with rising rates of overdose and opioid use disorder. While opioids are effective for managing pain, both chronic use and withdrawal are associated with increased pain sensitivity. Although most prior research has focused on adults, opioid use during adolescence is common and linked to a heightened risk for developing alcohol use disorder and other forms of substance misuse. Like opioids, acute alcohol use can be analgesic, but chronic alcohol exposure and withdrawal lead to increased pain sensitivity. Alcohol use disorder (AUD) reigns as one of the most pressing issues in the United States with high rates of complications, morbidity, and mortality. Recent studies have exhibited the potential for Glucagon-Like Peptide-1 (GLP-1) agonists to decrease alcohol cravings or alcohol-related adverse events through human and animal studies via reward and satiety signaling pathways. To investigate the ability of GLP-1 agonists to decrease alcohol use, alcohol cravings, and alcohol-related adverse events, this study conducted a scoping review of the current literature to uncover the potential benefit of GLP-1 agonists in patients with AUD. Alcohol withdrawal is characterized by various symptoms that include pain and negative affect in the absence of the drug. The neural underpinnings of these behaviors are not entirely understood, but orexin has emerged as a candidate target for the treatment of substance use disorders. Here, we explored changes in orexin system-related gene expression in brain regions important for mediating reward and stress, including the ventral tegmental area (VTA) and extended amygdala (including the central amygdala, nucleus accumbens shell, and bed nucleus of the stria terminalis), in adolescent and adult female Wistar rats following chronic alcohol exposure. Chronic pain affects over 20% of the global population and contributes to the vast burden of psychiatric illness. While effective treatments for chronic pain remain limited, both alcohol and cannabis have been used for centuries to manage pain and closely associated negative affective symptoms. However, persistent misuse of alcohol and/or cannabis in such a negative reinforcement fashion is hypothesized to increase the risk of severity of substance use disorders (SUDs). The current review describes neurobiological evidence for the analgesic efficacy of alcohol and primary cannabis constituents and how use or co-use of these substances may influence SUD risk.

Chronic alcohol exposure leads to a neuroinflammatory response involving activation of the nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3) inflammasome and pro-inflammatory cytokine production. Acute ethanol (EtOH) exposure activates GABAergic synapses in the central and basolateral amygdala (BLA) ex vivo, but whether this rapid modulation of synaptic inhibition is due to an acute inflammatory response and alters anxiety-like behavior in male and female animals is not known. Survivors of unwanted sexual contact have an increased likelihood of using substances in sexual situations, which puts them at heightened risk for intoxication-related harms. Separately, research has indicated that women may intentionally use substances in sexual situations to either enhance pleasure (i.e., increase sexual excitation) and/or reduce sexual anxiety or shame (i.e., reduce sexual inhibition), a phenomenon termed sex-linked substance use (SLSU). Individuals often engage in the comorbid use of alcohol (ethanol) and nicotine, leading to further-escalated health consequences than the use of either drug in isolation. Given the poor health outcomes resulting from the combined administration of ethanol and nicotine, some of which may be due to increased risky choice, more basic research is needed to better characterize drug/dose interactions on risky choice. Probability-discounting procedures involve a series of discrete choices between a smaller, certain reinforcer (i.e., a safer choice) and a larger, increasingly uncertain reinforcer (i.e., a riskier choice). This study’s goal was to test a novel model of obsessive-compulsive thoughts about drinking. Young adults completed tasks measuring potential drinking characteristics. Findings indicated demand was related to higher OCTD, a significant effect of CIC on OCTD via demand, and that the relationship was particularly strong for impulsive individuals.

This project expanded on the Acquired Preparedness Model of Risk (APMR) by examining how Negative Urgency (NU), the tendency to act rashly in negative emotional states, affects emotion-based changes in alcohol cognitions to produce risk for alcohol use. The APMR prioritizes the role of outcome expectancies as the means through which traits such as NU, convey alcohol use risk. However, this model treats these cognitions as static and often fails to assess their valence; further, alcohol-cognitions fluctuate in response to negative emotions and may become more salient during these states. While it is well documented that AUD is heritable, the shift from recreational alcohol use to abuse/dependence is poorly understood. In this dissertation, using postmortem brain tissue from individuals with alcohol dependence (AD), we profiled the genome-wide expression of circular RNA (circRNA), microRNA (miRNA), and messenger RNA (mRNA) to better understand the impact of gene expression on the development of AUD. Alcohol misuse and elevated social anxiety are found to be highly comorbid, and being exposed to social models consuming alcohol can increase one’s consumption. Yet no research has thus far examined whether the internal experience of alcohol consumption (e.g., social anxiety reduction) can also be transmitted via social modeling. This bar-lab study examined the impact of social modeling behavioral cues of social anxiety on emerging adult drinkers. Multisensory environments facilitate behavioral functioning in humans. The redundant signal effect (RSE) refers to the observation that individuals respond more quickly to stimuli when information is presented as multisensory, redundant stimuli rather than as a single stimulus presented to either modality alone. Our studies show that the disinhibiting effects of alcohol are attenuated when stop signals are multisensory versus unisensory. The present study expanded on this research to test the degree to which multisensory stop signals could also attenuate the disinhibiting effects of alcohol in those with attention-deficit hyperactivity disorder (ADHD), a clinical population characterized by poor impulse control. Older adults are at high risk for alcohol and medication interactions (AMI). Pharmacies have the potential to act as ideal locations for AMI education, as pharmacy staff play an important role in the community. This study examined the perspectives of pharmacy staff on AMI prevention programming messaging, potential barriers to and facilitators of older adult participation in such programming, and dissemination methods for AMI prevention information. Flyers, telephone calls, and site visits were used to recruit 31 pharmacy staff members who participated in semistructured interviews. The purpose of the present study was to better understand alcohol use problems by examining the effect of alcohol liking on alcohol attentional bias among non-dependent drinkers. An adapted model of Robinson and Berridge’s (1993) incentive-sensitization theory of addiction was proposed which theorized that manipulation of alcohol liking would produce alcohol attentional bias (assessed via visual probe task) among non-dependent drinkers. To test this adapted model, alcohol liking was manipulated and the effect on alcohol attentional bias was examined. Motives for substance use have garnered considerable attention due to the strong predictive utility of this construct, both in terms of use and problems associated with use. The current study examined the cross-lagged relations between alcohol use and motives, and marijuana use and motives over three yearly assessment periods in a large sample (N = 526, 48% male) of college students. The relations between substance use and motives were assessed at each time point, allowing for the examination of these inter-relations over time. Ethnic identity is an important buffer against drug use among minority youth. However, limited work has examined pathways through which ethnic identity mitigates risk. School-aged youth (N = 34,708; 52 % female) of diverse backgrounds (i.e., African American (n = 5333), Asian (n = 392), Hispanic (n = 662), Multiracial (n = 2129), Native American (n = 474), and White (n = 25718) in grades 4–12 provided data on ethnic identity, drug attitudes, and drug use. Background—Determining the brain-behavior profiles that differentiate heavy drinkers who are and are not alcohol dependent will inform treatment efforts. Working memory is linked to substance use disorders and can serve as a representation of the demand placed on the neurophysiology associated with cognitive control.

Methods—Behavior and brain activity (via fMRI) were recorded during an N-Back working memory task in controls (CTRL), nondependent heavy drinkers (A-ND) and dependent heavy drinkers (A-D). Typical and novel step-wise analyses examined profiles of working memory load and increasing task demand, respectively.

Background—People arrested for driving under the influence of alcohol (DUI) are at high risk to reoffend. One reason for this high rate of recidivism among DUI offenders is that these individuals systematically underestimate the degree to which alcohol impairs their ability to drive. This study compared perceived and objective driving ability following alcohol and performance feedback in drivers with and without a history of DUI.

Separate externalizing and internalizing pathways to problem drinking have been described. However, internalizing and externalizing are substantially correlated, thus, there is good reason to believe that these two forms of dysfunction behaviors do not operate independently.

We tested an integrative developmental model of transactions among internalizing symptomatology, externalizing personality, and psychosocial learning in the prediction of both drinking problems and future internalizing symptoms. Objective: To help clarify the effect of gender on the bidirectional relationship between alcohol use and strenuous physical activity in college students. Participants: Five hundred twenty-four (52% female) college students recruited in August 2008 and 2009 and followed up in April 2009 and April 2011, respectively. Methods: Participants reported their alcohol use and strenuous physical activity on 2 occasions (baseline and follow-up) spaced approximately 1 or 2 years apart. Alcohol use disorders (AUD) exacerbate neurocognitive dysfunction in Human Immunodeficiency Virus (HIV+) patients. We have shown that chronic binge alcohol (CBA) administration (13-14 g EtOH/kg/wk) prior to and during simian immunodeficiency virus (SIV) infection in rhesus macaques unmasks learning deficits in operant learning and memory tasks. The underlying mechanisms of neurocognitive alterations due to alcohol and SIV are not known. This exploratory study examined the CBA-induced differential expression of hippocampal genes in SIV-infected (CBA/SIV+; = 2) macaques in contrast to those of sucrose administered, SIV-infected (SUC/SIV+; = 2) macaques. Homeodynamic space (HDS) shrinks as vulnerability increases with aging and repeated damage to the cells. HDS is lost in alcoholic pancreatitis patients due to overconsumption of alcohol, smoking, and high fat diets. Etiologically relevant animal models for study of chronic pancreatitis (CP) are needed. In order to begin filling this gap a central purpose of this dissertation research was to examine relationships between the alcohol and high fat diet (AHF) and pancreatitis with attention to hypersensitivity and anxiety-like behaviors. The AHF diet induced pancreatitis described here etiologically mimics human risk factors of AHF consumption for advancement to alcoholic CP.

Prenatal alcohol and nicotine exposure have well known physiological effects on the fetus. However, it is the goal of this thesis to inform the reader of of the deleterious effects that these substances can have on cognitive and behavioral development in children. A literature review in relation to this topic was conducted through online databases using key words. Though some of the results were inconsistent, enough evidence exists for women to be educated on the increased risks for cognitive and behavioral deficits in children exposed to alcohol and nicotine in utero.

Alcohol use and abuse among emerging adults is highly correlated with increased risk for sexual victimization. Alcohol myopia theory has been used to explain impairments in Social information processing resulting in decreased attention to environmental Social cues including risk factors for sexual assault as well as facial emotional recognition. Those with deficits in Social information processing may be at particular risk for the misperception of salient risk factors for sexual assault by victims, perpetrators, and bystanders when intoxicated. The Interplay Of Trait Anger, Childhood Physical Abuse, And Alcohol Consumption In Predicting Intimate Partner Aggression, Rosalita C. Maldonado, Laura E. Watkins, David Dilillo

The current study examined three well-established risk factors for intimate partner aggression (IPA) within Finkel and Eckhardt’s I3 model, including two impellance factors—trait anger and childhood physical abuse history—and the disinhibiting factor of alcohol consumption. Participants were 236 male and female college students in a committed heterosexual dating relationship who completed a battery of self-report measures assessing childhood physical abuse, trait anger, alcohol consumption, and IPA perpetration. Results revealed a significant three-way interaction showing that as the disinhibition factor alcohol consumption increased, the interaction of the two impelling factors, trait anger and childhood physical abuse, became increasingly more positive. …

Work Intensity And Substance Use Among Adolescents Employed Part-Time In Entry-Level Jobs, Jessica Samuolis

This study investigated the relationship between number of hours worked, or work intensity, and substance use in a sample of adolescent employees of a supermarket chain. Employees working half-time or more per week (high-intensity hours) were over three times as likely to smoke compared to those working an average of 10 hours or less per week (low-intensity hours). Males working a high intensity number of hours were more than twice as likely to drink compared to males working at low intensity. This paper contrasts dual-process and personality approaches in the prediction of addictive behaviors and related risk behaviors. In dual-process models, behavior is described as the joint outcome of qualitatively different “impulsive” (or associative) and “reflective” processes. There are important individual differences regarding both types of processes, and the relative strength of both in a specific situation is influenced by prior behavior and state variables (e.g., fatigue, alcohol use). From this perspective, a specific behavior (e.g., alcohol misuse) can be predicted by the combined indices of the behavior-related impulsive processes (e.g., associations with alcohol), and reflective processes, including the ability to …
