Seventy percent of persons who suffer from psychiatric illness do not receive treatment. Cost-effective, automated treatment can be delivered through the Internet but can be complicated by the lack of professional supervision. This open study piloted a fully automated, publicly available treatment for jet lag as a means of highlighting some of the issues involved in delivering treatment over the Internet. Twenty study participants rated the severity of their jet lag symptoms and their adherence to a light-exposure schedule calculated to accelerate adaptation to a new time zone. Prediction Of Long-Term Intellectual And Neuropsychological Effects Of Closed Head Injury In Infants And Children, Tamara Sue Brenner

The ability to predict long-term intellectual and neuropsychological outcome (1-7 years post-injury) in children (ages 1 week - 14 years at injury) with a history of closed head injury were investigated. Clinical indicators of injury severity including EEG reading and age at injury as well as ’H-MRS variables of NAA/Ch, Ch/Cr, and lactate presence accurately classified 100% of children as functioning either within or above the average range or below the average range for most outcome measures. Combined clinical and H-MRS variables accounted for approximately 50% of the variance in outcome confirming the validity of their predictive use and also …

One of four children in the United States lives in a family in which the child is exposed to alcoholism. Children of alcoholics (COAs) are two to ten times more likely to develop alcoholism than non-COAs. This paper reviews studies that have attempted to identify risk factors that mediate the increased vulnerability and the protective factors that moderate the risk. Factors discussed include parental antisocial personality disorder, externalizing behavior, internalizing symptoms, differential response to the effects of alcohol, and positive and negative alcohol-related expectancies. This study investigates gender differences in the barriers and incentives that are most influential in the coronary patient's decision to participate in cardiac rehabilitation (CRPs) and suggests strategies to counter these barriers. Patient surveys were administered to consecutive English-speaking attenders and referred nonattenders to a cardiac rehabilitation center at a university healthcare system in Toronto, Canada. A survey questionnaire, constructed from a literature review and advice from key informants, examined potential factors affecting decisions to engage in CRPs. One129 attenders at a CRP and 61 referred nonattenders completed the questionnaire. Physician recommendation was reported to be the most important factor …

The development of emergency department medical interventions and the implementation of fast-transport trauma systems has decreased the rate of death resulting from traumatic brain injury (TBI). Without corresponding methods for long-term treatment and recovery, the prevalence of people disabled by TBI has increased, creating a growing public health problem. Investigations generated by physicians, rehabilitation programs, and social scientists, which attempt to associate standard measures of injury severity with outcome, leave unexplained variance in long-term functional status for persons with TBI.

Examined the relation of neonatal cranial ultrasonography abnormalities to later psychiatric disorder in 564 low birth weight children (aged 5.2–8.7 yrs). Psychiatric diagnoses were assessed by means of the Diagnostic Interview Schedule for Children–Parent version, and detailed information was collected on other predictors and outcomes. Results show that 454 Ss had no cranial abnormalities, 78 had germinal matrix hemorrhage (GMH) and/or intraventricular hemorrhage (IVH), and 32 had a parenchymal lesion (PL) or ventricular enlargement (VE). 22% of the Ss had at least 1 psychiatric disorder, the most common being attention deficit hyperactivity disorder (ADHD). The purpose of this qualitative study is to discover and examine the coping strategies concerning death and dying from the perspectives of hospice patients and hospice volunteers. A review of applicable literature, informal but structured interviews and participant observation are procedures for the study. The focus of the study lies in three areas: what are the internal attributes used to cope?; what are the external support systems used to cope? and, what are the adjustments in personal goals? Provides clinical details in the form of 11 case vignettes from a prospective epidemiological study that found the combination of childhood 'soft' signs and anxious behavior to be a strong risk factor for adolescent emotional disorders. The original study conducted a neuropsychiatric assessment of 180 African-American adolescents (aged 16–18 yrs) who had been followed through childhood. The at-risk Ss exhibited a persistent and specific pattern of both motor abnormalities and anxiety, obsessional compulsive, or depressive symptoms over time. Possible predictors of reported lower cognitive functioning in irradiated children with acute lymphoblastic leukemia (ALL) were investigated. Thirty-four subjects, 5-14 years old, with ALL in continuous complete remission and without evidence of current or past central nervous system disease, were examined 9-110 months after diagnosis, using standard measures of intelligence and academic achievement. Subjects with a history of post-irradiation somnolence syndrome were significantly older at diagnosis than nonsomnolent subjects. Intelligence (IQ) was found to be unrelated to history of somnolence syndrome. IQ and achievement were unrelated to age at irradiation, irradiation-examination interval, and radiation dosages. Fifteen adults from the general population with chronic tension headaches were divided into four groups. Four were assigned to relaxation instructions, biofeedback (EMG and temperature), and home relaxation practice; four were assigned to relaxation instructions and biofeedback; four were assigned to biofeedback alone; and three were assigned to a no-treatment group. Subjects completed a ten-day baseline period followed by a three-week treatment period. Measures were taken of headache frequency, duration, intensity, frontalis EMG, and temperature recordings. Comparisons of pre- and post-treatment data indicated that not one treatment group was significantly different from any other treatment group, including the no-treatment group, …

This chapter reports on finding from a study of ethnic older men, aged 65 an older (Jewish, Irish, and Italian) who were widowed from 2 to 8 years after a long-term study. It focuses on life reorganization after the initial bereavement period. It identifies key issues in the process concerning continuity and change in identity reformulation, changes in health and activity patterns, ethnic identity and lingering attachment to the deceased spouse. Ethnicity as a dynamic life course process, shaped by contextual and historical dimensions, and personal meaning processes are highlighted. Supported by NIH# R01-AG005204

At age 17 two motor signs, mirror movements and dysdiadochokinesis, were found in more than half the subjects known to have had the respective signs at age 7. These rates were significantly higher than rates found within the group of subjects who were sign free at age 7.

Two groups of children were selected from those evaluated and treated over a two-year period. The study attempted to contrast the mental health care needs and the health care delivery system of children from the uppermost classes against those of the lowest classes. Very few diagnostic discriminators were found. Prevalence of intellectual dysfunctions In the lower classes and neuroses in the upper classes did not correlate with behavior pattern of "acting out" or "acting in." This study was concerned with the problem of depression among elderly patients following surgery. The purpose of the study was to find out if older men following transurethral prostatectomy experienced a higher level of depression than normal elderly people in the general population. An attempt was made in the study to suggest factors in the patients' illness or environment which may have influenced the amount of depression they experienced.

To study emotional problems of dysphasia and the resultant effect on rate and extent of language recovery, two subjects (one male and one female) were interviewed. Interviews and data were also obtained from therapists and hospital records. The results showed that emotionality and some personality factors adversely affected language recovery. This would suggest the importance of alleviating stress immediately following the lesion, during the therapy period, and throughout the home situation.

Administering a combination of promethazine and chlorpromazine to patients with a "galloping psychosis" has an antipsychotic and tranquilizing effect which calms them down to a more manageable and less aggressive state. The drugs are chemically similar; promethazine's actions are strongly potentiated in the combination, so large doses must be given under careful supervision. Case histories demonstrate successful short term management of acutely psychotic, aggressive patients who were a danger to themselves and others.
