To build off of our early success, additional systems and tracking capabilities are being explored related to the following:

  • Bi-directional tracking of referrals
  • Care managers to pharmacists
  • Pharmacists to care managers
  • Delivery drivers as community health workers
  • Drug class specific interventions (ex. a focus on opioids)
  • Disease state specific interventions (ex. a focus on asthma or type 2 diabetes)

  • Fluid overload is treated with loop diuretics in patients with heart failure and in patients with acute kidney injury1,2
  • There are sparse data regarding use of very high dose continuous loop diuretic infusions. In some studies, very high doses were only used in outlying patients (maximum160 mg/hour)3, 4
  • The use of very high dose furosemide continuous infusions at Thomas Jefferson University Hospital is a rare practice. These infusions range in dose from 40-240 mg/hour, with no limitation on the duration

Problem Statement

According to the institute of medicine’s preventing medication errors report, the average hospitalized patient is subject to at least one medication error per day. Errors have been known to occur during admission, transfer and discharge of patients. An accurate medication history on admission is crucial and can go a long way in preventing medication reconciliation errors. Of note, more than 40% of medication errors occur from inadequate reconciliation, during admission, transfer and discharge of patients. Of these, 20% result in harm.

Project HOME is a non-profit organization in Philadelphia that provides housing, employment opportunities, medical care, and education for chronically homeless individuals with serious mental health conditions.

One nurse oversees the medication use process at three of Project HOME’s residences; however, non-clinical staff are responsible for carrying out the medication use process.

Neonatal abstinence syndrome (NAS) is a condition affecting newborns exposed to an opioid in utero. Symptoms of NAS include excessive crying, poor feeding, and disordered autonomic control. Up to 2/3 of infants will require pharmacologic therapies to reach symptom control. Opioids including morphine and methadone are the current first-line treatments. Buprenorphine is being investigated as a treatment of NAS. The purpose of this analysis was to evaluate the pharmacokinetics (PK) and pharmacodynamics (PD) of BUP in infants with NAS.

  • Methemoglobin (MetHb) is formed when the iron moiety of hemoglobin (Hgb) is oxidized from ferrous (Fe2+) to ferric (Fe3+) state à impaired O2 delivery to tissues
  • Acetaminophen Metabolism:

~90% is metabolized in liver via sulfation and glucuronidation

remainder is metabolized via CYP450 to a toxic oxidizing agent, N-acetyl-p-benzoquinone imine (NAPQI), which is detoxified via glutathione1

  • Acute Acetaminophen Overdose:

saturates sulfation and glucuronidation pathways

depletes glutathione stores

leads to excess NAPQI à oxidative stress à methemoglobinemia1

  • MetHb Reduction Pathway:

cytochrome b5 reductase removes 95% to 99% of endogenous MetHb
