Faith Community Nurses have common beliefs, principles, ethics, and a level of trust with their population making them well-positioned to positively impact parishioners. FCNs provide support for people’s spiritual needs and nursing care. This makes FCNs a vital and approachable resource for parishioners when seeking advisement and assistance regarding their mental healthcare. In this article, we define red flags as suicidal idealization, mania, and psychosis, which if not promptly assessed and referred for treatment may result in serious harm or death. A significant health burden exists related to type 2 diabetes. Evidence-based solutions through research are needed to improve diabetes outcomes. Faith community diabetes education can be utilized to provide the knowledge needed to manage the disease. The purpose of the current study was to determine if a faith-based community diabetes education program including health coaching can increase self-efficacy in people with type 2 diabetes.

Suggestions and resources for lifestyle modifications and patient education are included. Hospital readmissions affect over 80 percent of all Medicare enrollees. Hospitals have a responsibility to their Medicare patients to keep them safe after discharge in their homes and communities.
