Purpose: When individual patients are transitioned from home or acute care settings to more long-term care environments such as skilled nursing units or assisted living facilities, the residents are at an increased risk of experiencing loneliness, which is linked to depression, anxiety, and increasing cognitive decline, and other detrimental outcomes.

Aims: Combinations of peer-reviewed qualitative and quantitative research studies were analyzed to determine what factors contributed to changes in patient well-being, mental health and overall quality of life during these transitions.

A large portion of healthcare spending is focused on patients with multiple chronic illnesses, and most of these patients are on numerous medications, commonly referred to as polypharmacy or hyper-polypharmacy. Furthermore, hospice-eligible patients have at least one disease process that is considered life-limiting with a life expectancy of less than six months, and often have multiple medical conditions with complicated treatment regimens known as multimorbidity. Background: Evidence indicates that peaceful end-of-life care programs benefit terminally ill patients, with nurses playing a central role in program delivery. However, no structured end-of-life care programs currently exist for patients with head and neck cancer (HNC), who often experience profound physical and psychological distress. A nurse-led Peaceful End-of-Life Care (PEACE) program was therefore developed.
