Abstract

Purpose: When individual patients are transitioned from home or acute care settings to more long-term care environments such as skilled nursing units or assisted living facilities, the residents are at an increased risk of experiencing loneliness, which is linked to depression, anxiety, and increasing cognitive decline, and other detrimental outcomes.

Aims: Combinations of peer-reviewed qualitative and quantitative research studies were analyzed to determine what factors contributed to changes in patient well-being, mental health and overall quality of life during these transitions.

There is an increased prevalence of pressure-related injuries in the hospice setting. This can be due to age, diagnosis, comorbidities, inadequate nutrition or hydration, lack of mobility, or inadequate care due to lack of education. Prevention bundles and policies are at every facility, but are not as effective as needed. This looks at the theoretical framework and ways to prevent pressure injuries as effectively as possible. Pulling information from multiple other research studies to put the best care bundle together to decrease the amount of pressure injuries.

This literature review explores the effectiveness of providing spiritual and religious-based care to patients currently receiving palliative care settings. In the context of this research, palliative care is defined as the six month period before a person’s death. By examining peer-reviewed journal articles from 2020 to 2024, this literature review will incorporate findings from multiple studies (both qualitative and quantitative) in order to effectively compare and contrast spiritual and religious beliefs and practices for patients receiving palliative care. Postoperative delirium (POD) is a serious and multifactorial complication commonly affecting elderly surgical patients. POD is associated with increased morbidity, longer hospital stays, and higher mortality rates. The use of benzodiazepines and opioids during the perioperative period is a significant modifiable risk factor for POD. Despite the potential of Dexmedetomidine (Precedex) to reduce POD incidence due to its sedative, and analgesic properties, it is not widely available in many operating rooms (OR). 2.5 million people develop a hospital-acquired pressure injury each year, and 60,000 of these result in death as an outcome. Nationwide, hospitals have policies in place to prevent HAPIs, but that does not always stop them from occurring. For this project, we looked at one hospitals policy regarding prevention of pressure injuries. In this powerpoint, we outlined the products we found throughout our research to aide in prevention, as well as our recommendations for practice.

Background: Home health is the fastest-growing healthcare setting in the country. Through Home Health Value-Based Purchasing (HHVBP), the Center for Medicare and Medicaid Services (CMS) provides incentives or penalties to HHAs based on outcomes. Hospitalization and emergency department use are weighted heaviest as poor outcomes. HHVBP requires HHAs to report on whether they are engaging in advance care planning (ACP) conversations. For this study, ACP was defined as a conversation held in advance of a medical crisis with a loved one and/or a health care provider about goals; values; preferences for future medical treatments; and choice of a surrogate decision-maker. …
