Chronic ulcers, particularly prevalent in older adults, significantly impact patients' quality of life, leading to prolonged hospital stays and increased healthcare costs. This study explores the efficacy of two common therapies, Negative Pressure Wound Therapy (NPWT) and Maggot Debridement Therapy (MDT), in promoting tissue healing for chronic ulcers in adults aged 60 and older. A methodical search was conducted using databases such as CINAHL, PubMed, EBSCO, and ProQuest, employing specific search strategies and inclusion/exclusion criteria. Background: Heart failure (HF) is a chronic condition with high readmission rates, often due to inadequate patient knowledge about home care and symptom management. Individualized patient education combined with standardized approaches could improve patient outcomes.
Objective: This project’s goal was to determine if using individual patient knowledge to create a personalized education along with standard HF education would improve patient HF knowledge, improve home management and decrease HF readmission rates.

Background: Hypertension affects nearly half of the population in the U.S., with the prevalence disproportionately higher in African Americans (Centers for Disease Control and Prevention [CDC], 2023). Complications of uncontrolled hypertension, such as cardiovascular disorders and stroke, increase with age, prompting the importance of controlling high blood pressure in older adults. African American older adults are at higher risk of developing uncontrolled hypertension and related complications due to the lack of healthcare access, financial constraints leading to the inability to access healthier food choices, exercise capabilities, weight control, adherence to recommended medications, and knowledge deficit.

Background: Emergency Department (ED) overcrowding is a global healthcare crisis documented in the literature for more than a decade, impacting patient outcomes and resource availability. While awaiting delayed transfer to an inpatient room, older adults in the ED remain vulnerable to developing acute delirium. Boarding potentiates the risk for worsening health outcomes, which are further compounded by delirium. Scarcity of resources and time constraints delay delirium detection and increase risk for down-stream hospital-acquired complications, higher cost, and prolonged length of stay. Background: Falls are a significant concern in community living centers for older adult veterans. Promoting physical activity and resident engagement is crucial for reducing fall risk among this population.
Local Problem: A specific microsystem within this setting has a fall rate of 3 falls per month. This discrepancy underscores the need for targeted interventions to effectively address this issue.

การศึกษาความสัมพันธ์เชิงทำนายนี้มีวัตถุประสงค์เพื่อศึกษาอำนาจการทำนายระหว่างโรคร่วม การสนับสนุนทางสังคม ภาวะเปราะบาง ภาวะซึมเศร้า ความสามารถในการดูแลตนเอง และ การรับรู้ภาวะสุขภาพ กับคุณภาพชีวิตด้านสุขภาพของผู้สูงอายุที่ได้รับการเปลี่ยนลิ้นหัวใจเอออร์ติกตีบผ่านหลอดเลือดแดงใหญ่ กลุ่มตัวอย่าง คือ ผู้สูงอายุที่ได้รับการเปลี่ยนลิ้นหัวใจเอออร์ติกตีบผ่านหลอดเลือดแดงใหญ่ ทั้งเพศชายและหญิง อายุ 60 ปีขึ้นไป ที่เข้ารับการผ่าตัดในโรงพยาบาลจุฬาลงกรณ์สภากาชาดไทย, โรงพยาบาลรามาธิบดี และโรงพยาบาลศิริราช จำนวน 108 คน เลือกกลุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ใช้ในงานวิจัย ได้แก่ 1) แบบสอบถามปัจจัยส่วนบุคคล 2) แบบประเมินโรคร่วม 3) แบบประเมินการสนับสนุนทางสังคม 4) แบบประเมินภาวะเปราะบาง 5) แบบประเมินภาวะซึมเศร้า 6) แบบวัดความสามารถในการดูแลตนเอง 7) แบบประเมินการรับรู้ภาวะสุขภาพ และ 8) แบบประเมินคุณภาพชีวิตด้านสุขภาพของผู้สูงอายุที่ได้รับการเปลี่ยนลิ้นหัวใจ เอออร์ติกตีบผ่านหลอดเลือดแดงใหญ่ เครื่องมือมีค่าดัชนีความตรงตามเนื้อหาในชุดที่ 2-8 เท่ากับ 1.00, 0.93, 1.00, 0.93, 0.98, 0.93 และ 0.89 ตามลำดับ และค่าสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ 0.71, 0.85, 0.87, 0.81, 0.97, 0.86 และ 0.94 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติบรรยายและสถิติถดถอยพหุคูณแบบขั้นตอน ผลการศึกษาพบว่า โรคร่วมมีสัมประสิทธิ์การทำนายสูงสุด (β = -.336, p = < .01) รองลงมาคือ ความสามารถในการดูแลตนเอง (β = .324, p < .01) ภาวะซึมเศร้า (β = -.269, p < .01) และภาวะเปราะบาง (β = -.241, p < .01) ตามลำดับ โดยตัวแปรทั้ง 4 ตัวแปร สามารถร่วมทำนายความแปรปรวนของคุณภาพชีวิตด้านสุขภาพของผู้สูงอายุที่ได้รับการเปลี่ยนลิ้นหัวใจเอออร์ติกตีบผ่านหลอดเลือดแดงใหญ่ได้ ร้อยละ 78.90 อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 (Adjusted R2 = .789, p < .01) จากผลการศึกษาชี้ให้เห็นว่า การดูแลสุขภาพของผู้สูงอายุที่ได้รับการเปลี่ยนลิ้นหัวใจเอออร์ติกตีบควรให้ความสำคัญกับการประเมินโรคร่วม การส่งเสริมความสามารถในการดูแลตนเอง การลดภาวะซึมเศร้าและภาวะเปราะบาง เพื่อเพิ่มคุณภาพชีวิตด้านสุขภาพของผู้สูงอายุให้ดีขึ้น

การศึกษาวิจัยครั้งนี้เป็นการวิจัยเชิงทำนาย มีวัตถุประสงค์เพื่อศึกษาภาวะการทำหน้าที่ และปัจจัยที่สามารถทำนายภาวะการทำหน้าที่ในผู้ที่เป็นโรคมะเร็งศีรษะและลำคอที่กำลังรักษาด้วยการฉายรังสี กลุ่มตัวอย่างประกอบด้วยผู้ป่วยโรคมะเร็งศีรษะและลำคอที่มีอายุ 18 ปีขึ้นไป จำนวน 121 ราย ซึ่งเข้ารับการรักษาที่โรงพยาบาลระดับตติยภูมิในภาคตะวันออกเฉียงเหนือ ได้แก่ โรงพยาบาลมะเร็งอุดรธานี โรงพยาบาลศูนย์สุรินทร์ และโรงพยาบาลมะเร็งอุบลราชธานี เครื่องมือที่ใช้ในการวิจัยประกอบด้วย แบบสอบถามข้อมูลส่วนบุคคลและข้อมูลทางคลินิก แบบประเมินการสนับสนุนทางสังคม แบบประเมินประสบการณ์การมีอาการไม่พึงประสงค์ แบบสอบถามวิธีจัดการกับอาการ และแบบประเมินภาวะการทำหน้าที่ของผู้ที่เป็นโรคมะเร็ง ซึ่งแบบสอบถามทั้งหมดผ่านการตรวจสอบความตรงเชิงเนื้อหา และตรวจสอบความเที่ยงได้ค่าสัมประสิทธิ์แอลฟ่าครอนบาค เท่ากับ 0.88 0.96 0.88 0.89 และ 0.94 ตามลำดับ การวิเคราะห์ข้อมูลใช้สถิติเชิงพรรณนา การวิเคราะห์สหสัมพันธ์ของเพียร์สัน และการวิเคราะห์ถดถอยพหุคูณแบบนำตัวแปรทำนายทุกตัวเข้าสมการพร้อมกันในครั้งเดียว (Enter Multiple Regression Analysis) ผลการศึกษาพบว่า ผู้ที่เป็นโรคมะเร็งศีรษะและลำคอที่กำลังได้รับการฉายรังสีมีระดับภาวะการทำหน้าที่อยู่ในระดับต่ำ โดยมีคะแนนเฉลี่ยเท่ากับ 74.62 (S.D. = 17.97) จากการวิเคราะห์ค่าความสัมพันธ์โดยสถิติสัมประสิทธิ์ของเพียร์สัน (Pearson’s correlation) พบว่า ประสบการณ์อาการไม่พึงประสงค์มีความสัมพันธ์ทางลบกับภาวะการทำหน้าที่อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = –.406, p

การศึกษาความสัมพันธ์เชิงทำนายนี้มีวัตถุประสงค์เพื่อศึกษาความสม่ำเสมอในการรับประทานยากดภูมิคุ้มกันและปัจจัยทำนายความสม่ำเสมอในการรับประทานยากดภูมิคุ้มกันในผู้ที่ได้รับการปลูกถ่ายไต กลุ่มตัวอย่าง คือ ผู้ที่ได้รับการปลูกถ่ายไต อายุ 18 ปีขึ้นไป ที่รับบริการ ณ ศูนย์ปลูกถ่ายไต/คลินิก Post-Kidney Transplantation ในโรงพยาบาลพระมงกุฎเกล้า และโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย จำนวน 113 ราย เลือกกลุ่มตัวอย่างแบบหลายขั้นตอนเก็บข้อมูลโดยใช้แบบสอบถาม 8 ชุด เครื่องมือมีค่าดัชนีความตรงของเนื้อหาในชุดที่ 2-8 เท่ากับ 0.86, 0.84, 0.92, 0.80, 1, 1 และ 0.88 ตามลำดับ และค่าความเที่ยงของแบบสอบถามเท่ากับ 0.75, 0.86, 0.95, 0.96, 0.80, 0.86 และ 0.90 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติบรรยายและสถิติถดถอยพหุคูณแบบขั้นตอน ผลการศึกษาพบว่า 1) ผู้ที่ได้รับการปลูกถ่ายไตมีคะแนนเฉลี่ยความสม่ำเสมอในการรับประทานยากดภูมิคุ้มกัน เท่ากับ 23.09 (SD = 0.84) 2) ปัจจัยทำนายความสม่ำเสมอในการรับประทานยากดภูมิคุ้มกันในผู้ที่ได้รับการปลูกถ่ายไตได้สูงสุดอย่างมีนัยสำคัญทางสถิติ คือ ระยะเวลาหลังได้รับการปลูกถ่ายไต (β = -0.313, p < 0.01) รองลงมาคือ การสนับสนุนทางสังคม (β = 0.264, p < 0.01) การรับรู้อุปสรรคของการรับประทานยากดภูมิคุ้มกัน (β = -0.184, p < 0.05) และการรับรู้โอกาสเสี่ยงต่อการเกิดภาวะปฏิเสธไต (β = 0.173, p < 0.05) ตามลำดับ โดยสามารถร่วมทำนายความแปรปรวนของความสม่ำเสมอในการรับประทานยากดภูมิคุ้มกันในผู้ที่ได้รับการปลูกถ่ายไตได้ ร้อยละ 31.40 (Adjusted R2 = 0.314, p < 0.05)

การวิจัยแบบสหสัมพันธ์ ครั้งนี้มีวัตถุประสงค์เพื่อศึกษาความกระตือรือร้นในการดูแลตนเองของผู้ป่วยโรคหลอดเลือดสมองขาดเลือด และศึกษาความสัมพันธ์ระหว่างปัจจัยคัดสรร ได้แก่ อายุ การรับรู้ภาวะสุขภาพ การรับรู้สมรรถนะแห่งตน การสนับสนุนทางสังคม ภาวะโรคร่วม ภาวะซึมเศร้า กับความกระตือรือร้นในการดูแลตนเองของผู้ป่วยโรคหลอดเลือดสมองขาดเลือด กลุ่มตัวอย่างคือผู้ป่วยโรคหลอดเลือดสมองขาดเลือดจำนวน 104 คน ที่ได้รับการรักษาจนพ้นภาวะวิกฤติแล้ว 2 สัปดาห์ อายุระหว่าง 18 - 59 ปี ที่เข้ารับการรักษาในแผนกผู้ป่วยนอก โรงพยาบาลตติยภูมิ 3 แห่ง คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มอย่างง่าย เครื่องมือที่ใช้ใน การวิจัย ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินการรับรู้ภาวะสุขภาพ แบบวัดการรับรู้สมรรถนะแห่งตนในการดูแลตนเอง แบบสอบถามการสนับสนุนทางสังคม แบบประเมินโรคร่วม แบบประเมินภาวะซึมเศร้า และแบบสอบถามคะแนนความกระตือรือร้นในการดูแลตนเอง เครื่องมือวิจัยผ่านการตรวจสอบความตรงตามเนื้อหาโดย ผู้ทรงคุณวุฒิ 5 คน ทดสอบความเที่ยงของเครื่องมือโดยใช้สูตรสัมประสิทธิ์แอลฟาครอนบาค มีค่า ความเที่ยงเท่ากับ .96,.87,.96, .86, .90 และ .93 วิเคราะห์ข้อมูลโดยการหาค่าเฉลี่ยส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์แบบเพียร์สันและสเปียร์แมน ผลการวิจัย 1. กลุ่มตัวอย่างผู้ป่วยโรคหลอดเลือดสมองขาดเลือดวัยผู้ใหญ่ที่ได้รับการรักษาจนพ้นภาวะวิกฤตแล้ว 2 สัปดาห์ มีคะแนนความกระตือรือร้นในการดูแลตนเองเท่ากับ 91.06 (SD = 10.64) ส่วนใหญ่มีคะแนนความกระตือรือร้นอยู่ในระดับ 4 คือ มีการปรับตัวแสดงพฤติกรรมใหม่ และคงไว้แม้อยู่ในสภาวะกดดัน เป็นร้อยละ 96.2 2. การรับรู้ภาวะสุขภาพ และการรับรู้สมรรถนะแห่งตน มีความสัมพันธ์ทางบวกกับความกระตือรือร้นในการดูแลตนเองของผู้ที่เป็นโรคหลอดเลือดสมองขาดเลือดอย่างมีนัยสำคัญทางสถิติที่ระดับ.05 (r =.46 และr =.50) ส่วนภาวะโรคร่วมและภาวะซึมเศร้ามีความสัมพันธ์ทางลบกับความกระตือรือร้นในผู้ที่เป็นโรคหลอดเลือดสมองขาดเลือดอย่างมีนัยสำคัญทางสถิติที่ระดับ.05 ( r = -0.34, r -0.27) 3. อายุ และการสนับสนุนทางสังคมไม่มีความสัมพันธ์กับความกระตือรือร้นในการดูแลตนเองของผู้ที่เป็นโรคหลอดเลือดสมองขาดเลือด

วิจัยนี้เป็นการวิจัยกึ่งทดลองแบบสองกลุ่มวัดผลก่อนและหลังการทดลอง กลุ่มตัวอย่าง คือ ผู้ที่ได้รับการวินิจฉัยว่าเป็นวัณโรคปอดรักษาซ้ำ ทั้งเพศชายและเพศหญิง อายุ 20-60 ปี มารับการรักษาที่คลินิกวัณโรคโรงพยาบาลสรรพสิทธิประสงค์ จังหวัดอุบลราชธานี แบ่งเป็นกลุ่มทดลอง 30 คน และกลุ่มควบคุม 30 คน โดยกลุ่มทดลองได้รับโปรแกรมการจัดการตนเอง ประกอบด้วยการให้ความรู้ การให้การปรึกษา การจดบันทึกการรับประทานยา ร่วมกับการใช้แอปพลิเคชันไลน์ เป็นเวลา 8 สัปดาห์ ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา เปรียบเทียบความแตกต่างด้วยสถิติที และการทดสอบและสถิติไค-สแควร์ การศึกษานี้มีวัตถุประสงค์เพื่อ 1. เปรียบเทียบความร่วมมือในการรับประทานยาในผู้ที่เป็นวัณโรคปอดรักษาซ้ำ ก่อนและหลังได้รับโปรแกรมการจัดการตนเองโดยใช้แอปพลิเคชันไลน์ 2. เปรียบเทียบความร่วมมือในการรับประทานยาในผู้ที่เป็นวัณโรคปอดรักษาซ้ำ หลังได้รับโปรแกรมการจัดการตนเองโดยใช้แอปพลิเคชันไลน์ และกลุ่มที่ได้รับการพยาบาลปกติ 3. ประเมินผลการตรวจเสมหะจากพบเชื้อเป็นไม่พบเชื้อวัณโรคในผู้ที่เป็นวัณโรคปอดรักษาซ้ำ หลังได้รับโปรแกรมการจัดการตนเองโดยใช้แอปพลิเคชันไลน์ ผลการวิจัยสรุปได้ดังนี้ 1. ผู้ที่เป็นวัณโรคปอดรักษาซ้ำหลังได้รับโปรแกรมการจัดการตนเองโดยใช้แอปพลิเคชันไลน์ มีความร่วมมือในการรับประทานยาสูงกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ผู้ที่เป็นวัณโรคปอดรักษาซ้ำที่ได้รับโปรแกรมการจัดการตนเองโดยใช้แอปพลิเคชันไลน์ มีความร่วมมือในการรับประทานยาสูงกว่ากลุ่มได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 3. ผลตรวจเสมหะในผู้ที่เป็นวัณโรคปอดรักษาซ้ำหลังได้รับโปรแกรมการจัดการตนเองโดยใช้แอปพลิเค-ชันไลน์ เปลี่ยนเป็นไม่พบเชื้อมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยกึ่งทดลองแบบวัดผลซ้ำ (วันที่ 5 และสัปดาห์ที่ 6 หลังผ่าตัด) มีวัตถุประสงค์เพื่อประเมินผลของโปรแกรมการออกกำลังกายแบบเพิ่มแรงต้านด้วยยางยืดร่วมกับการให้แรงสนับสนุนทางสังคม ต่อองศาการเคลื่อนไหวข้อเข่าและความแข็งแรงของกล้ามเนื้อต้นขาในผู้สูงอายุหลังผ่าตัดเปลี่ยนข้อเข่าเทียม กลุ่มตัวอย่าง 48 ราย (อายุ 60 ปีขึ้นไป) เข้ารับการผ่าตัดที่โรงพยาบาลตำรวจ ระหว่างเดือนสิงหาคม–ธันวาคม 2567 แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลองเท่ากัน 24 ราย จับคู่ด้วยอายุและลักษณะพื้นฐาน กลุ่มควบคุมได้รับการพยาบาลตามปกติ ส่วนกลุ่มทดลองได้รับโปรแกรมที่พัฒนาขึ้น เครื่องมือทดลอง ได้แก่ โปรแกรมการออกกำลังกายแบบเพิ่มแรงต้านด้วยยางยืดร่วมกับการให้แรงสนับสนุนทางสังคม ผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 ท่าน เครื่องมือเก็บข้อมูล ได้แก่ เครื่องมือวัดองศาการเคลื่อนไหวข้อเข่า และแบบประเมินความแข็งแรงของกล้ามเนื้อต้นขา มีความเที่ยงด้วยวิธีวัดความเท่าเทียมกัน เท่ากับ 1 และ 0.92 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติบรรยาย และการทดสอบที กำหนดนัยสำคัญทางสถิติที่ระดับ .05 ผลการวิจัยสรุปได้ ดังนี้ 1. กลุ่มทดลองมีองศาการเคลื่อนไหวข้อเข่าและความแข็งแรงของกล้ามเนื้อต้นขาเฉลี่ย ในสัปดาห์ที่ 6 สูงกว่า วันที่ 5 อย่างมีนัยสำคัญทางสถิติ (p < .05) 2. กลุ่มทดลองมีองศาการเคลื่อนไหวข้อเข่าและความแข็งแรงของกล้ามเนื้อต้นขาเฉลี่ย สูงกว่ากลุ่มควบคุมในวันที่ 5 และสัปดาห์ที่ 6 อย่างมีนัยสำคัญทางสถิติ (p < .05)
