Congestive heart failure (CHF) is a chronic disease that impairs the quality of life (QoL) of patients and is associated with both high hospital readmission rates and healthcare resource utilization such as emergency services. This study proposed use a student led transitional care program in providing home care to NYHA functional class II and III CHF patients. Although depression in this population is associated with increased rates of re-hospitalization, falls, and suicides, it is frequently under diagnosed and under treated. This Capstone Report examined this problem through three interrelated manuscripts. The first manuscript explored the problem through a review of the literature. Chronic kidney disease (CKD) has become a major issue in our nation. Many of our neighbors, family, and friends have concerns and a call for our attention is necessary. CKD does not have a specific target, but individuals with diseases such as diabetes mellitus, cardiovascular disease, and obesity are all at increased risk. A descriptive study was initiated to investigate the use of peppermint oil to reduce the nausea of the palliative care and hospice patient. Two local palliative care and hospice programs were used as sites for the study. Patients eighteen and older complaining of nausea were invited to participate in the study. The study was conducted for one and a half months. A total of eight patients consented to participate in the study with a mean age of seventy one years. Objective: The objective of this study was to test the feasibility, acceptability, and preliminary clinical outcomes of a method to leverage existing home healthcare telemonitoring technology to deliver depression care management (DCM) to both Spanish- and English-speaking elderly homebound recipients of homecare services.

Materials and Methods: Three stand-alone, nonprofit community homecare agencies located in New York, Vermont, and Miami participated in this study. Evidence-based DCM was adapted to the telemonitor platform by programming questions and educational information on depression symptoms, antidepressant adherence, and side effects. Purpose

To describe and compare patient perceived barriers and motivators and decision-making conflict between two groups of hospitalized patients, those who receive flu vaccines and those who do not.

Data Sources

Data collection during 2003 included extracting data from databases and mailing two surveys to 436 discharged patients. One hundred eight (108) patients participated in the study.

Conclusions

Top barriers included fear of side effects from vaccine (35%) and fear of contracting the flu (30%). Top motivators for obtaining a flu vaccine included previous vaccination (93%) and provider recommendation (62%). A study was conducted on 18 older adult pet owners and non-pet owners residing in a mobile home park to determine self-perception of health status and the level of attachment to pets. Self-reported health status was high and equal for both pet owners and non-pet owners. The level of attachment to the pets was also high. Studies have demonstrated human-animal interactions being beneficial to both individuals and families; but for many residents of mobile home parks and common interest developments, such as condominiums and townhouses, rules prevent companion animal ownership. A study was conducted to identify selected needs of patients receiving nursing care at home through the Loma Linda University Hospital Department of Home Care. The descriptive survey was used. Fifty patient home care records were reviewed.

From the general information secured, it was found that 68 percent of the patients studied were women. One-half of all patients were under sixty years of age. Fifty-four percent lived with a spouse only, while 26 percent lived with their family. There were 20 percent who lived alone and six of these were over the age of sixty. Patients were found in nine …
