Over the past few years, reducing the number of false positive cardiac monitor alarms (FA) in the intensive care unit (ICU) has become an issue of the utmost importance. In our work, we developed a robust methodology that, without the need for additional non-ECG waveforms, suppresses false positive ventricular tachycardia (VT) alarms without resulting in false negative alarms. Our approach is based on features extracted from the ECG signal 20 seconds prior to a triggered alarm. Traditional pulmonary physiology teaches that moving from a supine to a partial Fowler’s position is an effective maneuver to increase functional residual capacity (FRC). This pilot study investigated whether a postural intervention improved FRC and the ratio of closing capacity (CC) to FRC in a sample of overweight and mildly to moderately obese persons. The study also evaluated whether, and to what extent, body mass and body fat distribution influenced these effects.

Aim: This paper reports a study to describe and interpret the meaning of being on long-term mechanical ventilation.

Background: Patients who require mechanical ventilation in a critical care unit for prolonged periods of time are typically sicker than those who are ventilated for shorter periods. Despite advances in treatment modalities for critically ill patients, many still require long-term mechanical ventilation for 7 days or more. Therefore, caring for a long-term ventilated patient is often an everyday occurrence for critical care nurses; however, there is insufficient evidence of the meaning of this experience from a patient perspective.

Critical care nurses are increasingly seeking to base patient care on evidence derived from research studies. The purpose of this study was to explore the meanings former patients attributed to being on long-term mechanical ventilation in a critical care unit (CCU) in Australia. Using Heideggerian phenomenology, unstructured interviews were undertaken with nine participants. Data were analysed thematically using the method developed by van Manen. Thematic analysis revealed four major themes. Lemierre's syndrome (LS) typically occurs in previously healthy young adolescents and young adults who become acutely ill following an attack of pharyngotonsillitis. Also known as post anginal sepsis, those afflicted develop pyrexia, rigours and multiple metastatic abscesses that lead to septic thrombophlebitis of the internal jugular vein. In the pre-antibiotic era this particularly virulent syndrome had a mortality rate in excess of 90%, but since the introduction of antibiotics and the widespread treatment of throat infections, it has became almost unknown.
