Pelvic organ prolapse (POP) is the descent of pelvic organs—such as the bladder, rectum, or uterus—into the vaginal canal, causing symptoms like discomfort, incontinence, and pain. Beyond physical effects, postpartum women may also experience anxiety, decreased self-confidence, and fear of symptom progression. Research by O’Boyle et al. (2002) highlights that POP studies have focused more on treatment than prevention and suggests that while pelvic floor muscle exercises (PFME) are supported as effective preventive strategies, POP remains prevalent in pregnancy. Our change project focuses on preventing pressure injuries in OR patients using an alternating pressure overlay and gel pad system during surgery and recovery. Pressure injuries cost the healthcare system upwards of $26 billion annually, and surgery patients are at high risk due to prolonged immobility, constant pressure on one area, hemodynamic changes, and excessive moisture. Nurses can play a critical role in reducing healthcare costs and improving patient outcomes by utilizing proper training and outcome tracking. Hospital-acquired pressure injuries negatively impact patient outcomes and can increase costs associated with a hospital stay. Studies have shown that visual reminders can improve nursing intervention compliance and correlate with improved patient outcomes.

Hospital-acquired pressure Injuries (HAPIs) are a “never event” and a nursing quality indicator closely monitored by hospitals. Pressure injury prevention strategies have been widely researched and used to reduce HAPI rates; however, despite this research, HAPI rates, especially in the intensive care unit (ICU), remain high. Prevention is a complex, individualized process that requires nursing understanding and application. Wound, Ostomy, and Continence nurses (WOC nurses) are specially trained and certified in wound management and can provide tremendous clinical insight into risk and prevention factors. This integrative review examines whether WOC nurse rounding in the ICU reduces HAPI rates.

Hospital-acquired pressure injuries (HAPIs) result in patient harm, discomfort, and even death, with an estimated 2.5 million HAPIs occurring annually in the United States. These pressure injuries from prolonged pressure on the skin and deeper tissues cause reduced blood flow and the breakdown of skin and tissues, resulting in wounds. Additionally, these injuries contribute to longer hospital stays and increased health care costs. Hospitals have programs aimed at reducing HAPIs as well as ongoing surveillance to identify new trends early on. Our single-center, quality improvement project evaluated the impact of a fall reduction plan while using a Just Culture Algorithm that included weekly fall reviews involving front line staff using a non-punitive structure. The project has shown successful results.

Methods

Prior to starting the program, data at this institution indicated falls were higher than the national fall rate of 3-5 per 1000 patient days. To achieve the goal of reducing the fall rate to below 3.1, an interdisciplinary fall committee was formed, consisting of nurses, nursing leaders, patient care technicians, pharmacists, and physical therapists. Problem: This quality improvement project aims to reduce the incidence of hospital-acquired delirium in an overflow unit, which serves a diverse patient population from medical-surgical and telemetry microsystems. The unit averages 96 delirium episodes annually, costing approximately $3,456,000. The goal is to reduce delirium incidence by 25%, decreasing monthly cases from 8 to 6 by October 31, 2024, and annual cases to 72 by April 2026.

Despite landmark breakthroughs in cancer research, African American adults (AA) bear the highest cancer burden compared to other racial groups in the United States (US). AA adults have twice the likelihood of dying from prostate and uterine cancers compared to White adults, suggesting that there are fundamental issues yet to be addressed when developing and implementing cancer-preventative programs for AA communities.
