Nationwide, approximately 4.1 million Americans suffer from chronic heart failure accounting for estimated direct and indirect costs of over $21 billion spent annually for health care. The incidence of heart failure has doubled each decade since the 1940's and slightly more men were inflicted with the illness. This trend changed three years ago, with a decline in the overall incidence of heart failure and a slight predominance of the illness in women rather than men. The incidence of this condition was reduced in males nearly three times that of females. Women survive longer after diagnosis, however they are hospitalized more, …

The quality of therapeutic alliance between the patient and healthcare provider is a pivotal contributing factor toward optimal healthcare in the current context of rising healthcare costs, shorter hospital stays, and patient's rights. The availability of a well-designed and sound therapeutic alliance instrument can provide understanding and insight into improving the quality of healthcare. The purpose of this study was to develop a reliable and valid therapeutic alliance instrument, to evaluate its psychometric properties, and to explore its usefulness in predicting patient satisfaction. Chemotherapy can be considered both a physiologic and a psychological stressor. Anxiety, nausea, emesis, and immunosuppression are commonly associated with chemotherapy, and may adversely affect the outcome or the process of treatment. Chemotherapy-related anxiety can exacerbate negative side-effects. Some psychological stressors, such as anxiety, can result in down-regulation of certain immune functions. Studies have demonstrated that behavioral interventions, such as specific relaxation strategies, can modify stress-related immune modulation. These behavioral interventions have also been shown to decrease the degree of nausea and vomiting associated with chemotherapy. The purpose of this study was to examine the effect of guilt, degree of spousal support, role conflict, and decreased psychological well-being experienced by educated stay at home mothers with young children. Social role theory is utilized as a basis for the development of a theoretical model. Relevant research literature does not identify a link between guilt, degree of spousal support, role conflict, and decreased psychological well-being in stay at home educated mothers. Crossing Myriad Borders: A Dimensional Analysis Of The Migration And Health Experiences Of Indigenous Oaxacan Women, Sharon Mcguire Phd, Op, Msn, Rn-C

The growing feminization of the migration of ethnic indigenous women, such as Mixtecs, Zapotecs and Triquis from the southern Mexican state of Oaxaca, are altering the dynamics and configuration of traditional Mexican migration, making it especially significant for the health professions. Informed by a postcolonial ecofeminist paradigm, this bi-national grounded theory study explored the migration and health experiences of indigenous Oaxacan immigrant/migrant women. Data collection using participant observation and interviewing occurred in Oaxaca, Mexico, along the US-Mexico border, and the interior of California. Dimensional analysis was used as the method of data analysis to generate theoretical understandings of these phenomena. …

A Report On Nurses' Response To Occupational Injuries And Exposures To Hazards In A Californian Community Hospital, Sandra Kilminster-Binder

This study set out to determine the reasons for underreporting of occupational injuries and exposures in the American health care environment. A cross sectional survey was used to report the responses and opinions of nurses who failed to report all occupational injuries and exposures, and the type of injuries that are least likely to be reported. The participants were a random sample of Registered Nurses employed in a California Health Care Center, who responded to a simple self-reporting questionnaire. The study revealed that the majority of occupational injuries, accidents and exposures by this group in the previous twelve month period …

Gender discrimination related to women's struggles to gain access to career ladders and equal compensation within a male-dominated society has been documented. However, little research has been conducted regarding discrimination among male nurses in a female-dominated work environment. In particular, there is a paucity of empirical data regarding male nurses in advanced clinical practice. As the number of male nurse practitioners increases, there is heightened concern about discrimination. The purpose of this study was to determine whether male nurse practitioners perceived gender discrimination in the workplace. Health promotion is the number one health priority of the nation, yet, health promoting behaviors (HPB) are rarely part of the management plan for persons with anterior knee pain (AKP). In an active duty military population, AKP can interfere with performance on the required, semi-annual physical readiness test (PRT). The purpose of this descriptive, comparative study was to compare the relationships among HPB, AKP and performance on the PRT between two groups of active duty Navy shipboard personnel (those with AKP and those without AKP). Many efforts have been made to maximize the utilization of health cue system. As a result the utilization of health care system by African Americans has increased, improving the health status (Blendon, Aiken, Freeman, & Corey, 1989). While some improvements have been made, recent reports show disparities between African Americans and Caucasians in the utilization of health care systems and in their health status. Barriers have been identified among those in the lower social economic status (SES). These barriers impede health care utilization and negatively affect health status. Dramatically absent from nursing's historical knowledge and professional recognition are the lives, roles, contributions, and legacies of Christian faith-based nurses. The purpose of this study was to investigate the ministry and service of Sister Tutor Dorothy Davis Cook, Church of the Nazarene missionary nurse in the African country of Swaziland 1940 to 1972. The multi-dimensional, multi-task expanded roles manifested in her integration of Christian missionary and nurse were explored and her legacy identified. The most significant primary source for this study was Mrs. Cook herself. She was interviewed on three occasions and has provided personal documents, journals, and photographs. Data …

Personal values are increasingly recognized as important factors in work environments, including health care systems (Cameron & Whetten, 1995; Covey, Merrill, & Merrill, 1995; Fagermoen, 1997). The personal values of nursing students have been explored (Eddy, Elfrink, Weis, & Schank, 1994; Sivberg & Petersson, 1997) as have those of nurses, in the context of work design (Koerner, 1993 ). This grounded theory study explored the experiences of formerly homeless individuals and families who have moved from homelessness into stable housing. This was an ethnographic study and involved the researcher staying in an east coast shelter where some of the formerly homeless participants had become staff members and reside. Data analysis was informed by dimensional analysis. Moving out of homelessness was the studies perspective. Findings of the study revealed the following dimensions that related the formerly homeless's experiences: (a)reacting to circumstances, or how they became homeless; (b)surviving as homeless, or how they lived while homeless; (c)crossing the fine, or what …

The focus of this descriptive-comparative study was to investigate the relationships between potential access (race, age, socioeconomic status (SES), and the means of discovery of breast cancer), realized access (the length of time between discovery of breast abnormality and diagnosis of breast cancer, and the length of time between diagnosis and initiation of breast cancer treatment), and stage at diagnosis of breast cancer, in African American and Caucasian women, receiving care within an equal economic access health care system. This study examined the relationship between three instruments used in measuring tympanic, axillary, and rectal temperatures in infants less than 1 year of age. Temperatures were measured by Oto-temp Pedi Q tympanic thermometers, Becton Dickinson axillary thermometer, and rectal thermometers. A convience sample of 5 infants less than 90 day and 54 greater than 90 days with fever, as well as 34 infants less than 90 days and 27 infants greater than 90 days without fever were studied. The research focused on the identification of entry-level competencies needed by Bachelor of Science in Nursing (BSN) graduates who will begin employment in acute healthcare agencies in the next 10 years in Tennessee. The purpose of the study was to gain increased awareness of the competencies needed by graduates of BSN programs in Tennessee in order to meet the demands of the acute healthcare agencies, in light of present and anticipated changes in the healthcare delivery system. Acts of resistance can be expressions of creativity, protest or non-cooperation by oppressed groups. Resistance is seen as always present in the face of domination. Acts of resistance can help us to understand how the powerless mediate power relations, and they can actually give hope to the powerless. This study looked at the issues of power and resistance through critical and feminist perspective. A central concept of feminist theory is that women, and thus nurses as a women's profession, are oppressed.
