The perioperative care setting can be a hazardous environment for patients undergoing surgery. Surgical teams caring for patients undergoing complex surgical procedures may create an opportunity for surgical errors impacting patient safety. The purpose of this project is to improve consistent standardized Surgical Safety Checklist (SSC) use and briefings in the orthopedic perioperative care setting in a large Midwestern hospital, thereby supporting a culture of safety through staff engagement and a team-based communication approach. Healthcare in the United States is fragmented and costly. Care coordination positively impacts healthcare cost and quality in healthcare systems. Case managers, social workers, and community health workers create health care teams in a health care home model. A team approach, formed by relationships built from collaboration, communication, and application of Complexity Theory, produces these outcomes. Relational coordination based on shared goals, shared knowledge, and mutual respect creates the framework from which to establish a team. Prognosis for terminally ill patients is intended to serve the purpose of educating patients on the expected clinical outcome of their illness. This information is generally communicated to a patient by their primary physician when options for fufiher treattnent or "cure" are limited. Learning a prognosis may be helpful to some patients, while burdensome to others. A review of the literature reveals many barriers to effective communication of prognosis.
