Background: Central line associated bloodstream infections (CLABSIs) are preventable healthcare associated infections (HAI) in the United States. CLABSIs are associated with increased length of hospital stays, mortality, and financial implications for both patients and hospital systems. Extensive research has shown implementing CLABSI prevention bundles can reduce infections and costs. These bundles are often misused, highlighting the importance of bundle compliance and education.
Local Problem: A medical surgical unit in the upper Midwest continues to have delayed central line removal despite a CLABSI prevention bundle. Efforts to decrease central line days were identified as a priority.

Background: The American Diabetes Association (ADA) recommends screening for prediabetes using a validated assessment tool in asymptomatic adults starting at age 35. An effective process to raise screening numbers to identify those at risk of prediabetes and diabetes is needed to provide quality care.
Methods: Nursing staff were educated on use of the risk assessment tool and principles of motivational interviewing (MI). Utilizing the ADA risk assessment tool identified those at risk. Background: Long emergency department (ED) length of stay (LOS) is associated with increased mortality, delay in care, longer inpatient (IP) stays, readmission risk, poor patient satisfaction, and opportunity for error. Within the ED LOS is a quality indicator measuring the time a provider determines a patient will be admitted to the time the patient leaves the ED for an IP unit, called ED-2. Prolonged ED-2 indicates impaired ED output.

Background: Being ‘with woman’ is a fundamental concept deeply embedded within the midwifery profession and profoundly impacts maternal and infant wellbeing and outcomes. The clinical experiences of midwifery students shape their learning outcomes, future practice, and career aspirations. The World Health Organization (WHO) called for the expansion of all nursing roles, including advanced practice nurses (APNs), nurse practitioners (NPs) and clinical nurse specialists (CNSs). การตัดสินใจเชิงจริยธรรมเป็นหนึ่งในความท้าทายสำคัญของพยาบาลวิชาชีพ โดยเฉพาะในบริบทของการดูแลผู้ป่วยระยะสุดท้ายที่มีความหลากหลายทางเพศ ซึ่งมักเผชิญอุปสรรคด้านกฎหมาย วัฒนธรรม และการยอมรับทางสังคม บทความนี้มีวัตถุประสงค์เพื่อวิเคราะห์ความซับซ้อนในการตัดสินใจเชิงจริยธรรมของพยาบาลสำหรับผู้ป่วยที่มีความหลากหลายทางเพศ ในระยะสุดท้ายของชีวิต โดยเน้นการประยุกต์ใช้แนวคิดและทฤษฎีจริยธรรม เช่น ทฤษฎีจริยศาสตร์ ทฤษฎีพัฒนาการทางจริยธรรม และทฤษฎีต้นไม้จริยธรรม ผ่านกระบวนการตัดสินใจเชิงจริยธรรม 5 ขั้นตอน ได้แก่ การรวบรวมข้อมูล การระบุประเด็นขัดแย้ง การพิจารณาทางเลือก การตัดสินใจ และการประเมินผล

วัตถุประสงค์: เพื่อพัฒนาและทดสอบความเป็นไปได้ของนวัตกรรมผ้ารัดหน้าท้องเจลสมุนไพรประคบเย็นบรรเทาอาการปวดในผู้ป่วยหลังผ่าตัดเปิดช่องท้อง

รูปแบบการวิจัย: การวิจัยและพัฒนานวัตกรรมต้นแบบ เป็นส่วนหนึ่งของการวิจัยเรื่อง ผลของการใช้ผ้ารัดหน้าท้องเจลสมุนไพรประคบเย็นต่อความปวดของผู้ป่วยหลังผ่าตัดเปิดช่องท้อง

วิธีการดำเนินการวิจัย: กระบวนการพัฒนานวัตกรรมประกอบด้วย 3 ระยะ 1) ศึกษาสภาพปัญหาของผู้ป่วยหลังผ่าตัดร่วมกับการทบทวนวรรณกรรม 2) ออกแบบและผลิตนวัตกรรมภายใต้ทฤษฎีควบคุมประตู และจากการทบทวนวรรณกรรม และ 3) นำไปทดสอบความเป็นไปได้กับผู้ป่วยหลังผ่าตัด นวัตกรรม ผ่านการประเมินความเหมาะสมของนวัตกรรมต้นแบบจากผู้เชี่ยวชาญ 5 คน การทดลองใช้งานโดยคัดเลือกกลุ่มตัวอย่างแบบเจาะจงเป็นผู้ป่วยหลังผ่าตัดเปิดช่องท้อง จำนวน 5 ราย เครื่องมือที่ใช้ในการศึกษาประกอบด้วย นวัตกรรมต้นแบบ แบบประเมินความพึงพอใจในการใช้งานนวัตกรรม และแบบประเมินความปวดวิเคราะห์ข้อมูลด้วย ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน

ผลการศึกษา: พบว่านวัตกรรมผ้ารัดหน้าท้องเจลสมุนไพรประคบเย็น มีคะแนนเฉลี่ยความสะดวกต่อการใช้งานเท่ากับ 4.6 (± 0.54) คะแนนเฉลี่ยการใช้ประโยชน์ได้จริงเท่ากับ 4.80 (±0.44) นวัตกรรมมีคะแนนเฉลี่ยความปลอดภัยต่อการใช้งานเท่ากับ 5.00 (±0.00 ) คะแนนเฉลี่ยความพึงพอใจต่อการใช้นวัตกรรมเท่ากับ 4.80 (±0.44) และเปรียบเทียบคะแนนความปวดก่อนและหลังการใช้นวัตกรรม พบว่า สามารถลดอาการปวดได้ โดยคะแนนเฉลี่ยความปวดก่อนได้รับนวัตกรรมอยู่ที่ 8.2 คะแนน และหลังได้รับนวัตกรรมอยู่ที่ 5 คะแนน ซึ่งมีความเหมาะสมที่จะนำไปศึกษาต่อในกลุ่มตัวอย่างที่มีขนาดใหญ่ในผู้ป่วยหลังผ่าตัดเปิดช่องท้อง จึงมีความเหมาะสมนำไปศึกษาในระยะถัดไปกับจำนวนผู้ป่วยหลังผ่าตัดเปิดช่องท้องที่มีขนาดใหญ่ขึ้น

วัตถุประสงค์: เพื่อพัฒนาและศึกษาผลของรูปแบบการดูแลภาวะข้อเข่าเสื่อมในชุมชน โดยการมีส่วนร่วมของพยาบาลเวชปฏิบัติ นักการแพทย์แผนไทย และหมอนวดแผนไทยชุมชน

รูปแบบการวิจัย: การวิจัยและพัฒนา

วิธีดำเนินการวิจัย: เป็นการวิจัยแบบสำรวจสัดส่วนและความรุนแรงของโรคข้อเข่าเสื่อม กลุ่มตัวอย่างแบ่งเป็น 1) บุคคลที่มีอายุ 50 ปีขึ้นไป จำนวน 136 คน เพื่อศึกษาปัญหาและความต้องการดูแล 2) พยาบาลเวชปฏิบัติ นักการแพทย์แผนไทย และหมอนวดแผนไทยชุมชน รวม 18 คน เพื่อพัฒนารูปแบบการดูแลฯ 3) บุคคลที่มีภาวะข้อเข่าเสื่อม จำนวน 40 คน ถูกเลือกแบบเจาะจง เพื่อใช้ในการดำเนินและประเมินผลรูปแบบการดูแลภาวะข้อเข่าเสื่อมในชุมชน เครื่องมือที่ใช้ 1) แบบประเมินคัดกรองภาวะข้อเข่าเสื่อม 2) แบบประเมินระดับความรุนแรงข้อเข่าเสื่อม 3) แบบประเมินผลรูปแบบการดูแลภาวะข้อเข่าเสื่อมในชุมชน มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.85, 0.87 และ 0.81 ตามลำดับ มีค่าความเที่ยงสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ 0.82, 0.84 และ 0.79 ตามลำดับ วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา และทดสอบค่าเฉลี่ยโดยใช้สถิติทดสอบที

ผลการวิจัย: พฤติกรรมการจัดการตนเองของผู้ที่มีภาวะข้อเข่าเสื่อมดีขึ้น อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ระดับความปวดและระดับความรุนแรงของภาวะข้อเข่าเสื่อมลดลง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05
