Residents of high-deprivation neighborhoods shoulder a greater burden of age-related health conditions, including experiencing worse chronic musculoskeletal pain outcomes, compared to residents of more affluent neighborhoods. Advancing age is a risk factor for chronic health conditions, which epigenetic modifications may explain. Epigenetic age acceleration occurs when an individual’s epigenetic age is older than their chronological age, and that discordance has been identified as a strong predictor of age-related conditions. Nursing programs are challenged with retaining nurse faculty due to many complex factors, one being the problem of suboptimal transition to the role.

Purpose:

The purpose of this project was to assess the impact of an onboarding and orientation program, the Teaching Excellence Program (TEP), on the transition of newly hired nurse faculty.

Methods:

Program assessment data, including quantitative and qualitative measures of 14 participants' needs and experiences, were collected over one academic year.

Results:

Background: Maternal-fetal attachment (MFA) is the bond between a mother and her unborn child during pregnancy. Pregnant women with diabetes are considered high-risk and their numbers are expected to increase as levels of obesity rise and women wait longer to have children. Background: Women are at an increased risk of experiencing domestic violence (DV) from an intimate partner during the antenatal and post-partum period, contributing to poorer health outcomes for mother and baby. The antenatal period presents a critical window of opportunity for screening and intervention. In Australia, screening guidelines vary across state and territory health departments. This historical discussion paper is an overview for nurses working in mental health of medieval and Early Modern texts known as Ars Moriendi literature, which focuses on the art of dying, grief and loss. Primary and secondary historical documents are used to describe Ars Moriendi literature and how historical understandings of death and dying were shaped within a spiritual context. Ars Moriendi concepts are compared with modern Western secularised and medicalised notions to prompt reflection on historical versus modern approaches to dying, grief and loss.

การวิจัยเชิงคุณภาพตามวิธีการวิจัยปรากฏการณ์วิทยาของ Husserl มีวัตถุประสงค์เพื่อบรรยายประสบการณ์การกลับไปโรงเรียนในระหว่างการรักษาด้วยเคมีบำบัดของผู้ป่วยวัยรุ่นโรคมะเร็ง คัดเลือกผู้ให้ข้อมูลหลักแบบเฉพาะเจาะจงตามเกณฑ์ เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์แบบเจาะลึก และวิเคราะห์ข้อมูลตามวิธีของ Colaizzi ผลการศึกษาพบว่า ผู้ให้ข้อมูลหลักมีทั้งสิ้น 9 ราย เป็นเพศชาย 5 ราย เพศหญิง 4 ราย มีอายุระหว่าง 12-16 ปี เป็นผู้ป่วยโรคมะเร็งเม็ดเลือดขาวเฉียบพลันชนิดลิมฟอยด์ 8 ราย และผู้ป่วยโรคมะเร็งต่อมน้ำเหลือง 1 ราย ผู้ให้ข้อมูลหลักได้เล่าถึงประสบการณ์การกลับไปโรงเรียนในระหว่างการรักษาด้วยเคมีบำบัด 5 ประเด็น คือ 1) กลับไปโรงเรียนอีกครั้งเพื่อตนเอง 2) กลับไปโรงเรียนอีกครั้ง แต่ไม่เหมือนเดิม 3) คนรอบข้างช่วยเหลือ 4) กำลังใจทำให้อยากอยู่ต่อ และ 5) ความสุขของการกลับไปโรงเรียนอีกครั้ง ผลการศึกษานี้ ก่อให้เกิดความเข้าใจที่ชัดเจนมากยิ่งขึ้นเกี่ยวกับประสบการณ์การกลับไปโรงเรียนในระหว่างการรักษาด้วยเคมีบำบัดของผู้ป่วยวัยรุ่นโรคมะเร็ง และสามารถนำข้อมูลการวิจัยนี้ไปใช้เป็นข้อมูลพื้นฐานเพื่อการสนับสนุนให้ผู้ป่วยวัยรุ่นโรคมะเร็งที่มีความพร้อม มีความต้องการกลับไปโรงเรียนให้สามารถกลับไปโรงเรียนได้อีกครั้ง รวมทั้งการวิจัยในอนาคตในหัวข้อที่เกี่ยวข้องต่อไป

การวิจัยนี้เป็นการวิจัยเชิงบรรยาย มีวัตถุประสงค์เพื่อศึกษาความเหนื่อยหน่ายของคู่ชีวิตของผู้ป่วยจิตเภท และเพื่อศึกษาความสัมพันธ์ระหว่าง เพศ ระยะเวลาในการดูแลผู้ป่วย สัมพันธภาพระหว่างผู้ป่วยกับคู่ชีวิต ความเครียด การสนับสนุนทางสังคม ภาระการดูแลผู้ป่วย การรับรู้การถูกตีตราของคู่ชีวิต ด้านผู้ป่วยเป็นอาการทางบวก และอาการทางลบ กับความเหนื่อยหน่ายของคู่ชีวิตของผู้ป่วยจิตเภท กลุ่มตัวอย่าง คือ คู่ชีวิตของผู้ป่วยโรคจิตเภทที่เข้ารับการรักษา ในโรงพยาบาลจิตเวชและโรงพยาบาลระดับตติยภูมิที่มีแผนกผู้ป่วยนอกจิตเวช ในเขตกรุงเทพมหานคร โดยวิธีการสุ่มอย่างง่าย จำนวน 180 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคลของคู่ชีวิตของผู้ป่วยจิตเภท แบบประเมินความเหนื่อยหน่ายของคู่ชีวิตของผู้ป่วยจิตเภท แบบวัดการรับรู้ความเครียด แบบสอบถามการสนับสนุนทางสังคม แบบประเมินสัมพันธภาพของคู่ชีวิตกับผู้ป่วยจิตเภท แบบประเมินความรู้สึกเป็นภาระการดูแลผู้ป่วย แบบประเมินการรับรู้ตราบาป แบบประเมินอาการทางบวกผู้ป่วยจิตเภท และแบบประเมินอาการทางลบผู้ป่วยจิตเภท แบบสอบถามทุกชุดได้รับการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ จำนวน 5 คน และคำนวณค่าความเที่ยงสัมประสิทธิ์แอลฟาครอนบาคได้เท่ากับ .85, .94, .92, .75, .94, .98, .89 และ .82 ตามลำดับ วิเคราะห์ข้อมูลโดยการหาค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติทดสอบทีและสถิติค่าสัมประสิทธิ์สหสัมพันธ์เพียร์สัน ผลการวิจัยสรุปได้ว่า 1. ระดับความเหนื่อยหน่ายของคู่ชีวิตผู้ป่วยจิตเภท ด้านอาการอ่อนล้าทางอารมณ์อยู่ในระดับต่ำ (Mean=11.02, SD=10.83) ด้านการมองข้ามความเป็นบุคคลของผู้ป่วยและผู้อื่นอยู่ในระดับปานกลาง (Mean=6.36, SD=8.06) และด้านความไม่สมหวังในผลสำเร็จของผู้ดูแลอยู่ในระดับสูง (Mean=13.45, SD=9.64) 2. สัมพันธภาพผู้ป่วยกับคู่ชีวิต และการสนับสนุนทางสังคมมีความสัมพันธ์ทางลบกับความเหนื่อยหน่ายของคู่ชีวิตผู้ป่วยโรคจิตเภทในระดับต่ำ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r= -0.450) และ -.154 ตามลำดับ) 3. อาการทางลบมีความสัมพันธ์ทางบวกกับความเหนื่อยหน่ายของคู่ชีวิตผู้ป่วยโรคจิตเภท ในระดับต่ำ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r= .305) 4. อาการทางบวกมีความสัมพันธ์ทางบวกกับความเหนื่อยหน่ายของคู่ชีวิตผู้ป่วยโรคจิตเภท ในระดับปานกลาง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r= .509) 5. ภาระการดูแลผู้ป่วยและการรับรู้การรถูกตีตราของคู่ชีวิตมีความสัมพันธ์ทางบวกกับความเหนื่อยหน่ายของคู่ชีวิตผู้ป่วยโรคจิตเภทในระดับสูง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r= .779 และ .711 ตามลำดับ) 6. ความเครียดมีความสัมพันธ์ทางบวกกับความเหนื่อยหน่ายของคู่ชีวิตผู้ป่วยโรคจิตเภทในระดับสูงมาก …

This cross-sectional study aimed to explore the stages of personal recovery and associated predictive factors among 231 community-dwelling Thai aged 18 years and older with schizophrenia. การศึกษาครั้งนี้เป็นการศึกษาบรรยายเชิงเปรียบเทียบเพื่อศึกษาภาพลักษณ์ของผู้ป่วยก่อนและหลังผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ กลุ่มตัวอย่าง คือ ผู้ป่วยที่ได้รับการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ จำนวน 320 คน คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มแบบหลายขั้นตอนจากผู้ป่วยที่เข้ารับบริการ ณ แผนกผู้ป่วยนอก โรงพยาบาลตติยภูมิ เขตพื้นที่กรุงเทพมหานคร จำนวน 3 โรงพยาบาล เก็บรวบรวมข้อมูลโดยใช้แบบสอบถาม 2 ส่วน คือ 1) แบบบันทึกข้อมูลส่วนบุคคล 2) แบบสอบถามภาพลักษณ์ ผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ค่าสัมประสิทธิ์แอลฟาครอนบาค ของแบบสอบถามภาพลักษณ์ได้เท่ากับ 0.88 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา สถิติทดสอบทีและสถิติการวิเคราะห์ความแปรปรวนแบบวัดซ้ำ ผลการวิจัยสรุปได้ดังนี้ 1. ภาพลักษณ์ของผู้ป่วยก่อนการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ จากการศึกษาครั้งนี้พบว่า กลุ่มตัวอย่างมีภาพลักษณ์ก่อนการผ่าตัดทำทางบี่ยงหลอดเลือดหัวใจอยู่ในระดับดีมาก โดยค่าเฉลี่ยคะแนนภาพลักษณ์เท่ากับ 13.10 ± 4.14 คะแนน 2. ภาพลักษณ์ของผู้ป่วยหลังการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ จากการศึกษาครั้งนี้พบว่า กลุ่มตัวอย่างมีภาพลักษณ์หลังการผ่าตัดทำทางบี่ยงหลอดเลือดหัวใจอยู่ในระดับดี โดยค่าเฉลี่ยคะแนนภาพลักษณ์เท่ากับ 19.49 ± 6.61 คะแนน 3. เมื่อเปรียบเทียบภาพลักษณ์ก่อนและหลังผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ พบว่า ภาพลักษณ์ก่อนและหลังผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 (t = -24.16, df = 319, p = 0.00) 4. เมื่อพิจารณา 7 ตัวแปร ได้แก่ ตัวแปรเพศ อายุ อาชีพ โรคร่วม ดัชนีมวลกาย สถานภาพสมรสและระดับการศึกษา พบว่าภาพลักษณ์ก่อนและหลังผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 (F = 544.484, F = 136.410, F = 199.973, F = 183.928, F = 165.042, F = 391.982, F = 361.773 ตามลำดับ)

การวิจัยกึ่งทดลองนี้ ศึกษาผลของโปรแกรมการสอนแนะที่เน้นการออกกำลังกายแบบนิวโรบิกส์ต่อความจำและการทำหน้าที่ด้านการบริหารจัดการของผู้ป่วยโรคหลอดเลือดสมอง กลุ่มตัวอย่าง คือ ผู้ป่วยโรคหลอดเลือดสมองชนิดขาดเลือดระยะหลังเฉียบพลัน ทั้งเพศชายและหญิง อายุ 40 ปี ขึ้นไป ที่เข้ารับการรักษา ณ Siriraj Acute Stroke Unit โรงพยาบาลศิริราช จำนวน 34 คน แบ่งเป็นกลุ่มทดลอง และกลุ่มควบคุม กลุ่มละ 17 คน กลุ่มทดลองได้รับโปรแกรมการสอนแนะที่เน้นการออกกำลังกายแบบนิวโรบิกส์ ที่พัฒนาจากแนวคิดการสอนแนะของ Girvin (1999) ร่วมกับแนวคิดการออกกำลังกายแบบนิวโรบิกส์ของ Katz and Rubin (1999) 3 ครั้งต่อสัปดาห์ เป็นเวลา 4 สัปดาห์ และกลุ่มควบคุมได้รับการพยาบาลตามปกติ ระหว่างการวิจัยมีกลุ่มควบคุม 1 คน ถูกคัดออกจากการศึกษา จึงเหลือกลุ่มตัวอย่างทั้งหมด 33 คน ประเมินความจำ ด้วยแบบประเมินพุทธิปัญญา และประเมินการทำหน้าที่ด้านการบริหารจัดการ ด้วยแบบทดสอบการทำหน้าที่ของสมองกลีบฟรอนทรัล ค่าความเที่ยงของทั้งสองเครื่องมือ จากการหาความสอดคล้องภายใน ได้ค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ .82 เท่ากัน วิเคราะห์ข้อมูลด้วย Descriptive Statistics, Pearson Chi-square, Fisher's Exact test และ t-test ผลการวิจัยสรุปได้ ดังนี้ ค่าเฉลี่ยคะแนนความจำและการทำหน้าที่ด้านการบริหารจัดการหลังทดลองของทั้งสองกลุ่มสูงกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และค่าเฉลี่ยคะแนนความจำและการทำหน้าที่ด้านการบริหารจัดการหลังทดลองของกลุ่มที่ได้รับโปรแกรมสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ผลการวิจัยนี้แสดงให้เห็นว่า โปรแกรมการสอนแนะที่เน้นการออกกำลังกายแบบนิวโรบิกส์ สามารถช่วยให้ความจำและการทำหน้าที่ด้านการบริหารจัดการหลังโรคหลอดเลือดสมองมีการฟื้นตัวเพิ่มมากขึ้น พยาบาลและบุคลากรทางสุขภาพสามารถนำไปประยุกต์ใช้เพื่อส่งเสริมการฟื้นตัวด้านความจำและการทำหน้าที่ด้านการบริหารจัดการในผู้ที่เป็นโรคหลอดเลือดสมองได้

การศึกษาการจัดบริการทางการพยาบาลสำหรับผู้ป่วยที่ติดเชื้อโคโรนาไวรัส 2019ที่เข้ารับการรักษาในหอผู้ป่วยเฉพาะกิจ, กัณฐิกา สายปัญญา

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาการจัดบริการทางการพยาบาลสำหรับผู้ป่วยที่ติดเชื้อโคโรนาไวรัส 2019 ที่เข้ารับการรักษาในหอผู้ป่วยเฉพาะกิจ โดยใช้เทคนิคเดลฟาย ในการรวบรวมฉันทามติของกลุ่มผู้เชี่ยวชาญ จำนวน 20 คน เก็บรวบรวมข้อมูล จำนวน 3 รอบ วิเคราะห์ข้อมูลโดยวิเคราะห์เนื้อหา และใช้สถิติหาค่ามัธยฐาน และค่าพิสัยระหว่างควอไทล์ ผลการวิจัยพบว่า ประกอบด้วย 7 ด้าน ดังนี้ 1) ด้านการวางแผนการจัดบริการ หัวหน้าพยาบาลเป็นผู้กำหนดแผนการจัดบริการทางการพยาบาลและถ่ายทอดนโยบายมาสู่พยาบาลควบคุมการติดเชื้อในโรงพยาบาลและผู้จัดการพยาบาล ในการคัดเลือกสถานที่ จัดเตรียมเอกสาร เวชภัณฑ์ และอุปกรณ์ทางการแพทย์ 2) ด้านการจัดการโครงสร้าง หัวหน้าพยาบาลเป็นผู้จัดโครงสร้างขององค์การ โดยแบ่งหน้าที่ความรับผิดชอบ และจัดอัตรากำลังโดยผู้จัดการพยาบาล รวมถึงมีการจัดโครงสร้างของหอผู้ป่วยเฉพาะกิจโดยพยาบาลควบคุมการติดเชื้อในโรงพยาบาล 3) ด้านการบริหารจัดการบุคลากรผู้ปฏิบัติงาน ประกอบด้วย การให้ความรู้และฝึกอบรม การมอบหมายหน้าที่ การปฐมนิเทศ และการจัดสวัสดิการและจูงใจการทำงานให้แก่บุคลากรทางการพยาบาล 4) ด้านการควบคุมกำกับการปฏิบัติงาน ประกอบด้วย การควบคุมกำกับการบริหารจัดการความเสี่ยงโดยหัวหน้าทีมการพยาบาล การควบคุมกำกับและป้องกันการติดเชื้อในการปฏิบัติงานโดยผู้จัดการพยาบาล และการควบคุมกำกับการปฏิบัติงานตามมาตรฐานการพยาบาลโดยหัวหน้าทีมการพยาบาล 5) ด้านการติดต่อสื่อสารและประสานงาน ประกอบด้วย การติดต่อภายใน และภายนอกหอผู้ป่วยเฉพาะกิจ เพื่อให้การพยาบาลผู้ป่วยอย่างต่อเนื่อง 6) ด้านการรายงาน ผู้จัดการพยาบาลเป็นผู้รายงานสถานการณ์ปัญหาและยอดผู้ป่วยในแต่ละวันให้หัวหน้าพยาบาลและผู้ที่มีส่วนเกี่ยวข้องทราบ 7) ด้านงบประมาณ หัวหน้าพยาบาลมีการบริหารจัดการให้มีค่าเสี่ยงภัย และจัดสรรเวชภัณฑ์และอุปกรณ์ทางการแพทย์ให้เพียงพอและพร้อมใช้
