วัตถุประสงค์: เพื่อเปรียบเทียบอาการออทิสติกของเด็กออทิสติกวัยก่อนเรียนก่อนและหลังได้รับการสอนแนะพฤติกรรมการเลี้ยงดูของครอบครัว และเปรียบเทียบอาการออทิสติกของเด็กออทิสติกวัยก่อนเรียนระหว่างกลุ่มที่ได้รับการสอนแนะพฤติกรรมการเลี้ยงดูของครอบครัวกับกลุ่มที่ได้รับการพยาบาลปกติ
รูปแบบการวิจัย: การวิจัยกึ่งทดลอง
วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ เด็กออทิสติกอายุ 3-6 ปี และครอบครัวที่มารับบริการที่แผนกผู้ป่วยใน สถาบันราชานุกูล ซึ่งมีคุณสมบัติตามเกณฑ์ โดยได้รับการจับคู่ (Matched pair) และการสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม จำนวนกลุ่มละ 20 คู่ กลุ่มทดลองได้เข้าร่วมการสอนแนะพฤติกรรมการเลี้ยงดูของครอบครัวต่ออาการออทิสติกของเด็กออทิสติกวัยก่อนเรียนเป็นระยะเวลา 6 สัปดาห์ ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการทดลอง คือ คู่มือการสอนแนะพฤติกรรมการเลี้ยงดูของครอบครัวสำหรับพยาบาล และคู่มือการดูแลเด็กออทิสติกสำหรับครอบครัว เก็บข้อมูลโดยใช้แบบประเมินอาการรุนแรงของโรคออทิสซึม แบบประเมินความสามารถของครอบครัวในการเลี้ยงดูเด็กออทิสติก มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ .87 และ .88 ตามลำดับ และมีค่าความเที่ยงสัมประสิทธิ์แอลฟาครอนบาค เท่ากับ .92 และ .89 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติทดสอบที (t-test)
ผลการวิจัย: 1) อาการออทิสติกของเด็กออทิสติกวัยก่อนเรียนหลังได้รับการสอนแนะพฤติกรรมการเลี้ยงดูของครอบครัวน้อยกว่าก่อนได้รับการสอนแนะอย่างมีนัยสำคัญทางสถิติที่ระดับ .00 และ 2) อาการออทิสติกหลังการทดลองของเด็กออทิสติกวัยก่อนเรียนที่ได้รับการสอนแนะพฤติกรรมการเลี้ยงดูของครอบครัวลดลงมากกว่ากลุ่มที่ได้รับการพยาบาลตามปกติแต่ไม่แตกต่างกัน
สรุป: การสอนแนะพฤติกรรมการเลี้ยงดูของครอบครัวช่วยให้อาการออทิสติกของเด็กออทิสติกวัยก่อนเรียนลดลงได้

Postoperative delirium (POD) is a public health and research priority (American Society of Anesthesiologists, 2019). POD is a risk factor for long-term neurocognitive decline, and the rate of decline is directly proportional to the severity of POD (Vasunilashorn et al., 2018). Baseline cognitive function is a strong, independent predictor for POD (Culley et al., 2017). The International Perioperative Neurotoxicity Working Group recommends baseline cognitive function be assessed for older patients prior to surgery and anesthesia (Berger, et al., 2018). Background
Delirium is more prevalent in older people and estimated to occur in up to 50% of the hospital population. Delirium comprises a spectrum of behaviours, including cognitive and attention deficits, and fluctuating levels of consciousness, often associated with an underlying physiological disturbance. Delirium has been increasingly associated with adverse outcomes. Background
Early childhood caries disproportionately affects vulnerable groups and remains a leading cause of preventable hospital admissions for Western Australian children. The Western Australia State Oral Health Plan seeks to improve child oral health through universal and targeted health promotion initiatives with primary caregivers. Background:
Midwives are well placed to promote vaccination awareness throughout a women’s pregnancy and strengthen childhood vaccination demand following hospital discharge. In Perth, Western Australia, Aboriginal children experience some of the lowest vaccination coverage rates across the nation. To identify factors preventing greater vaccination uptake amongst the target population, a theory-based study was conducted with midwives across two Perth maternity hospitals to explore behavioural attributes, knowledge, attitudes and beliefs surrounding vaccination provision and the vaccines administered to Aboriginal children.

วัตถุประสงค์: เพื่อศึกษาประสิทธิภาพของระบบการให้คะแนนสัญญาณเตือนภาวะวิกฤตของระบบ Search out severity (SOS) และ National Early Warning Score (NEWS) ในการทำนายภาวะใส่ท่อช่วยหายใจหรือการเข้ารับการรักษาในหอผู้ป่วยหนักโดยไม่ได้วางแผนของผู้ป่วยสูงอายุ
แบบแผนงานวิจัย: การศึกษาเชิงวิเคราะห์ไปข้างหน้า
วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยอายุตั้งแต่ 60 ปีขึ้นไป ที่เข้ารับการรักษาในหอผู้ป่วยอายุร กรรม โรงพยาบาลจุฬาลงกรณ์ ตั้งแต่วันที่ 1 พฤศจิกายน พ.ศ. 2562 ถึง วันที่ 31 กรกฎาคม พ.ศ. 2563 การเลือกกลุ่มตัวอย่างแบบเจาะจงตามคุณสมบัติที่กำหนด ได้กลุ่มตัวอย่างจำนวน 85 ราย เก็บข้อมูลโดยใช้แบบประเมินสัญญาณเตือนภาวะวิกฤตตามระบบ NEWS และระบบ SOS นำข้อมูลมาคำนวณหาค่าความไว ความจำเพาะ และความแม่นยำ และทดสอบประสิทธิภาพโดยใช้พื้นที่ใต้กราฟ (Area under the curve: AUC)

รูปแบบการวิจัย: การวิจัยเชิงทำนาย
วิธีการดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยเนื้องอกสมองปฐมภูมิที่มาติดตามการรักษาในโรงพยาบาลรัฐบาลในเขตบริการสาธารณสุข กรุงเทพมหานคร 2 แห่ง จำนวน 143 ราย เครื่องมือวิจัยประกอบด้วย 1) ข้อมูลสภาวะส่วนบุคคล 2) ความทุกข์ทรมานจากอาการ 3) พฤติกรรมการจัดการอาการ และ 4) คุณภาพชีวิตที่เกี่ยวข้องกับสุขภาพ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา วิเคราะห์ความสัมพันธ์และหาปัจจัยทำนายด้วยสถิติไคสแควร์ สถิติสหสัมพันธ์เพียร์สัน และสถิติถดถอยพหุคูณ
ผลการวิจัย: ผู้ป่วยเนื้องอกสมองมีคุณภาพชีวิตโดยรวมอยู่ในระดับดี ความทุกข์ทรมานจากอาการโดยรวมอยู่ในระดับปานกลาง พฤติกรรมการจัดการอาการโดยรวมอยู่ในระดับน้อย และพบว่าปัจจัยที่มีอิทธิพลทำนายคุณภาพชีวิตของผู้ป่วยเนื้องอกสมองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ได้แก่ ความทุกข์ทรมานจากอาการ พฤติกรรมการจัดการอาการ ตำแหน่งเนื้องอกสมองที่ Frontal lobe ตำแหน่งเนื้องอกสมองที่ Temporal lobe
สรุป: ผลการวิจัยทำให้ได้ข้อมูลที่มีประโยชน์ สามารถนำไปเป็นข้อมูลพื้นฐานสำหรับวางแผนส่งเสริมพฤติกรรมการจัดการอาการของผู้ป่วยโรคเนื้องอกสมอง เพื่อบรรเทาความทุกข์ทรมาน และส่งเสริมคุณภาพชีวิตได้ต่อไป

รูปแบบการวิจัย: การวิจัยกึ่งทดลอง
วิธีดำเนินการวิจัย: การศึกษานี้ใช้แนวคิดการมีส่วนร่วมของครอบครัวของ Schepp และ Pai ร่วมกับกรอบแนวคิดทฤษฎีการกระตุ้นประสาทรับความรู้สึกในผู้ป่วยไม่รู้สึกตัวของ Sosnowski และ Ustik กลุ่มตัวอย่าง คือ ผู้บาดเจ็บที่ศีรษะรุนแรง ที่โรงพยาบาลแห่งหนึ่งในภาคตะวันออก จำนวน 26 ราย โปรแกรมประกอบด้วย 1) การให้ข้อมูลแก่ญาติ และ 2) การฝึกทักษะและการส่งเสริมให้ญาติมีส่วนร่วมในการกระตุ้น เป็นเวลา 14 วัน มีเครื่องมือ ดังนี้ 1) แบบบันทึกข้อมูลส่วนบุคคล 2) แบบประเมินระดับความรู้สึกตัวของกลาสโกว์ 3) โปรแกรมการกระตุ้นประสาทรับความรู้สึก 4) คู่มือการกระตุ้นประสาทรับความรู้สึกของญาติ 5) แบบประเมินพฤติกรรมการตอบสนองและการรับรู้ แรนโชลอส อะมิกอส และ 6) แบบประเมินการฟื้นสภาพสมองสมาร์ท ผ่านการตรวจสอบความตรงเชิงเนื้อหา มีค่าสัมประสิทธิ์แอลฟาครอนบาคเท่ากับ .80 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา การวิเคราะห์ความแปรปรวนแบบวัดซ้ำ และสถิติวิลคอกซัน

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยจิตเภทเรื้อรังที่รับการรักษาในแผนกผู้ป่วยใน สถาบันจิตเวชศาสตร์สมเด็จเจ้าพระยา ซึ่งมีคุณสมบัติตามเกณฑ์ จำนวน 40 ราย ได้รับการจับคู่ (Matched pair) และสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 ราย กลุ่มทดลองได้รับการดูแลตามโปรแกรมการจัดการอาการทางลบที่ผู้วิจัยสร้างขึ้น ประกอบด้วย 7 กิจกรรม เป็นระยะเวลา 3 สัปดาห์ ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการทดลอง คือ โปรแกรมการจัดการอาการทางลบ เก็บข้อมูลโดยใช้แบบวัดทักษะชีวิต และแบบประเมินอาการทางจิตโรคจิตเภทฉบับภาษาไทย เฉพาะอาการทางลบ ซึ่งมีค่าดัชนีความตรงตามเนื้อหา เท่ากับ .92 และ 1 ตามลำดับ และมีค่าความเที่ยงสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .88 และ .85 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติทดสอบที (t-test)
ผลการวิจัย: 1) คะแนนเฉลี่ยการทำหน้าที่ของผู้ป่วยจิตเภทเรื้อรังหลังได้รับโปรแกรมการจัดการอาการทางลบสูงกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และ 2) คะแนนเฉลี่ยการทำหน้าที่ของผู้ป่วยจิตเภทเรื้อรัง กลุ่มที่ได้รับโปรแกรมการจัดการอาการทางลบสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05
สรุป: การใช้โปรแกรมการจัดการอาการทางลบในการดูแลผู้ป่วยจิตเภทเรื้อรังสามารถทำให้การทำหน้าที่ของผู้ป่วยจิตเภทเรื้อรังดีขึ้นได้ ซึ่งเป็นไปตามมติฐานที่ตั้งไว้

โรคติดเชื้อโคโรนา 2019 (Coronavirus diseases 2019: COVID-19) เป็นโรคอุบัติใหม่ที่กำลังระบาดไปทั่วโลก ผู้ป่วยส่วนใหญ่จะมีอาการไข้ ปวดเมื่อยตามตัว ไอแห้ง การรับกลิ่น รับรส ผิดปกติ ในรายที่มีอาการรุนแรงจะเกิดอาการปอดอักเสบ หายใจลำบาก และหอบเหนื่อย ทำให้เกิดภาวะหายใจล้มเหลวเฉียบพลัน (Acute respiratory distress syndrome: ARDS) การรักษาด้วยการใช้เครื่องช่วยการพยุงการทำงานของปอดและหัวใจ (Extracorporeal membrane oxygenation: ECMO) จะสามารถช่วยรักษาผู้ป่วยได้ เพื่อช่วยพยุงการทำงานของปอดชั่วคราว ระหว่างการรักษาพื้นฟูปอดของผู้ป่วยให้กลับมาเป็นปกติ พยาบาลปริศัลยกรรมต้องมีความรู้ ความเชี่ยวชาญ ความสามารถในการที่จะดูแลผู้ป่วยโรคติดเชื้อโคโรนา 2019 ที่ต้องได้รับการใส่ ECMO ทั้ง 3 ระยะคือ ก่อน ขณะ และหลังผ่าตัด เพื่อให้ผู้ป่วยเกิดความปลอดภัยและไม่เกิดภาวะแทรกซ้อน อีกทั้งยังต้องมีการป้องกันและควบคุมการติดเชื้อ การแพร่กระจายเชื้อโคโรนา 2019 ได้อีกด้วย

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยเด็กวัยก่อนเรียนที่ได้รับการรักษาโดยการใส่สายสวนทางหลอดเลือดดำ ณ หอผู้ป่วยกุมารเวชกรรม โรงพยาบาลศูนย์ราชบุรี จำนวน 60 คน โดยการสุ่มเข้ากลุ่มทดลองหรือกลุ่มควบคุมและจับเข้าคู่กันด้วยการพิจารณาจากอายุและประสบการณ์การใส่สายสวนทางหลอดเลือดดำ ผู้ป่วยเด็กวัยก่อนเรียนในกลุ่มควบคุมได้รับการพยาบาลเพื่อลดความกลัวตามปกติ ขณะที่กลุ่มทดลองได้รับโปรแกรมการพยาบาลเน้นการลดความเจ็บปวด เก็บรวบรวมข้อมูลโดยใช้แบบสังเกตพฤติกรรมความกลัวการใส่สายสวนทางหลอดเลือดดำของผู้ป่วยเด็กวัยก่อนเรียน โดยผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ 5 ท่าน มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ .98 และค่าความเที่ยงของการสังเกตเท่ากับ .94 วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและใช้สถิติทดสอบที (t-test)

Purpose: Encourage prediabetes screening, testing, and referral to a CDC-recognized Diabetes Prevention Program (DPP) as a first-line treatment.
Background: The prevalence of diabetes and prediabetes incidence is highest among Medicare-eligible Washingtonian patients, with 19% and 16%, respectively. DPP results in a 71% reduction of diabetes incidence in 65 years and older. However, only 3% of healthcare sites screen, test, and refer to DPP.

Eating disorders have a high mortality rate, increasing prevalence rate, and often go unrecognized and untreated. Primary care providers are uniquely positioned to intervene early in the development of these disorders yet screening for eating disorders in primary care is limited. Purpose: This program evaluation project aims were to review the current state of simulation experiences in pre-licensure undergraduate nursing programs in Washington State and determine policy recommendations related to the future use of simulation experiences in clinical nursing education. The evaluation compared student outcomes of NCLEX pass rates associated with clinical simulation versus traditional clinical nursing experiences. Programs were evaluated for compliance with INACSL Simulation Standards of Best Practice.

This policy evaluation project evaluated how federal mental health policy changes impact individuals diagnosed with severe mental illness through analyzing how institutionalization and deinstitutionalization policies impact persons with severe mental illness (SMI). In order to determine how federal policy changes impact outcomes for people diagnosed with SMI, psychiatric hospitalization rates per 100,000 from 1850 to 2015 are compared to the life expectancy, incarceration rates per 100,000, and percentage of unhoused population with SMIs in the United States (U.S.) during the same years. Design: A retrospective, observational review was conducted at a small critical access hospital. Descriptive statistics including Mann Whitney and Chi square were performed to evaluate significance.
