Maternal Provision Of Mother's Milk And Birth Trauma In The Neonatal Intensive Care Unit: A Grounded Theory Analysis, Emma West, Samrawit Kelkay

Less than half of very low birth weight (VLBW, < 1500 grams) infants are discharged from the neonatal intensive care unit (NICU) receiving mother’s own milk (MOM), despite its demonstrated importance. Nevertheless, few studies have explored the impact of maternal perceived trauma (MPT) on infant feeding outcomes. The purpose of this study is to describe MPT among mothers of hospitalized VLBW infants and examine infant feeding outcomes with MPT. This analysis is derived from a grounded theory study exploring the maternal provision of milk among a convenience sample of mothers with VLBW infants at a Midwest NICU. This study utilized semi-structured interviews and content analysis to identify common themes (n=30). Data were analyzed using Dedoose, web-based data management and analytical software. A preliminary analysis identified trauma as a prominent theme. Trauma was defined as a mother’s perceived inability to process an experience at the time of the event, a lack of understanding during the time of an event, and the rapid decline of either maternal or infant health. The occurrence of MPT (n=865) was reported most frequently in women described as Black (46%), multipara (57%), married (75%), and ages > 30 (57%). The MPT frequency was greater with cesarean (54%) and hypertensive (71%) births. The MPT frequency with MOM was 63%, but only 23% with exclusive MOM at discharge. This study’s limitations include using a single hospital location and only English-speaking mothers, making the results less generalizable. Additionally, the interviewer’s Latina racial identity may have led to a sense of kinship with some mothers. A better understanding of MPT and those at risk for MPT will allow providers the opportunity to identify interventions to increase MOM feedings at discharge proactively.

Background: Musculoskeletal pain commonly affects the elderly, but the extent of this problem within the Malaysian community remains unknown. This study aimed to investigate the prevalence and impact of musculoskeletal pain among the elderly living in the community.

Methods: Elderly individuals aged 60 years and above with musculoskeletal pain and intact cognition were recruited for this study. Purpose and Rationale: This project is designed to translate the collective knowledge and evidence-based interventions surrounding the reduction of central line-associated bloodstream infections (CLABSIs) and use these evidence-based practices to create an extracorporeal membrane oxygenation (ECMO) cannula site bundle. The desired outcome of creating an ECMO cannula-site bundle is increased cannula site integrity, decreased frequency of dressing changes, and minimized risk of cannula-site infections.

The American Diabetes Association (ADA) supports routine screening for depression as part of standard diabetes management. The PHQ2 and PHQ9 questionnaires are good diagnostic screening tools used for major depressive disorders in Type 2 diabetes mellitus (DM2). Incivility is a well-known issue in nursing, negatively impacting every aspect of the profession. In response to high incivility levels within a baccalaureate nursing program in central Texas, an incivility education module was created for use in adult medical-surgical courses. The module incorporated online preparatory learning followed by active classroom learning, roleplay practice, and faculty modeling to teach students to define, identify, and respond to incivility using evidence-based cognitive-behavioral strategies. Workplace violence incidence rates are increasing in the medical field. The staff in the emergency department (ED) is especially prone to these workplace violence events due to patients arriving in such unpredictable states. Alarmingly, most medical staff members report experiencing some type of workplace violence, whether that is verbal, physical, or sexual violence. The former director of the Parkland ED sent out a survey to the ED staff regarding workplace violence.
