The aim of the systematic review was to determine the effectiveness of organizational-level person-centered care for people living with dementia in relation to their quality of life, mood, neuropsychiatric symptoms and function. ALOIS, the Cochrane Dementia and Cognitive Improvement Group Specialised Register databases, were searched up to June 2018 using the terms dementia OR cognitive impairment OR Alzheimer AND non-pharmacological AND personhood OR person-centered care. Reviewed studies included randomized controlled trials (RCTs), cluster-randomized trials (CRTs) and quasi-experimental studies that compared outcomes of person-centered care and usual (non-person-centered) care, for people with a diagnosis of dementia. Background: This study was the first phase of a larger study which explored the past, present and future of nursing in early parenting services in Australia.

Aim: The aim of this paper is to describe the history of nursing within an early parenting service in Western Australia (WA).

Methods: Triangulation of multiple data sources was used to summarise the nursing role over 120 years. The history was discovered through a document analysis of archives, including oral histories, organisational documents, focus groups, nurses’ diaries and interviews with nurses.

Background: General practitioners (GPs) and general practice nurses (GPNs) face increasing demands to provide end-of-life care (EoLC) as the population ages. To enhance primary palliative care (PC), the care they provide needs to be understood to inform best practice models of care.

Objective: To provide a comprehensive description of the self-reported role and performance of GPs and GPNs in (1) specific medical/nursing roles, (2) communication, (3) care co-ordination, (4) access and out-of-hours care, and (5) multidisciplinary care.

การศึกษาครั้งนี้เป็นการวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาการตีความตามแนวคิดของมาร์ติน ไฮเดกเกอร์ มีวัตถุประสงค์เพื่อบรรยายประสบการณ์ของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ ผู้ให้ข้อมูลจำนวน 11 คน โดยคัดเลือกผู้ให้ข้อมูลแบบเฉพาะเจาะจง เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึก การบันทึกเทปและนำข้อมูลมาถอดคำต่อคำ การสังเกต และการจดบันทึกในระหว่างการสัมภาษณ์ ร่วมกับวิเคราะห์เนื้อหาตามแบบวิธีการของแวน มาเนน ผลการศึกษาสามารถแบ่งเป็น 2 ส่วน ได้แก่ การให้ความหมายของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ และประสบการณ์ของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ โดยการให้ความหมายของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ คือ การดูแลเอาใจใส่เพื่อไม่ให้กลับป่วยซ้ำ เกิดจากความรักใคร่ ห่วงใยและผูกพัน ผู้ป่วยคือคนสำคัญ มีคุณค่า และมีความหมายต่อผู้ดูแล พบ 2 ประเด็นย่อย คือ 1. การทำหน้าที่ด้วยความรักความห่วงใย 2. เกิดความผูกพันรักใคร่ในครอบครัว ส่วนประสบการณ์ของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ พบ 3 ประเด็นหลัก และ 6 ประเด็นย่อย ดังนี้ 1.การรับรู้ว่าผู้ป่วยผิดปกติและแสวงหาการรักษา 1.1) การรับรู้ว่าผู้ป่วยมีอาการผิดปกติ 1.2) แสวงหาการรักษาที่เหมาะสมกับผู้ป่วย 2. การทำใจยอมรับสิ่งที่เกิดขึ้น 2.1) หลากหลายความทุกข์ถาโถมเข้ามาในชีวิต 2.2) การทำใจยอมรับเพื่อตนเองและผู้ป่วย 3. การดูแลฟื้นฟู และป้องกันไม่ให้ผู้ป่วยกลับป่วยซ้ำ 3.1) การทำหน้าที่ตอบสนองทางด้านร่างกาย จิตใจ และสังคมของผู้ป่วย 3.2) ให้ผู้ป่วยได้กลับมาประกอบอาชีพ และมีรายได้เป็นของตนเอง ผลการศึกษาครั้งนี้ทำให้เข้าใจประสบการณ์ของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ ตั้งแต่ผู้ป่วยจิตเภทเริ่มมีอาการทางจิต การดูแลฟื้นฟูเพื่อป้องกันการกลับป่วยซ้ำ และผลกระทบโดยตรงที่เกิดขึ้นกับผู้ดูแลหลัก เป็นแนวทางสำหรับพยาบาลและสหวิชาชีพ สามารถนำไปพัฒนาระบบการดูแลทั้งผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำและผู้ดูแลหลักอย่างมีประสิทธิภาพและเหมาะสมเพื่อป้องกันการกลับป่วยซ้ำ

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาองค์ประกอบและตัวแปรที่อธิบายการดูแลแบบประคับประคองสำหรับผู้ป่วยโรคหลอดเลือดสมอง กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในการดูแลผู้ป่วยโรคหลอดเลือดสมองอย่างน้อย 2 ปีขึ้นไป จำนวน 378 คน ได้จากการสุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ใช้ในการเก็บข้อมูลคือแบบสอบถามองค์ประกอบการแบบประคับประคองสำหรับผู้ป่วยโรคหลอดเลือดสมอง ซึ่งผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิและหาความเที่ยงของเครื่องมือ โดยใช้สูตรสัมประสิทธิ์อัลฟาครอนบาค ได้ค่าความเที่ยงเท่ากับ 0.95 วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์ตัวประกอบ สกัดตัวประกอบด้วยวิธีตัวประกอบหลัก (Principle Components Analysis) หมุนแกนแบบออโธโกนอล (Orthogonal Rotation) ด้วยวิธีแวริแมกซ์ (Varimax Method) ผลการวิจัยพบว่า องค์ประกอบการดูแลแบบประคับประคองสำหรับผู้ป่วยโรคหลอดเลือดสมองประกอบด้วย 11 องค์ประกอบ บรรยายด้วย 81 ตัวแปร มีค่าความแปรปรวนรวมคิดเป็นร้อยละ 64.28 ได้แก่ 1) องค์ประกอบด้านการดูแลระยะสุดท้าย ประกอบด้วย 10 ตัวแปร อธิบายความแปรปรวนได้มากที่สุด คิดเป็นร้อยละ 9.58 2) องค์ประกอบด้านการมีส่วนร่วมและกำหนดเป้าหมายของการดูแล ประกอบด้วย 12 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 8.88 3) องค์ประกอบด้านการจัดการกับความปวดและอาการข้อติดแข็ง ประกอบด้วย 11 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 8.55 4) องค์ประกอบด้านการส่งต่อและระบบการดูแลต่อเนื่อง ประกอบด้วย 11 ตัวแปร อธิบายความแปรปรวนได้ ร้อยละ 7.69 5) องค์ประกอบด้านการดูแลจิตใจและอารมณ์ ประกอบด้วย 9 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 7.39 6) องค์ประกอบด้านการดูแลจิตวิญญาณ ประกอบด้วย 7 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 5.74 7) องค์ประกอบด้านการดูแลความผิดปกติด้านร่างกายหลังเกิดโรคหลอดเลือดสมอง ประกอบด้วย 6 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 5.26 8) องค์ประกอบด้านการให้ข้อมูลที่มีประสิทธิผล ประกอบด้วย 5 ตัวแปรอธิบายความแปรปรวนได้ร้อยละ 3.96 9) องค์ประกอบด้านการเอื้ออำนวยความสะดวกในระบบบริการประกอบด้วย 4 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 3.68 10) องค์ประกอบด้านการป้องกันการเกิดแผลกดทับ ประกอบด้วย …

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อ ศึกษาปัจจัยทำนายความตั้งใจในการฉีดวัคซีนป้องกันเชื้อ HPV ในชายรักชายที่มารับบริการสุขภาพทางเพศ จากปัจจัยด้านความรู้เกี่ยวกับเชื้อ HPV ความคาดหวังผลลัพธ์การฉีดวัคซีนป้องกันเชื้อ HPV การควบคุมตนเองในการฉีดวัคซีนป้องกันเชื้อ HPV การรับรู้สถานการณ์การฉีดวัคซีนป้องกันเชื้อ HPV และการรับรู้ความสามารถของตนเองในการฉีดวัคซีนป้องกันเชื้อ HPV กลุ่มตัวอย่าง คือ ชายรักชายที่มารับบริการสุขภาพทางเพศ ในสถานบริการสุขภาพ 3 แห่ง ในกรุงเทพมหานครที่มีบริการสุขภาพทางเพศให้กับชายรักชาย จำนวน 140 คน อายุ 18-40 ปี คัดเลือกกลุ่มตัวอย่างแบบตามสะดวก เครื่องมือที่ใช้ในการรวบรวมข้อมูล คือ แบบวัดความรู้เกี่ยวกับเชื้อ HPV แบบสอบถามความคาดหวังผลลัพธ์การฉีดวัคซีนป้องกันเชื้อ HPV แบบสอบถามการควบคุมตนเองในการฉีดวัคซีนป้องกันเชื้อ HPV แบบสอบถามการรับรู้สถานการณ์ในการฉีดวัคซีนป้องกันเชื้อ HPV แบบสอบถามการรับรู้ความสามารถของตนเองในการฉีดวัคซีนป้องกันเชื้อ HPV และแบบสอบถามความตั้งใจในการฉีดวัคซีนป้องกันเชื้อ HPV ได้รับการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 คน มีค่าดัชนีความตรงเท่ากับ 1.00, .87, 1.00, 1.00, 1.00 และ1.00 ตามลำดับ และมีค่าสัมประสิทธิ์ KR-20 และครอนบาคแอลฟาเท่ากับ .82, .81, .81, .94, .94 และ.90 ตามลำดับ วิเคราะห์ปัจจัยทำนายด้วยสถิติถดถอยพหุคูณแบบขั้นตอน ผลการวิจัยสรุปได้ดังนี้ 1) ชายรักชายที่มารับบริการสุขภาพทางเพศในสถานบริการสุขภาพมีค่าเฉลี่ยคะแนนความตั้งใจในการฉีดวัคซีนป้องกันเชื้อ HPV อยู่ในระดับสูง (mean=11.39, SD=2.39) 2) การรับรู้ความสามารถของตนเองในการฉีดวัคซีนป้องกันเชื้อ HPV (Beta=.419) การรับรู้สถานการณ์การฉีดวัคซีนป้องกันเชื้อ HPV (Beta=.231) และความรู้เกี่ยวกับเชื้อ HPV (Beta=.148) สามารถร่วมกันทำนายความตั้งใจในการฉีดวัคซีนป้องกันเชื้อ HPV ของชายรักชายที่มารับบริการสุขภาพทางเพศได้ ร้อยละ 40.1 อย่างมีนัยสำคัญทางสถิติที่ระดับ.05 สำหรับปัจจัยความคาดหวังผลลัพธ์การฉีดวัคซีนป้องกันเชื้อ HPV และการควบคุมตนเองในการฉีดวัคซีนป้องกันเชื้อ HPV ไม่สามารถร่วมทำนายความตั้งใจในการฉีดวัคซีนป้องกันเชื้อ HPV ในชายรักชายที่มารับบริการสุขภาพทางเพศได้ (p>.05) ผลการวิจัยในครั้งนี้จะเป็นประโยชน์ในการส่งเสริมความตั้งใจในการฉีดวัคซีนป้องกันการติดเชื้อ HPV ให้กับชายรักชายเพื่อลดอุบัติการณ์ของมะเร็งปากทวารหนักต่อไป

การวิจัยครั้งนี้เป็นการศึกษาความสัมพันธ์เชิงบรรยายเพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับพฤติกรรมแสวงหาบริการสุขภาพของบุคคลที่มีความเสี่ยงโรคมะเร็งลำไส้ใหญ่และทวารหนัก กลุ่มตัวอย่าง คือ บุคคลที่มีความเสี่ยงโรคมะเร็งลำไส้ใหญ่และทวารหนัก จำนวน 122 คน คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มแบบหลายขั้นตอน จากผู้ที่มารับบริการแผนกผู้ป่วยนอกระบบทางเดินอาหาร โรงพยาบาลราชวิถี โรงพยาบาลตำรวจและโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย เก็บข้อมูลโดยใช้แบบสอบถาม 6 ส่วน คือ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามการรับรู้เกี่ยวกับการเจ็บป่วย 3) แบบสอบถามการสนับสนุนทางสังคม 4) แบบวัดความวิตกกังวล 5) แบบสอบถามความกลัว และ 6) แบบสอบถามพฤติกรรมแสวงหาบริการสุขภาพของบุคคลที่มีความเสี่ยงโรคมะเร็งลำไส้ใหญ่และทวารหนัก ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ค่าความเที่ยงของเครื่องมือเท่ากับ .86, .89, .86, .87 และ .86 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้วิธีการทดสอบค่าสัมประสิทธิ์สหสัมพันธ์เพียร์สัน ผลการวิจัยสรุปได้ ดังนี้ 1. บุคคลที่มีความเสี่ยงโรคมะเร็งลำไส้ใหญ่และทวารหนักมีคะแนนเฉลี่ยพฤติกรรมแสวงหาบริการสุขภาพและการสนับสนุนทางสังคมอยู่ในระดับสูง (Mean = 31.31, SD = 4.93; Mean = 72.63, SD = 7.83 ตามลำดับ) ส่วนการรับรู้เกี่ยวกับการเจ็บป่วยและความกลัวมีคะแนนเฉลี่ยอยู่ในระดับปานกลาง (Mean = 74.66, SD = 17.03; Mean = 26.81, SD = 5.11 ตามลำดับ) และความวิตกกังวลมีคะแนนเฉลี่ยอยู่ในระดับต่ำ (Mean = 31.94, SD = 6.80) 2. การรับรู้เกี่ยวกับการเจ็บป่วยและการสนับสนุนทางสังคมมีความสัมพันธ์ทางบวกกับพฤติกรรมแสวงหาบริการสุขภาพของบุคคลที่มีความเสี่ยงโรคมะเร็งลำไส้ใหญ่และทวารหนักอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = .471, .327 ตามลำดับ) และความวิตกกังวลมีความสัมพันธ์ทางลบกับพฤติกรรมแสวงหาบริการสุขภาพของบุคคลที่มีความเสี่ยงโรคมะเร็งลำไส้ใหญ่และทวารหนักอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = -.367)

การวิจัยกึ่งทดลองนี้เป็นการศึกษาแบบสองกลุ่มวัดผลสองครั้งก่อนและหลังการทดลอง มีวัตถุประสงค์ คือ 1) เพื่อเปรียบเทียบอาการทางบวกของผู้ป่วยจิตเภท ระหว่างก่อนและหลังได้รับการจัดการความเครียดแบบกลุ่ม และ 2) เพื่อเปรียบเทียบอาการทางบวกของผู้ป่วยจิตเภทระหว่างกลุ่มที่ได้รับการจัดการความเครียดแบบกลุ่มกับกลุ่มที่ได้รับการพยาบาลปกติ กลุ่มตัวอย่าง คือ ผู้ป่วยจิตเภทที่มารับบริการที่แผนกผู้ป่วยนอก โรงพยาบาลพระศรีมหาโพธิ์ และมีคุณสมบัติตามเกณฑ์ จำนวน 24 คน ซึ่งได้รับการจับคู่เข้ากลุ่มทดลองและกลุ่มควบคุม จำนวนกลุ่มละ 12 คน กลุ่มทดลองได้รับการจัดการความเครียดแบบกลุ่ม ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ 1) การจัดการความเครียดแบบกลุ่มของผู้ป่วยจิตเภท 2) แบบประเมินอาการทางจิตของผู้ป่วยจิตเภท ฉบับภาษาไทย ซึ่งได้ผ่านการทดสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 ท่าน มีค่าความเที่ยงสัมประสิทธิ์อัลฟาครอนบราคเท่ากับ 0.82 วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิตินอนพาราเมตริก ผลการวิจัยสรุปได้ดังนี้ 1) อาการทางบวกของผู้ป่วยจิตเภทหลังได้รับการจัดการความเครียดแบบกลุ่ม ต่ำกว่าก่อนได้รับการจัดการความเครียดแบบกลุ่มอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2) อาการทางบวกของผู้ป่วยจิตเภทกลุ่มที่ได้รับการจัดการความเครียดแบบกลุ่ม ต่ำกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

Hospital readmissions are a major burden for patients with end stage renal disease (ESRD). On average, one in three hospital discharges among patients with ESRD are followed by a readmission within 30 days. Currently, dialysis facilities are held accountable for readmissions via the ESRD Quality Incentive Program standardized readmission ratio (SRR) clinical measure. However, little is known about facility-level factors associated with readmission. Additionally, unlike other standardized measures of quality in the dialysis setting, incident patients within their first 90-days of dialysis are included in the performance calculation. Cardiac surgery patients have some of the highest reported incidence and prevalence of pressure injuries (PI). A growing subset of cardiac surgery include patients with end-stage heart failure who undergo ventricular assist device (VAD) or total artificial heart (TAH) surgery. The risk of PI and their natural history of development in this population are unknown and the specific risk factors for PI development remain unexplored.

The nursing culture of an inpatient pediatric unit was resistant to activating pediatric rapid response team (PRRT) alerts despite guidelines for activation.
