PURPOSE: To evaluate the adherence to the CMS sepsis recommendations and sepsis bundle used by the study health system before and after the implementation of Best Practice Alerts (BPAs) and assessing the effect of these alerts on patient outcomes.

METHODS: The study was a single-center, process evaluation through a retrospective chart review within a southwest healthcare system. The sample consisted of 73 patients for the pre-implementation period (May 1, 2016-September 7, 2016) and 75 patients for the post-implementation period (September 8, 2016-April 30, 2017).

Background/Introduction: Post-operative hospitalization for the Fontan procedure tends to have an extended hospital length of stay (LOS). From 2013-2014, the average LOS at Children’s Mercy for the Fontan procedure is 14 days, whereas the U.S. national reported average is 11 days. Post-operative management of this patient population is often caregiver dependent. It has been theorized that developing a more standardized post-operative management regimen tailored specifically toward the unique physiology of Fontan patients may be able to improve outcomes and decrease LOS.

SUMMARY

The most common indications for starting tandem procedures were ASFA Category III indications, followed by un-categorized indications.

Similar to other reports in literature, hypocalcemia and hypotension were the most common complications. Transfusion reactions were rare, despite the use of large volumes of plasma.

Tandem procedures in critically ill pediatric patients require close monitoring and excellent communication between staff members.

Introduction: The majority of laboratory testing errors occur in the pre-analytical stage, including specimen collection. Children provide special challenges including small blood volumes, small veins, and an inability to cooperate. We describe collaboration between laboratorians and nursing staff to improve specimen collection and the reliability of test results.

Methods: We identified three areas for improvement: 1) Volume of blood for culture, 2) Urine collection, and 3) Appropriate blood collection through a line. Base line data was established. Guidelines for specimen collection were updated. Nurse members of the Laboratory Utilization Committee developed educational tools.

ทารกแรกเกิดน้ำหนักน้อยเสี่ยงต่อการเกิดปัญหาพฤติกรรมการตอบสนอง เนื่องจากระบบประสาทที่ยังพัฒนาไม่สมบูรณ์ และการขาดการกระตุ้นการสัมผัสจากมารดาที่ต่อเนื่อง การวิจัยนี้เป็นวิจัยกึ่งทดลองแบบศึกษาสองกลุ่มที่ไม่เท่าเทียมกันวัดแบบอนุกรมเวลาเพื่อเปรียบเทียบพฤติกรรมการตอบสนองของทารกแรกเกิดน้ำหนักน้อยในมารดาครรภ์แรก ระหว่างกลุ่มทดลองที่ได้รับโปรแกรมการเสริมพลังอำนาจมารดาร่วมกับการสัมผัสแบบอ่อนโยนและกลุ่มควบคุมที่ได้รับการพยาบาลแบบปกติคัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงคือ ทารกแรกเกิดน้ำหนักน้อยที่เกิดจากมารดาครรภ์แรกที่ได้รับการรักษาในแผนกทารกแรกเกิดมีปัญหา โรงพยาบาลตำรวจ จำนวน 44 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 22 คน เครื่องมือที่ใช้ในการวิจัย คือ 1) โปรแกรมการเสริมพลังอำนาจมารดาร่วมกับการสัมผัสแบบอ่อนโยน 2) แบบประเมินพลังอำนาจของมารดา มีค่าความเที่ยงเท่ากับ .89 3) แบบประเมินพฤติกรรมการตอบสนองทารก มีค่าความเที่ยงเท่ากับ .98 ผ่านการประเมินโดยผู้ทรงคุณวุฒิจำนวน 7 ท่าน วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา และสถิติแปรปรวนสองทางแบบวัดซ้ำ (Two-Way Repeated Measure ANOVA ) และเมื่อพิจารณาวิเคราะห์พฤติกรรมการตอบสนองเป็นรายด้าน คือ การหลับตื่น และการเคลื่อนไหว พบว่า 1. พฤติกรรมการหลับตื่นของทารกแรกเกิดน้ำหนักน้อยกลุ่มทดลอง และทารกแรกเกิดน้ำหนักน้อยกลุ่มควบคุม มีความแตกต่างกันในพฤติกรรมกลุ่ม Quiet sleep, Drowsy, Awake alert, Active awake และ Crying และไม่พบความแตกต่างในพฤติกรรมกลุ่ม Active sleep อย่างมีนัยสำคัญทางสถิติที่ .05 2. พฤติกรรมการเคลื่อนไหวร่างกายของทารกแรกเกิดน้ำหนักน้อยกลุ่มทดลอง และทารกแรกเกิดน้ำหนักน้อยกลุ่มควบคุม มีความแตกต่างกันในพฤติกรรมกลุ่ม Behavioral distress cues, No movement และ Smiles และไม่พบความแตกต่างในพฤติกรรมกลุ่ม Motor Activity อย่างมีนัยสำคัญทางสถิติที่.05

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาตัวชี้วัดผลลัพธ์การพยาบาลผู้ป่วยผ่าตัดเปิดทวารเทียม โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือผู้เชี่ยวชาญจำนวน 20 คน ประกอบด้วย พยาบาลเฉพาะทางด้านการดูแลผู้ป่วยแผล ออสโตมี และควบคุมการขับถ่ายไม่ได้ อาจารย์และ/หรือคณะกรรมการจัดอบรมหลักสูตรพยาบาลแผล ออสโตมี และควบคุมการขับถ่ายไม่ได้ พยาบาลผู้ปฏิบัติการพยาบาลขั้นสูง ผู้บริหารทางการพยาบาล และแพทย์ผู้เชี่ยวชาญด้านการดูแลผู้ป่วยผ่าตัดทวารเทียม วิธีดำเนินการวิจัยประกอบด้วย 3 รอบ คือ รอบที่ 1 การสัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับตัวชี้วัดผลลัพธ์การพยาบาลผู้ป่วยผ่าตัดเปิดทวารเทียม รอบที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหา แล้วสร้างเป็นแบบสอบถามให้ผู้เชี่ยวชาญประมาณค่าแนวโน้มความสำคัญของตัวชี้วัดผลลัพธ์การพยาบาลผู้ป่วยผ่าตัดเปิดทวารเทียม และรอบที่ 3 นำข้อมูลที่ได้จากรอบที่ 2 มาคำนวณหาค่า มัธยฐาน ค่าพิสัยระหว่างควอไทล์ จัดทำแบบสอบถามให้ผู้เชี่ยวชาญยืนยันความคิดเห็น เพื่อสรุปเป็นตัวชี้วัดผลลัพธ์การพยาบาลผู้ป่วยผ่าตัดเปิดทวารเทียม ผลการวิจัยพบว่า ตัวชี้วัดผลลัพธ์การพยาบาลผู้ป่วยผ่าตัดเปิดทวารเทียม ประกอบด้วย 4 ด้าน ดังนี้ 1) ด้านความปลอดภัยจากการผ่าตัดเปิดทวารเทียม จำนวน 17 ตัวชี้วัด 2) ด้านการได้รับการตอบสนองทางร่างกาย จิตใจ อารมณ์ และสังคม จำนวน 9 ตัวชี้วัด 3) ด้านความสามารถในการดูแลตนเองของผู้ป่วยจำนวน 4 ตัวชี้วัด 4) ด้านคุณภาพชีวิตของผู้ป่วย จำนวน 13 ตัวชี้วัด

การวิจัยกึ่งทดลองครั้งนี้ มีวัตถุประสงค์เพื่อเปรียบเทียบความรู้สึกตราบาปของผู้ป่วยจิตเภทในชุมชน ก่อนและหลังได้รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตน และเพื่อเปรียบเทียบความรู้สึกตราบาปของผู้ป่วยจิตเภทในชุมชน ระหว่างกลุ่มที่ได้รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนกับกลุ่มที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่างคือ ผู้ป่วยจิตเภทที่มารับบริการแบบผู้ป่วยนอกโรงพยาบาลชุมชน จำนวน 40 คน คัดเลือกเป็น 2 กลุ่ม คือ กลุ่มทดลอง 20 คน และกลุ่มควบคุม 20 คน โดยการจับคู่ด้วยอายุ และระดับคะแนนอาการทางจิตของกลุ่มตัวอย่าง กลุ่มทดลองได้รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนที่ผู้วิจัยพัฒนาขึ้น ส่วนกลุ่มควบคุมได้รับการดูแลตามปกติ เครื่องมือที่ใช้ในการวิจัยคือ โปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตน และแบบวัดความรู้สึกตราบาป ผ่านการตรวจสอบความตรงของเนื้อหาของผู้ทรงคุณวุฒิ จำนวน 5 ท่าน มีค่าความเที่ยงเท่ากับ .87 วิเคราะห์ข้อมูลโดยใช้สถิติทดสอบที ผลการวิจัยสรุปได้ ดังนี้ 1. ความรู้สึกตราบาปของผู้ป่วยจิตเภทในชุมชน ภายหลังได้รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนต่ำกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ความรู้สึกตราบาปของผู้ป่วยจิตเภทในชุมชน ที่ได้รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนต่ำกว่าจากกลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญที่ระดับ .05

การวิจัยเชิงสำรวจครั้งนี้ มีวัตถุประสงค์เพื่อศึกษา 1) ประสบการณ์การมีอาการใน 4 มิติ (มิติการเกิดอาการ มิติความถี่ มิติความรุนแรง และมิติความทุกข์ทรมาน) 2) กลวิธีการจัดการอาการ และ 3) เปรียบเทียบประสบการณ์การมีอาการทั้ง 4 มิติ (ภาระอาการ) จำแนกตามตัวแปร เพศ อายุ โรคร่วม ระยะเวลาในการล้างไตทางช่องท้อง ระดับของฮีโมโกลบิน บียูเอ็น ครีเอตินิน อัลบูมิน แคลเซียม ฟอสเฟต และความพอเพียงของการล้างไต กลุ่มตัวอย่าง คือ ผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการล้างไตทางช่องท้องแบบต่อเนื่องที่มารับบริการ ณ หน่วยบริการล้างไตผ่านทางช่องท้องในระบบหลักประกันสุขภาพแห่งชาติในเขตกรุงเทพมหานคร 2 แห่ง จำนวน 178 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามอาการในผู้ป่วยโรคไตเรื้อรัง และแบบสอบถามกลวิธีการจัดการอาการ ค่าความตรงเชิงเนื้อหาของแบบสอบถามอาการในผู้ป่วยโรคไตเรื้อรังและแบบสอบถามกลวิธีการจัดการอาการ เท่ากับ 0.97 และ 0.94 ค่าสัมประสิทธิ์แอลฟาของครอนบาคของแบบสอบถามอาการในผู้ป่วยโรคไตเรื้อรังทั้ง 4 มิติ เท่ากับ 0.77, 0.83, 0.84, และ 0.86 ตามลำดับ ส่วนของแบบสอบถามกลวิธีการจัดการอาการทั้ง 2 มิติ คือ การเลือกปฏิบัติกลวิธีการจัดการอาการและประสิทธิผลของกลวิธีการจัดการอาการ เท่ากับ 0.73 และ 0.80 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา สถิติทดสอบที สถิติทดสอบแมน-วิทนีย์ และสถิติวิเคราะห์ความแปรปรวนทางเดียว ผลการวิจัย พบว่า 1. อาการที่ผู้ป่วยรายงานมิติการเกิดอาการและมิติความถี่มากที่สุด คือ อาการผิวแห้ง (ร้อยละ 78.1) (mean ± SD = 7.65 ± 2.83) ส่วนอาการที่ผู้ป่วยรายงานมิติความรุนแรงและมิติความทุกข์ทรมานมากที่สุด คือ อาการคัน (mean ± SD = 5.04 ± 2.76, 4.75 …

Purpose: To describe the unmet informational, psychological, emotional, social, practical, and physical needs and preferences for posttreatment survivorship care of individuals living with multiple myeloma to inform the development of relevant, personcentered, survivorship services.

Methods: An exploratory, descriptive study using 2 focus groups with 14 participants, 6 to 49 months postdiagnosis. Results: Thematic analysis revealed 7 key themes: information needs, experience with health-care professionals, coping with side effects, communicating with family and friends, dealing with emotions, support needs, and living with the chronicity of myeloma. Breast cancer is the most common cancer among women in the United States with over 60% of cases diagnosed as early stage disease. For those women without prohibiting clinical or cosmetic concerns, a choice between breast-conserving surgery and mastectomy can be made. Either choice confers equivalent survival. BACKGROUND: Multiple treatment options, combined with disease heterogeneity, have created nursing challenges in the management of adverse events (AEs) during antilymphoma therapy. Testing has revealed that less than half of participating nurses correctly graded peripheral neuropathy and neutropenia related to antilymphoma regimens.
