The purpose of this article is to assist nurse practitioners (NPs) and other primary care providers in differentiating between lactose intolerance, celiac disease, and diarrhea-predominant irritable bowel syndrome in adults. Based on subtle characteristics gathered from the history and physical examination, the NP’s examination and approach to testing will help distinguish between the 3 conditions. NPs should use a sequential process of examination and testing to distinguish gastrointestinal disorders that share common symptoms. A best practice algorithm is provided.

Burnout and compassion fatigue among critical care nurses can affect individual patients, healthcare systems, and nurses’ health and wellbeing. Both burnout and compassion fatigue have been well described in the literature as key elements that contribute to a nurse’s overall professional quality of life, and can be directly influenced by the health of the nurses’ work environment. Chronic dialysis patients are dependent on a hemodialysis machine to cleanse their blood of waste that is normally excreted by healthy functioning kidneys. The average number of hemodialysis treatments required per week are three with an average treatment time being three to four hours per treatment. Discontinuation of hemodialysis will result in death. The only way to eliminate the need for hemodialysis is a kidney transplant (National Kidney Foundation, 2008, 2013). Hemodialysis is not without its inherent risks. Background

Parents of hospitalized children, especially parents of children with complex and chronic health conditions, report not being adequately prepared for self‐management of their child's care at home after discharge.

Problem

No theory‐based discharge intervention exists to guide pediatric nurses' preparation of parents for discharge.

Purpose

To develop a theory‐based conversation guide to optimize nurses' preparation of parents for discharge and self‐management of their child at home following hospitalization.

Androgen deprivation therapy (ADT) results in testosterone suppression, a hypothesized mechanism linking ADT to depressive symptoms. This study investigated the relationship between ADT and the risk of subsequently being diagnosed with depressive disorder (DD) during a 3-year follow-up period. The patient sample for this population-based, retrospective cohort study was retrieved from the Taiwan Longitudinal Health Insurance Database 2005. We included all 1714 patients aged over 40 years with a first-time diagnosis of prostate cancer (PC) during 2001 to 2010 who did not have an orchiectomy. Background: Nurses working in intensive care units (ICUs) frequently care for patients and their families at the end of life (EOL). Providing high-quality EOL care is important for both patients and families, yet ICU nurses face many obstacles that hinder EOL care. The prevalence of peanut allergies (PAs) continues to rise through recent decades, despite the best attempts to reverse that trend. PAs are unpredictable and can be life-threatening. Therefore, it is imperative that nurse practitioners (NPs) are fully aware of the most recent guidelines and evidence regarding diagnosis, treatment, and prevention of PAs. This article presents information on current research in diagnosis and treatment of PA, as well as the latest guidelines established to prevent PA development. NPs should understand this information, allowing them to provide the best care possible for their patients.

This study evaluated the potential impact of an online spiritual care educational program on pediatric nurses’ attitudes toward and knowledge of spiritual care and their competence to provide spiritual care to children with cancer at the end of life. It was hypothesized that the intervention would increase nurses’ positive attitudes toward and knowledge of spiritual care and increase nurses’ level of perceived spiritual care competence. Background/Aims: Performing endoscopic retrograde cholangiopancreatography (ERCP) in patients who have undergone Roux-en-Y gastric bypass (RYGB) is challenging. Standard ERCP and enteroscopy-assisted ERCP are associated with limited success rates. Laparoscopy- or laparotomy-assisted ERCP yields improved efficacy rates, but with higher complication rates and costs. We present the first multicenter experience regarding the efficacy and safety of endoscopic ultrasound (EUS)-directed transgastric ERCP (EDGE) or EUS. Methods: All patients who underwent EDGE at two academic centers were included. Clinical success was defined as successful ERCP and/or EUS through the use of lumen-apposing metal stents (LAMS). Heart disease remains the number one killer of men and women globally, yet women have traditionally received less research attention in the cardiovascular literature, and an androcentric bias has persisted. Specifically, symptoms of myocardial infarction (MI) have been shown to be varied between men and women. Feminist standpoint theory is one perspective that researchers can use to explore the situated knowledge that women possess, given their unique experiences. Pregnancy weight gain is believed to contribute to female overweight and obesity. However, most studies do not account for the changes in body weight expected to occur as women age. We examined the long-term weight trajectory of childbearing women relative to weight progression that could be expected in the absence of pregnancy.

Methods

From the hospital records of 32,187 women with two births in Wisconsin during 2006 to 2013, we extracted the maternal weight at pregravid, delivery, and subsequent pregravid. Background: Timing of birth is a major determinant of newborn health. African American women are at increased risk for early birth, particularly via the inflammatory pathway. Variants of the IL1RN gene, which encode the interleukin-1 receptor antagonist (IL-1Ra) protein, are implicated in early birth. The biological pathways linking these variables remain unclear. Evidence also suggests that inflammatory pathways differ by race; however, studies among African American women are lacking.

Obesity has reached worldwide epidemic proportions and is associated with the leading causes of death. A person’s predisposition to obesity is strongly related to genetics and specific genes have been identified that influence weight control. The aim of this quantitative retrospective chart review is to identify the impact of behavioral genetics on weight loss maintenance following a medically supervised very low calorie diet. There is increasing interest in the phenomena of international student mobility and the growing global demand for skilled nurses. Little is known, however, about the learning experiences of Chinese nursing students at Australian universities. This study begins to address this gap. A narrative inquiry methodology was employed. In-depth interviews and focus group discussions, along with field notes and observations were conducted with six Chinese undergraduate nursing students studying undergraduate nursing in Western Australia. Chinese nursing students in Australia experienced fear and anxiety, driven by unfamiliarity with the hospital environment, education methods, and assessment expectations. Background: Significant reduction in the global burden of neonatal mortality was achieved through the millennium development goals. In Nigeria, however, only a marginal reduction was realized. This study assesses the rural–urban differences in neonatal mortality rate (NMR) and the associated risk factors in Nigeria.

Methods: The dataset from the 2013 Nigeria demographic and health survey (NDHS), disaggregated by rural–urban residence (n = 20 449 and 9935, respectively), was explored using univariate, bivariate, and multivariable analysis.
