The purpose of this project was to highlight an intervention to increase breastfeeding initiation and exclusive breastfeeding during the birth hospitalization in a coastal mid-Atlantic inner city hospital. Although researchers and national standards promote early initiation and exclusive breastfeeding, there continues to be a significant number of women who do not breastfeed and/or supplement with formula. The advantages of breastfeeding for mother and infant are substantial and include protecting babies from allergens to reducing maternal breast and ovarian cancer. Ineffective transitional care programs for ensuring the continuation of care from the hospital setting to the home setting often result in rehospitalization for elderly heart failure patients age 65 and older. The purpose of this project was to develop a home health transitional care program for elderly patients transitioning from inpatient settings to home settings using care bundles consisting of evidence-based practices to reduce preventable rehospitalizations within 30 days of discharge. The home-based chronic care model, which provides a foundation for home health's integral role in chronic disease management by ensuring patient-centered evidence-based care, guided the development of this program. …

Current practice behaviors for obesity management have remained refractory. Research has shown that lack of time and poor training/skills by primary care providers has led to poor or absent obesity counseling in practice. The purpose of this intervention was to facilitate obesity counseling in a pediatric office by using an evidence-based model called the 5As model of behavioral change. Gestational diabetes mellitus occurs in up to 10% of pregnancies and often leads to labor and delivery complications for both the mother and the baby. Early identification of gestational diabetes and educational intervention are needed to improve the self-management and knowledge among pregnant women. The purpose of the project was to implement newly established national guidelines to ensure that women with gestational diabetes are identified during the first trimester of pregnancy and begin diabetes education early in gestation. Current literature indicates that holistic spiritual care is important to the healthcare of patients. Research has shown that telephone drug reminder scripts can be an effective way to increase drug adherence and decrease health care costs for persons with Type 2 diabetes. The purpose of this project was to develop and validate a telephone drug reminder script for Type 2 diabetes patients based on evidence-based practice. Background. Collaboration between occupational therapists and nurses is key to a positive prognosis for their patients. Currently, there is a gap in the research on professional relationships between occupational therapists and registered nurses in acute care settings. Purpose. To examine interprofessional collaboration between registered nurses and occupational therapy in an acute care setting. Methods. A phenomenological, qualitative design with use of semi-structured interviews was used. วัตถุประสงค์: เพื่อเปรียบเทียบพฤติกรรมการป้องกันโรคกระดูกพรุน และการสลายมวลกระดูกในสตรีวัยก่อนหมดประจำเดือนก่อนและหลังได้รับกิจกรรมการประยุกต์ใช้ทฤษฎีขั้นตอนการเปลี่ยนแปลงพฤติกรรมและระหว่างกลุ่มทดลองและกลุ่มควบคุม\n\nรูปแบบการวิจัย: การวิจัยกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ สตรีวัยก่อนหมดประจำเดือนอายุ 30-45 ปี ที่อยู่ในระยะพฤติกรรมพร้อมที่จะปฏิบัติ ที่มีคุณสมบัติตามเกณฑ์ที่กำหนด จำนวนกลุ่มละ 30 คน กลุ่มทดลองได้รับกิจกรรมการประยุกต์ใช้ทฤษฎีขั้นตอนการเปลี่ยนแปลงพฤติกรรม ใน 2 สัปดาห์แรก และวัดผลหลังการทดลองในสัปดาห์ที่ 14 กลุ่มควบคุมได้รับการดูแลตามปกติ เก็บรวบรวมข้อมูลโดยใช้ แบบบันทึกข้อมูลส่วนบุคคล แบบบันทึกค่าสลายมวลกระดูก และแบบสอบถามพฤติกรรมการป้องกันโรคกระดูกพรุน มีค่าสัมประสิทธิ์แอลฟ่าของครอนบาคเท่ากับ .80 วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา และสถิติทดสอบค่าที\n\nผลการวิจัย: 1) สตรีวัยก่อนหมดประจำเดือนหลังเข้าร่วมโครงการมีคะแนนพฤติกรรมการป้องกันโรคกระดูกพรุน (x̄ = 68.73, SD = 4.59)สูงกว่าก่อนเข้าร่วมโครงการ (x̄ = 64.67, SD = 5.83) และมีค่าการสลายมวลกระดูก (x̄ = 0.233, SD = 0.05) น้อยกว่าก่อนเข้าร่วมโครงการ (x̄ = 0.289, SD = 0.07) อย่างมีนัยสำคัญทางสถิติ (p<.001) 2) สตรีวัยก่อนหมดประจำเดือนที่เข้าร่วมโครงการมีค่าเฉลี่ยคะแนนพฤติกรรมการป้องกันโรคกระดูกพรุน (x̄ = 68.73, SD = 4.59) ไม่แตกต่างจากกลุ่มควบคุม (x̄ = 66.63, SD = 6.65) (p>.05) แต่มีค่าเฉลี่ยการสลายมวลกระดูก (x̄ = 0.233, SD = 0.05) น้อยกว่ากลุ่มควบคุม (x̄ = 0.296, SD = 0.08) อย่างมีนัยสำคัญทางสถิติ (p<.001)\n\nสรุป: พยาบาลที่ปฏิบัติงานในโรงพยาบาลส่งเสริมสุขภาพตำบล ควรนำกิจกรรมการประยุกต์ใช้ทฤษฎีขั้นตอนการเปลี่ยนแปลงพฤติกรรม มาใช้เป็นแนวทางส่งเสริมสุขภาพเพื่อป้องกันโรคกระดูกพรุนและชะลอการสลายมวลกระดูกในสตรีวัยก่อนหมดประจำเดือน\n

วัตถุประสงค์: เพื่อเปรียบเทียบความพึงพอใจในบริการของผู้ป่วยมะเร็งเต้านมกลุ่มที่ได้รับการดูแลโดยใช้รูปแบบการจัดการผู้ป่วยรายกรณีกับกลุ่มที่ได้รับการดูแลตามปกติ และเปรียบเทียบการรับรู้คุณค่าวิชาชีพพยาบาลของพยาบาลประจำการระหว่างกลุ่มที่ใช้รูปแบบการจัดการผู้ป่วยรายกรณีกับกลุ่มที่ใช้การดูแลตามปกติ\n \nรูปแบบการวิจัย: แบบกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยมะเร็งเต้านม จำนวน 60 คน และพยาบาลประจำการ หอผู้ป่วยศัลยกรรมทั่วไปหญิง โรงพยาบาลจุฬาลงกรณ์ จำนวน 20 คน เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย แผนการอบรม แผนการดูแลผู้ป่วยมะเร็งเต้านม และคู่มือการจัดการผู้ป่วยรายกรณี เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามความพึงพอใจในบริการของผู้ป่วย และแบบสอบถามการรับรู้คุณค่าวิชาชีพพยาบาล เครื่องมือทั้งหมดได้รับการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ แบบสอบถามมีค่าสัมประสิทธิ์แอลฟ่าของครอนบาคเท่ากับ .95 และ .96 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน การทดสอบสถิติที และการทดสอบ Wilcoxon Rank Sum Test\n\nผลการวิจัย: 1) ค่าเฉลี่ยคะแนนความพึงพอใจในบริการของผู้ป่วยมะเร็งเต้านมกลุ่มที่ได้รับการดูแลโดยใช้รูปแบบการจัดการผู้ป่วยรายกรณี (x̄ = 4.38) สูงกว่ากลุ่มที่ได้รับการดูแลตามปกติ (x̄ = 3.37) อย่างมีนัยสำคัญทางสถิติระดับที่ .05 2) ค่าเฉลี่ยลำดับที่ของคะแนนการรับรู้คุณค่าวิชาชีพพยาบาลของพยาบาลประจำการที่ใช้รูปแบบการจัดการผู้ป่วยรายกรณี (x̄ = 4.74) สูงกว่ากลุ่มที่ใช้การดูแลตามปกติ (x̄ = 4.01) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n\nสรุป: รูปแบบการจัดการผู้ป่วยรายกรณีส่งผลให้ผู้ป่วยมะเร็งเต้านมมีความพึงพอใจในบริการและพยาบาลประจำการมีการรับรู้คุณค่าวิชาชีพพยาบาลมากขึ้น\n

This study reports the use of BL mode in one of the courses entitled Advanced Concepts of Community Health nursing (ACCHN), offered for undergraduate nursing students, for the first time in Karachi Pakistan. วัตถุประสงค์: เพื่อศึกษาประสิทธิผลของหุ่นจำลองทางการแพทย์เพื่อการฝึกกำหนดตำแหน่งอิเล็กโทรดในการใช้คลื่นไฟฟ้าหัวใจแบบ 12 ลีดมาตรฐานต่อความรู้และความพึงพอใจสำหรับนักเรียนพยาบาล\n\nรูปแบบการวิจัย : การวิจัยแบบก่อนทดลอง\n\nวิธีดำเนินการวิจัย : กลุ่มตัวอย่าง คือ นักศึกษาพยาบาลหญิงชั้นปีที่ 2 จำนวน 68 คน เครื่องมือที่ใช้ในการทดลอง คือ หุ่นจำลองทางการแพทย์เพื่อการฝึกกำหนดตำแหน่งการติดอิเล็กโทรดในการวัดคลื่นไฟฟ้าหัวใจแบบ 12 ลีดมาตรฐานสำหรับนักเรียนพยาบาล เก็บรวบรวมข้อมูลโดยใช้แบบบันทึกข้อมูลส่วนบุคคล แบบทดสอบความรู้เรื่องการติดอิเล็กโทรดตรวจวัดคลื่นไฟฟ้าหัวใจชนิด 12 ลีดมาตรฐาน และแบบสอบถามความพึงพอใจในการฝึกติดอิเล็กโทรดตรวจวัดคลื่นไฟฟ้าหัวใจชนิด 12 Lead มาตรฐาน แบบสอบถามได้ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 1 คน วิเคราะห์ข้อมูลโดยคำนวณหาค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และทดสอบสถิติที (Dependent t-test)\n\nผลการวิจัย: 1) นักศึกษาพยาบาลหญิงชั้นปีที่ 2 มีค่าเฉลี่ยคะแนนความรู้หลังการทดลอง (x̄ = 23.16, SD = 3.65) มากกว่าก่อนการทดลอง (x̄ = 17.07, SD = 3.55) อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01 (t = -9.86, p < .01) 2) นักศึกษาพยาบาลหญิงชั้นปีที่ 2 มีค่าเฉลี่ยคะแนนความพึงพอใจโดยรวม (x̄ = 4.37, SD = 0.65) ต่อการฝึกติดอิเล็กโทรดตรวจวัดคลื่นไฟฟ้าหัวใจชนิด 12 Lead มาตรฐาน ทดลอง อยู่ในระดับ มากที่สุด (Excellent)\n \nสรุป: ผู้บริหารของคณะพยาบาลศาสตร์ควรสนับสนุนให้อาจารย์พยาบาลนำหุ่นจำลองทางการแพทย์เพื่อการฝึกกำหนดตำแหน่งอิเล็กโทรดในการใช้คลื่นไฟฟ้าหัวใจแบบ 12 ลีด ไปใช้ในการเรียนการสอนเนื่องจากมีประสิทธิผลให้คะแนนความรู้เพิ่มขึ้นและนักศึกษาพยาบาลมีความพึงพอใจต่อหุ่นจำลองมากที่สุด\n

วัตถุประสงค์: เพื่อศึกษาคุณภาพชีวิตและปัจจัยที่มีความสัมพันธ์กับคุณภาพชีวิตของผู้ป่วยภาวะเบาหวานขึ้นจอประสาทตา\n\nรูปแบบการวิจัย: การวิจัยแบบบรรยายเชิงความสัมพันธ์\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยภาวะเบาหวานขึ้นจอประสาทตา อายุ 20-59 ปี จำนวน 140 คน ซึ่งมารับการรักษาที่คลินิกจอประสาทตา โรงพยาบาลศิริราช โรงพยาบาลรามาธิบดี และโรงพยาบาลจุฬาลงกรณ์ เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคลแบบสอบถามคุณภาพชีวิต แบบสอบถามการสนับสนุนทางสังคม แบบสอบถามความวิตกกังวล และแบบสอบถามการจัดการตนเองของผู้ป่วยภาวะเบาหวานขึ้นจอประสาทตา แบบสอบถามทั้งหมดผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน มีค่าสัมประสิทธิ์อัลฟ่าของครอนบาคเท่ากับ .90, .90, .90 และ .71 ตามลำดับ วิเคราะห์ข้อมูลโดยหาค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์แบบอีต้า สเปียร์แมน และเพียร์สัน\n \nผลการวิจัย: 1. คุณภาพชีวิตโดยรวมของผู้ป่วยภาวะเบาหวานขึ้นจอประสาทตาอยู่ในระดับดีมาก (x̄ = 3.52, SD = 0.71) 2. เพศและอายุไม่มีความสัมพันธ์กับคุณภาพชีวิตของผู้ป่วยภาวะเบาหวานขึ้นจอประสาทตา (p > .05) 3. การมีภาวะแทรกซ้อนเรื้อรังโรคเบาหวานมีความสัมพันธ์กับคุณภาพชีวิตของผู้ป่วยภาวะเบาหวานขึ้นจอประสาทตาอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05(η = .52) 4. ระดับการมองเห็น ระดับความรุนแรงของภาวะเบาหวานขึ้นจอประสาทตา และความวิตกกังวลมีความสัมพันธ์ทางลบกับคุณภาพชีวิตของผู้ป่วยภาวะเบาหวานขึ้นจอประสาทตาอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (ρ = -.44, ρ = -.37 และ r = -.79 ตามลำดับ) 5. การสนับสนุนทางสังคมและการจัดการตนเองที่เกี่ยวกับโรคเบาหวานมีความสัมพันธ์ทางบวกกับคุณภาพชีวิตของผู้ป่วยภาวะเบาหวานขึ้นจอประสาทตาอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05(r = 0.72 และ r = 0.53 ตามลำดับ)\n\nสรุป: พยาบาลควรคำนึงถึงปัจจัยด้าน ภาวะแทรกซ้อนเรื้อรังโรคเบาหวาน ระดับการมองเห็น ระดับความรุนแรงของภาวะเบาหวานขึ้นจอประสาทตา ความวิตกกังวล การสนับสนุนทางสังคม และการจัดการตนเองที่เกี่ยวกับโรคเบาหวาน ในการวางแผนจัดกิจกรรมการพยาบาลให้แก่ผู้ป่วยภาวะเบาหวานขึ้นจอประสาทตาเพื่อส่งเสริมคุณภาพชีวิตผู้ป่วยต่อไป\n

วัตถุประสงค์: เพื่อวิเคราะห์ตัวแปรที่สามารถจำแนกการเลือกใช้บริการของผู้รับบริการโรงพยาบาลรัฐบาลและเอกชน กรุงเทพมหานคร\n \nรูปแบบการวิจัย: เป็นการวิจัยแบบบรรยาย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้รับบริการที่มารับการรักษาพยาบาลในโรงพยาบาล กรุงเทพมหานคร จำนวน 400 คน แบ่งเป็นกลุ่มผู้รับบริการที่มารับการรักษาพยาบาลในโรงพยาบาลรัฐบาลจำนวน 200 คน และโรงพยาบาลเอกชน จำนวน 200 คน คัดเลือกโดยการสุ่มตัวอย่างแบบแบ่งชั้น (Stratified random Sampling) เครื่องมือที่ใช้ในการเก็บข้อมูล คือ แบบสอบถามเหตุผลที่มาเลือกใช้บริการของผู้รับบริการโรงพยาบาลรัฐบาลและเอกชน กรุงเทพมหานคร ผ่านการตรวจสอบความตรงตามเนื้อหา และวิเคราะห์ค่าความเที่ยงของเครื่องมือโดยการคำนวณค่าสัมประสิทธิ์แอลฟ่าของครอนบาคมีค่าเท่ากับ .83 วิเคราะห์ข้อมูลโดยใช้สถิติวิเคราะห์จำแนกกลุ่ม (Discriminant analysis) ด้วยวิธีขั้นตอน (Stepwise method) และวิธี Enter\n\nผลการวิจัย: พบว่า มี 2 ตัวแปร ที่สามารถจำแนกการเลือกใช้บริการของผู้รับบริการโรงพยาบาลรัฐบาล ได้แก่ มีการเบิกค่ารักษาพยาบาลได้ และการแนะนำของบุคคลใกล้ชิด ส่วนตัวแปรจำนวน 6 ตัวแปร ที่สามารถจำแนกการเลือกใช้บริการของผู้รับบริการโรงพยาบาลเอกชน ได้แก่ คุณภาพการรักษาของแพทย์ มีระบบการให้บริการที่ดี พฤติกรรมบริการของพยาบาล มีการประชาสัมพันธ์โรงพยาบาลผ่านตามสื่อต่างๆ ราคาค่าใช้จ่ายเหมาะสมกับบริการที่ได้รับ และสถานที่ให้บริการ โดยตัวแปรทั้ง 8 ตัวแปร สามารถร่วมกันเป็นปัจจัยจำแนกการเลือกใช้บริการของผู้รับบริการโรงพยาบาลรัฐบาลและเอกชนได้ร้อยละ 99.25\n\nสรุป: ผู้บริหารการพยาบาลทั้งในโรงพยาบาลรัฐบาลและเอกชน สามารถนำผลการวิจัยไปใช้ในการวางแผนเพื่อพัฒนาคุณภาพการบริการเพื่อดึงดูดผู้รับบริการให้มาใช้บริการ\n

Clinical documentation is an essential quality element that is often not adequately completed. Current data demonstrate that 32% of home health claims do not meet the requirements for federal reimbursement.
