The Balanced Counseling Strategy Plus (BCS+) toolkit, developed and tested in Kenya and South Africa, provides information and materials that healthcare providers need to offer complete, high-quality family planning counseling to clients living in areas with high rates of HIV and STIs. Content is in accordance with the 2015 WHO Medical Eligibility Criteria. The BCS+ provides evidence on clinical indications for the provision of family planning methods, counseling cards, instructions for providers, and guidance for supplemental counseling and services that family planning clients may need. Maternal mortality remains a challenge in developing countries which bear 99% of global maternal deaths (WHO, 2014). Nigeria, India, Pakistan, Afghanistan and Ethiopia carry more than 50% of the global burden of maternal mortality. According to the UNDP Human Development Report (2014), Nigeria’s maternal mortality ratio of 630 per 100,000 live births ranks among the highest in Africa. We present the results of a double-blinded randomized sham-controlled research study of a non-pharmacologic low back pain intervention. Calmare therapy is an neurocutanous electrical stimulation approach for pain management. The intervention group received Calmare therapy and the other received sham. Differences in pain severity and interference scores, pain sensitivity measures and gene expression profiles are reported. Patterns of downregulated gene expression suggest that Calmare may alter proteins involved in pain transduction may have implications for the treatment of other chronic pain conditions.

The purpose of this dissertation was to evaluate outcomes associated with blood component (BC) transfusion in adult trauma patients. Preterm birth (PTB) and low birthweight (LBW) babies are the source of a large burden of infant, neonatal, and childhood morbidity. The purpose of this project was to expand the use of the CenteringPregnancyTM Group Prenatal Care Model as an evidence-based intervention for management of both medical and psychosocial risk in low-income, ethnic and racial minorities in New York City. วัตถุประสงค์: เพื่อเปรียบเทียบพฤติกรรมมารดาในการป้องกันภาวะตัวเหลืองจากการได้รับนมมารดาไม่เพียงพอ ระหว่างกลุ่มที่ได้รับการสอนแนะกับกลุ่มที่ได้รับการพยาบาลตามปกติ\n \nรูปแบบการวิจัย: การวิจัยแบบกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ทารกแรกเกิดครบกำหนดที่มีภาวะสุขภาพดี จากการวินิจฉัยของแพทย์ น้ำหนักมากกว่าหรือเท่ากับ 2,500 กรัม และมารดาทารกแรกเกิดคลอดครบกำหนด คลอดปกติ มีอายุครรภ์มากกว่า 37 สัปดาห์ ถึง 42 สัปดาห์ แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 20 คู่ โดยจับคู่ในด้าน ประสบการณ์การเลี้ยงดูทารกตัวเหลือง การสนับสนุนทางสังคม และระดับการศึกษา กลุ่มควบคุมได้รับการดูแลตามปกติ กลุ่มทดลองได้รับโปรแกรมการสอนแนะ ตามแนวคิดการสอนแนะของ Helfer and Wilson (1982) ที่ประกอบด้วยคู่มือแนวทางการดำเนินกิจกรรมการดูแลทารกแรกเกิดในการป้องกันภาวะตัวเหลือง ภาพพลิก และแผนการสอน เก็บรวบรวมข้อมูล โดยใช้ แบบสอบถามพฤติกรรมการป้องกันภาวะตัวเหลืองจากการได้รับนมมารดาไม่เพียงพอ ที่ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิจำนวน 5 คน มีค่าสัมประสิทธิ์แอลฟ่าของครอนบาค เท่ากับ .91 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติทดสอบที (Independent t-test)\n\nผลการวิจัย: คะแนนเฉลี่ยพฤติกรรมมารดาในการป้องกันภาวะตัวเหลืองจากการได้รับนมมารดาไม่เพียงพอในทารกแรกเกิดครบกำหนดของกลุ่มมารดาที่ได้รับโปรแกรมการสอนแนะสูงกว่ากลุ่มที่มารดาได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติ (p<.01)\n\nสรุป: โปรแกรมการสอนแนะเป็นกิจกรรมการพยาบาลที่มีประสิทธิภาพต่อพฤติกรรมของมารดาในการป้องกันภาวะตัวเหลืองจากการได้รับนมมารดาไม่เพียงพอในทารกแรกเกิดครบกำหนด\n

วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมสร้างการตระหนักรู้และส่งเสริมความมีคุณค่าในตนเองต่อพฤติกรรมป้องกันการมีเพศสัมพันธ์ของวัยรุ่นหญิงตอนต้น\n\nรูปแบบการวิจัย: การวิจัยแบบกึ่งทดลอง\n \nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ วัยรุ่นหญิงตอนต้นชั้นประถมศึกษาปีที่ 6 ในจังหวัดสุราษฎร์ธานี จำนวน 105 คน แบ่งเป็นกลุ่มทดลอง 51 คน และกลุ่มควบคุม 54 คน กลุ่มทดลองได้รับโปรแกรมสร้างการตระหนักรู้และส่งเสริมความมีคุณค่าในตนเอง เป็นเวลา2 สัปดาห์ จำนวน 4 ครั้ง กลุ่มควบคุมได้รับความรู้ตามปกติ เก็บข้อมูลโดยใช้แบบสอบถามพฤติกรรมป้องกันการมีเพศสัมพันธ์ ได้รับการตรวจสอบคุณภาพเครื่องมือโดยผู้ทรงคุณวุฒิจำนวน 5 คน ได้ค่าดัชนีความตรงตามเนื้อหา (CVI) เท่ากับ .84 ค่าสัมประสิทธิ์อัลฟ่าของครอนบาคเท่ากับ .83 เก็บข้อมูลก่อนการทดลองและหลังการทดลอง 4 สัปดาห์ วิเคราะห์โดยใช้สถิติพรรณนาและสถิติ t-test\n\nผลการวิจัย: 1) คะแนนเฉลี่ยพฤติกรรมป้องกันการมีเพศสัมพันธ์ของกลุ่มทดลองหลังการทดลอง 4 สัปดาห์ สูงกว่าก่อนการทดลองอย่างมีนัยสำคัญอย่างมีนัยสำคัญทางสถิติ (p<.05) 2) คะแนนเฉลี่ยพฤติกรรมป้องกันการมีเพศสัมพันธ์หลังการทดลอง 4 สัปดาห์ของกลุ่มทดลองสูงกว่ากลุ่มควบคุมอย่างมนัยสำคัญทางสถิติ (p<.05)\n\nสรุป: พยาบาลอนามัยชุมชนหรือพยาบาลโรงเรียนสามารถนำโปรแกรมไปประยุกต์ใช้เพื่อส่งเสริมพฤติกรรมป้องกันการมีเพศสัมพันธ์ในวัยรุ่นหญิงตอนต้น อันจะส่งผลลดการตั้งครรภ์และการติดเชื้อโรคติดต่อทางเพศสัมพันธ์ต่อไป\n

วัตถุประสงค์: เพื่อศึกษาความสัมพันธ์ระหว่าง ความปวด ความเหนื่อยล้า อาการหายใจลำบาก ความวิตกกังวล ภาวะซึมเศร้า และความต้องการการดูแลแบบสนับสนุนกับคุณภาพชีวิตของผู้ป่วยมะเร็งปอด และปัจจัยทำนายคุณภาพชีวิตของผู้ป่วยมะเร็งปอด\n\nรูปแบบการวิจัย: การวิจัยความสัมพันธ์เชิงทำนาย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยมะเร็งปอด จำนวน193 คน ที่มารับการรักษาที่แผนกผู้ป่วยนอกของโรงพยาบาลในเขตกรุงเทพมหานคร จำนวน 3 แห่ง เครื่องมือที่ใช้ในการเก็บข้อมูล คือ แบบบันทึกข้อมูลส่วนบุคคล แบบประเมินความปวด แบบประเมินความเหนื่อยล้า แบบประเมินอาการหายใจลำบากแบบประเมินอาการวิตกกังวลและซึมเศร้าแบบประเมินความต้องการการดูแลแบบสนับสนุน และแบบประเมินคุณภาพชีวิต ซึ่งผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ และมีค่าสัมประสิทธิ์แอลฟ่าของครอนบาค เท่ากับ .89, .92, .71, .83, .94 และ .92 ตามลำดับ วิเคราะห์ข้อมูลโดยหาค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน และการวิเคราะห์ถดถอยพหุคูณแบบขั้นตอน\n \nผลการวิจัย: 1) คุณภาพชีวิตของผู้ป่วยมะเร็งปอด อยู่ในระดับสูง (x̄ = 73.01, SD = 15.44) 2) ความเหนื่อยล้า อาการหายใจลำบาก ความวิตกกังวล ภาวะซึมเศร้า และความต้องการการดูแลแบบสนับสนุน มีความสัมพันธ์ทางลบกับคุณภาพชีวิตของผู้ป่วยมะเร็งปอดอย่างมีนัยสำคัญทางสถิติที่ระดับ .05แต่ความปวดไม่มีความสัมพันธ์กับคุณภาพชีวิตของผู้ป่วยมะเร็งปอด 3) ภาวะซึมเศร้า (β = -.52) อาการหายใจลำบาก (β = -.28) ความเหนื่อยล้า(β = -.30) และความต้องการการดูแลแบบสนับสนุน (β = -.14)เป็นปัจจัยทำนายคุณภาพชีวิตผู้ป่วยของมะเร็งปอด ได้อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 โดยสามารถอธิบายความแปรปรวนได้ร้อยละ 43 (R2 = .43)\n \nสรุป: องค์ความรู้ที่ได้จากงานวิจัย สามารถนำไปพัฒนาเป็นโปรแกรมการดูแลผู้ป่วยมะเร็งปอด เพื่อให้ผู้ป่วยมีคุณภาพชีวิตที่ดีโดยมุ่งเน้นการดูแลเพื่อจัดการกับกลุ่มอาการและตอบสนองความต้องการที่เกิดขึ้น \n

Background: Computerized infusion devices called smart infusion pumps or smart pumps are clinical tools capable of decreasing IV medication infusion calculation errors through the use of embedded drug libraries. These devices are now important tools contributing to a “culture of safety” defined by the Joint Commission. Background: Current management of the second stage of labor often follows tradition-based routines rather than evidence-based practices. A lack of situational awareness and tunnel vision can limit medical decision-making. Northern New England Perinatal Quality Improvement Network (NNEPQIN) has listed Second Stage Situational Awareness as a priority initiative. Standardized checklists are useful for maintaining situational awareness. Regular debriefings using a standardized tool have been shown to improve communication and team based care, which generally leads to improved patient outcomes. The number of medication errors associated with preventable deaths in healthcare facilities remains at a high rate for healthcare leaders. Practices of medication delivery remain similar to those 10 years ago. Hospitals that have implemented barcoding medication administration systems have reported a decrease in medication errors ranging from 60% to as high as 93%. Despite this utility of barcoding, only 50% of U.S. hospitals have implemented barcode medication administration. The current study aims to assess tracheal suctioning practices among health care practitioners; nurses, critical care technicians and physiotherapist. Employing literature review as the methodology, multiple databases were searched focusing on three phases of tracheal suctioning (a) the pre suctioning phase, (b) the suctioning phase, (c) the post suctioning phase and complications related to tracheal suctioning. For most of the 20th century, the saying “once a cesarean, always a cesarean” was a rule in the United States. Today, the National Institutes of Health (NIH) opposes the dictum and urges women to consider trial of labor after cesarean (TOLAC). However, the factors that lead to a successful outcome remain unclear, as research continues to be conducted in hopes of creating a predictive model for vaginal birth after cesarean (VBAC) success.
