Nursing is a discipline in transition. As the complexity and acuity of patients increase, nurses are taking on a more comprehensive role in health care leadership and patient outcomes. As the discipline has evolved so has the curricular framework of nursing educational programs, moving from being based on a specific nursing theory, to a general metaparadigm, to the current focus on meeting curricular content standards developed by national accrediting agencies. When considering the skills needed to fully engage in critical thinking and patient advocacy there may be room for an additional curricular focus: that of metacognitive development based on critical …

Preconception Care For Women With Pregestational Diabetes: Information For The Family Nurse Practitioner, Kathryn Conover Morrill

Purpose: The purpose of this article is to evaluate present literature and guidelines regarding preconception care for women with type I diabetes mellitus (T1DM) or type II diabetes mellitus (T2DM) and familiarize the family nurse practitioner (FNP) with current clinical recommendations. Data Sources: The guidelines reviewed include the Clinical Practice Recommendations from the American Diabetes Association 2013, the American College of Obstetricians and Gynecologists pregestational diabetes mellitus guideline reaffirmed in 2010, and the National Institute for Health and Clinical Excellence on how to manage diabetes and its complications from preconception to the postnatal period 2008. Aim: We aimed to explore links between heart rate variability (HRV) and clinical depression in patients with acute coronary syndrome (ACS), through a review of recent clinical research literature.

Background: Patients with ACS are at risk for both cardiac autonomic dysfunction and clinical depression. Both conditions can negatively impact the ability to recover from an acute physiological insult, such as unstable angina or myocardial infarction, increasing the risk for adverse cardiovascular outcomes. HRV is recognized as a reflection of autonomic function.

Purpose: The purpose of this study is to examine the effect of a PPM on clinical nurses’ perceptions of their practice environment and job satisfaction. Secondly, this study examined clinical nurses’ perceptions of the PPM through survey questions focusing on elements of the PPM.

Design: A descriptive, pretest-posttest design using secondary data to analyze nurses’ perception of their practice environment and job satisfaction before and after implementation of the healthcare organizations PPM. Also, a prospective design using a survey method was used to measure nurses’ perceptions of the PPM.

Obesity is an epidemic and continues to rise. The associated increased risk of mortality and co-morbid conditions is well documented. The use non-nutritive sweeteners (NNS) is a strategy being used to manage weight by reducing calorie intake. Research is inconclusive regarding the efficacy of NNS in weight management due to their inappropriate use and also the potential they may increase appetite. To most effectively manage weight, a comprehensive individualized plan must be devise of which proper use NNS may be a part.

The high cost of medications is one reason that patients do not adhere to treatment plans. Background: There continues to be a need for increases in adult vaccination rates, especially among those working in environments which may easily become communicable disease outbreak centers, such as school employees in the school environment. The purpose of this study was to evaluate why rural Utah school employees were non-compliant with the influenza and measles, mumps, and rubella (MMR) vaccines, as well as to identify their views on mandatory vaccination policies.

This project seeks to prove the practicality of using Reninjazas (traditional birthing attendants) in rural Madagascar to better prenatal care and diminish the maternal and infant mortality rates in this country. Prenatal care is of vital importance to expecting mothers. Without it, birth defects, complicated labor and delivery, miscommunications concerning fetal development, and even death can occur (Mayo Clinic, 2014). Superior skin care is a nurse-sensitive outcome measure (Young, T., & Clark, M. (2009). Re-positioning for pressure ulcer prevention (protocol). The Cochrane Database of Systematic Review (2009) 3). This study sought to decrease incidence and risk of skin breakdown in the pediatric cardiac intensive care unit (PCICU). The study was conducted in a large hospital in upstate New York. A practice guideline was created and guided PCICU nurses on the interventions for potential skin-breakdown issues in their patients. The patients had a significant change in skin breakdown with a one-sided Fisher's Exact Test (p=.0422). A logistic regression model showed intervention …

Each year approximately 210 million pregnancies occur worldwide and more than one-third (75 to 80 million) of these are unintended, more than half of these (46 million) unintended pregnancies undergo unsafe abortions. In Pakistan 16% of the births are reported to be unwanted and if the unwanted fertility is eliminated, total fertility in Pakistan would be 3.1 births per woman. Total demand for family planning in Pakistan is 55%, while the contraceptive prevalence rate is 35% and unmet need for family planning is 20%. Purpose

The purposes of this descriptive study were (a) to identify the relative frequencies of a priori categories of phantom limb pain (PLP) quality descriptors reported by Mid-Southerners with limb loss, (b) to analyze their descriptions for emerging categories of PLP, and (c) to identify the relative frequencies of the emerging categories.

Design

This cross-sectional descriptive verbal survey assessed PLP descriptors. A content analyses determined relative frequencies of a priori PLP descriptors as well as emerging categories that were identified.

Findings

It's now a common practice for hospital-owned mobile phones to be shared among healthcare employees from shift to shift. Despite the benefit of increased, timely communication between caregivers, sharing mobile devices can lead to the spread of healthcare-associated infections (HAIs) if they aren't properly disinfected. The Guidelines for Disinfection and Sterilization in Healthcare Facilities describe "non-critical environmental surfaces" as items that are frequently touched by the hand and may pose a risk of secondary infection transmission.
