Bolstering Moral Distress Knowledge, Ethical Decision-Making Confidence, and Moral Courage Among Nurse Leaders

Background

Nurse leaders report low confidence navigating ethical dilemmas. Ethical dilemmas arise when ethical values are challenged, causing uncertainty about how to proceed. Left unresolved, ethical dilemmas impact ethical decision-making and can lead to moral distress. Moral distress is the anguish that occurs when there is recognition of an ethical dilemma necessitating a professional obligation to act, yet barriers exist to taking the ethically correct course of action.

Background:

Nitrous oxide is an inexpensive and fast-acting gas used in a variety of inpatient and outpatient perioperative settings to hasten the onset of and emergence from general anesthesia. Problematically, nitrous oxide has been identified as a risk factor for postoperative nausea and vomiting (PONV), one of the most common adverse events after anesthesia. Unresolved PONV is linked to increased pain, wound dehiscence, aspiration, and longer stays in the recovery area and/or hospital. An Apfel Simplified Risk Score can be calculated to predict PONV based on four patient characteristics: female sex, non-smoker, history of PONV or motion sickness, and …

Background

Residual neuromuscular blockade increases morbidity and mortality following surgery with general anesthesia and paralysis. Optimal antagonism of pharmacologic muscle relaxation is critical to avoiding residual weakness and subsequent pneumonia. The clinical question that guided the investigation of the evidence was, ‘Among healthy adults requiring neuromuscular blockade during general anesthesia for elective, noncardiac surgery, does the use of sugammadex compared to neostigmine for reversal of neuromuscular blockade influence the rate of postoperative pneumonia within seven days?’

Background: Healthcare organizations pursuing the American Nurse Credentialing Center (ANCC) Magnet® Award must assess the educational needs of Registered Nurses and develop relevant educational offerings that positively affect patient outcomes. Challenges include collecting survey response rates, report build time, and accessible of reports to those responsible for the education. Four hospitals in the Pacific Northwest used electronic workflows to disseminate the educational needs of acute care nurses by unit, specialty, and role with improved analysis of data. Incorporating a learning platform for survey distribution increased the response rate from 15% to 70% over 3 years. The valid response rate …

Title: Demographic differences between nurses who provide a gut microbiome sample for research and those who do not

Background: In the United States (U.S.), there are over 4.1 million registered nurses and 60% work in acute care to treat hospitalized patients. On average, the nursing workforce is female, Caucasian, and approximately 49 years old. Only 60% of U.S. nurses report a bachelor’s degree education or higher. Nursing night shift work is essential in the hospital yet is linked to negative outcomes such as cardiometabolic illnesses including type 2 diabetes. Short-term studies (lasting a few days) of simulated night shift work have identified night-time decreases in insulin sensitivity and glucose tolerance as potential causes of negative cardiometabolic health outcomes.
