Home healthcare agencies transition patients from the hospital to the home where caregivers may assist in providing care for indwelling Foley catheters. About half of urinary tract infections are in the presence of an indwelling catheter. This doctor of nursing practice (DNP) project presents an organizational quality improvement initiative implementing a text message reminder system to decrease appointment no-show rates in the outpatient mental health clinic. The project was developed based on the high rate of appointment no-shows at the DNP project site. Appointment no-shows are costly to both clinics and patients. Clinics experience decreased revenue and efficiency, and patient outcomes can be compromised. The project evaluated whether text message reminders sent 24 hours before a scheduled appointment time decreased no-show rates by at least 10%. Depression represents a significant public health concern that often remains undiagnosed or untreated, primarily due to insufficient systematic screening in primary care settings. The lack of standardized screening tools such as the Patient Health Questionnaire-9 (PHQ-9) elevates patient risks, increasing symptom severity, increasing suicide rates, and delaying interventions. Opioid misuse and overdose continue to pose a significant public health crisis in the United States, particularly among patients with a history of substance use disorder (SUD). Approximately 40% of the clinic’s patient population exhibited aggressive behaviors, including verbal threats, and physical altercations. However, nurses primarily relied on pharmacological interventions and physical restraints to manage these behaviors, which failed to address underlying causes, such as psychiatric conditions and environmental stressors. Summary This doctoral project developed a clinical practice guideline (CPG) to address the critical health issue of early identification of delirium among hospitalized high-risk patients. Hospital delirium is associated with prolonged hospital stay, increased nursing home or rehab use after hospitalization, and higher mortality. This Doctor of Nursing Practice project addressed the underutilization of heat therapy for lower back pain management at an outpatient clinic through an educational intervention aimed at enhancing multidisciplinary team knowledge. My project implemented a comprehensive educational program for 12 healthcare providers. The intervention included structured learning sessions with didactic presentations, case discussions, and hands-on demonstrations. Pre- and post-intervention assessments evaluated provider understanding of heat therapy's physiological mechanisms, clinical applications, safety considerations, and treatment protocols. Analysis revealed significant improvements in provider knowledge, with mean scores increasing from 33.33% pre-intervention to 92.25% post-intervention (p<0.001). Recommendations included integrating regular educational sessions into organizational development programs, developing standardized protocols, and creating patient education materials. The project demonstrated the effectiveness of structured educational interventions in improving provider knowledge while supporting broader implementation of non-pharmacological pain management strategies.
