A primary care medical clinic experienced an increase in hospital readmission for hypertensive patients, as evidenced by a consistent lack of standardized patient education on medication adherence and a notable increase in readmission rates for uncontrolled hypertension. At the practice site, older residents face inconsistent and untimely blood sugar assessments, contributing to avoidable complications such as hypoglycemia, hyperglycemia, and hospitalizations. Many caregivers lack standardized training in blood glucose monitoring, which poses a significant risk to residents with diabetes or prediabetes. This staff education project focused on the practice problem of a knowledge deficit of licensed vocational nurses (LVNs) related to evidence-based medication adherence strategies for psychiatric patients. The practice-focused question addressed how effective an education would be for improving medication adherence knowledge, confidence, and practice readiness among LVNs in an outpatient psychiatric clinic. Objective: Will a central line and peripherally inserted central line reeducation program increase the nurses’ knowledge pertaining to dressing changes, cap changes, and chlorohexidine baths? Method: 26 nurses on an observation unit were sent a power point presentation. Of the 26 nurses that received the power point 18 nurses participated. The power point presentation had a pretest and posttest embedded into it. Both the pretest and posttest consist3ed of the same 10 questions. Its goal was to enhance providers' ability to prescribe and manage CGMs effectively. A review of existing literature and a provider survey identified key barriers, such as limited time and insufficient provider education on CGMs. The survey emphasized educational needs related to initiating and prescribing CGMs. Educational materials, including a PowerPoint presentation on prescribing, monitoring, and management, were developed and shared with clinic staff. Summary This project is a Doctor of Nursing Practice initiative to educate healthcare professionals at a primary healthcare clinic. The focus was on improving staff knowledge of hypertensive crises in African American patients aged 20 to 40. The project included face-to-face training sessions and educational resources to empower providers and enhance patient outcomes. Hypertensive crises are a significant health risk, especially in African Americans. This group has higher rates of hypertension, leading to serious health complications.
