Conclusions:

Direct observation and the use of the observational instruments are not clinically useful in predicting milk intake in preterm infants.

PHA, PNA, and BFE should not be used arbitrarily as predictor variables

Test weighing is the most reliable method to measure milk intake.

The PIBBS is a reliable instrument when used by trained lactation specialists but not predictive of milk intake in preterm infants for the volume of milk intake in preterm infants.

Subglottic Secretion Aspiration In The Prevention Of Ventilator-Associated Pneumonia: A Review Of The Literature., Rachel Scherzer

Ventilator-associated pneumonia is a common nosocomial infection that results in both negative patient outcomes and increased health care costs. Recently, many efforts have been targeted at ventilator-associated pneumonia prevention, including the practice of subglottic secretion aspiration. Six randomized control studies examining the effectiveness of subglottic secretion aspiration in the prevention of ventilator-associated pneumonia were reviewed for this article. Results consistently show that subglottic secretion aspiration significantly reduces the incidence of ventilator-associated pneumonia in a variety of patient populations. Purpose/Objectives: To establish initial reliability and validity of a Web-based survey focused on oncology advanced practice nurses' (APNs') knowledge, attitudes, and practice behaviors regarding advanced care planning, and to obtain preliminary understanding of APNs' knowledge, attitudes, and practice behaviors and perceived barriers to advanced care planning.

Design: Descriptive, cross-sectional, pilot survey study.

Setting: The eastern United States.Sample: 300 oncology APNs.Methods: Guided by the Theory of Planned Behavior, a knowledge, attitudes, and practice behaviors survey was developed and reviewed for content validity. Executive Summary:

On March 1-2, 2010, a state-of-the-science invitational conference titled "Ambulatory Care Registered Nurse Performance Measurement" was held to focus on measuring quality at the RN provider level in ambulatory care.

The conference was devoted to ambulatory care RN performance measurement and quality of health care.

The specific emphasis was on formulating a research agenda and developing a strategy to study the testable components of the RN role related to care coordination and care transitions, improving patient outcomes, decreasing health care costs, and promoting sustainable system change.

Abstract

Purpose: To describe a project that introduces middle school and high school students living in Pennsylvania’s rural geographic regions to nursing careers through outreach extended to students regardless of gender, ethnicity, or socioeconomic status.

Method: The authors employed many strategies to inform students about careers in nursing. The methods included: working with guidance counselors, participating in community health fairs, taking part in school health career fairs, collaborating with Area Health Education Centers, serving on volunteer local education advisory boards, developing a health careers resource guide, and establishing a rural health advisory board.

Executive Summary:

Ambulatory care faces challenges in sustaining a nursing workforce in the future as newly licensed nurses are heavily recruited to inpatient settings and retirements will impact ambulatory care sooner than other areas.

Building a diverse team by recruiting nurses of different ages (generations) and skills may result in a more successful and robust organization.

Knowledge about generational characteristics and preferences will aid nurse leaders and recruiters in attracting high-quality, talented nurses.

Nurses of Generations X and Y can increase their likelihood of success in ambulatory care by better understanding intergenerational issues.

The clinical use of automated implantable cardioverter defibrillators (AICDs) has been rapidly increasing since the results of several randomized trials confirmed the efficacy of AICDs in the secondary and primary prevention of sudden cardiac death. Patients with AICDs require high-quality care and intense follow-up to ensure safe and effective device performance. According to international guidelines these patients should be followed at 1- to 4 month intervals, depending on the device model and the patient’s clinical status (Schoenfeld, 2004). The need for evidence based teaching has become an important ideology for nurse educators who are frequently encouraged to expand their teaching strategies based on recent advances in technology and student learning styles. Traditional lecture is often preferred by students, yet the literature encourages case study methodology for the development of critical thinking. A pilot study was conducted comparing learning outcomes using two different teaching strategies: lecture and case study instruction. Recommendations for using case study as a teaching methodology are offered.

Delirium is a serious public health problem among older adults and is one of the most common cognitive disorders affecting postoperative older adults. Delirium is a treatable illness that disproportionately places postoperative older adults at increased risk for functional and cognitive decline, as well as increased morbidity and mortality. More than 50 percent of postoperative patients develop delirium during hospitalization. Epidemiologic and clinical studies over the past two decades have provided ample evidence that delirium in hospitalized older adults is under-recognized by nurses and physicians, often leading to untoward events that the safety and well-being of hospitalized older adults.

Vitamin D deficiency is common in the general public and in patients with cancer. Optimizing vitamin D intake is increasingly recognized in cancer risk reduction, particularly in decreasing colorectal cancer risk. Therefore, summarizing the current evidence to promote best practices related to vitamin D intake and colorectal cancer risk reduction is important. The objectives of this article are to examine the current evidence regarding the impact of vitamin D on colorectal cancer risk reduction and provide practice recommendations for clinicians. Providing a consistent curriculum to educate and train more than 2,000 members of a nursing department and over 1000 nursing students in a tertiary care hospital on a complex clinical documentation system can be a challenge to the educator. Training new employees on an ongoing basis and an entire department when periodic system upgrades occur requires that the curriculum and methodology be consistent and made available at any time – day or night. Prompted by an increase in falls in December of 2006, a group of nurses convened to address new approaches in fall prevention. This group quickly identified the need for a multidisciplinary approach. During the first year, the interdisciplinary committee effected policy changes in many departments and provided multi-hospital wide education. The committee also supported the development of a nursing Fall Prevention Resource Group that meets monthly and is composed of nurses and aides from each unit. The members are the “Champions for Fall Prevention”. The monthly meetings of both groups keep the fall prevention initiative alive.

Opioids are the mainstay of post-operative pain management and may produce side effects that impact patient recovery. Use of Extended Release Epidural Morphine (EREM) has been shown to result in significantly less average morphine usage and to have superior analgesic efficacy than other modes of postoperative pain management. The purpose of this retrospective review was to compare the incidence and onset of side effects of ERE and other post-operative Analgesic regiments.
