Access To Prenatal Care And Mental Health In Pregnant Women During Covid-19: A Systematic Review, Chloe Feezel, Marin Kahoun, Kaija Kunttu

Williams Honors College, Honors Research Projects

COVID-19 affected the world in many ways. Pregnant women were greatly impacted by a change in their access to prenatal care, from in-person care to telehealth methods, and social support being limited by social isolation. Compared with prevalence of anxiety and depression in pregnant women pre-COVID-19, pregnant women during COVID-19 reported higher levels of anxiety and depression symptoms due to the stress of circumstantial changes. The purpose of this systematic review is to review and critically appraise the evidence about access to prenatal care and mental health in pregnant women during the COVID-19 pandemic. The following PICOT formatted question will …

Background:

Preterm birth is the leading cause of perinatal morbidity and mortality and is associated with adverse developmental and long-term health outcomes, including several cardiometabolic risk factors and outcomes. However, evidence about the association of preterm birth with later body size derives mainly from studies using birth weight as a proxy of prematurity rather than an actual length of gestation. We investigated the association of gestational age (GA) at birth with body size from infancy through adolescence.

Methods and Findings:

We conducted a two-stage individual participant data (IPD) meta-analysis using data from 253,810 mother-child dyads from 16 general population-based cohort …

Maternal educational attainment (MEA) shapes offspring health through multiple potential pathways. Differential DNA methylation may provide a mechanistic understanding of these long-term associations. We aimed to quantify the associations of MEA with offspring DNA methylation levels at birth, in childhood and in adolescence. Using 37 studies from high-income countries, we performed meta-analysis of epigenome-wide association studies (EWAS) to quantify the associations of completed years of MEA at the time of pregnancy with offspring DNA methylation levels at birth (n = 9 881), in childhood (n = 2 017), and adolescence (n = 2 740), adjusting for relevant covariates. MEA was …

Understanding Nursing Students' Cultural Competency Levels And Implicit Biases, Deja J. Gillans

Honors College Theses

In the United States, non-Hispanic Black women are approximately three times as likely to die from pregnancy-related complications as non-Hispanic White or Hispanic women. Racism has been identified as a significant barrier to health equality and the improvement of maternal health outcomes for Black patients. Cultural competence and implicit bias in health care has emerged in part to address factors that may contribute to racial/ethnic disparities in health care. Both can impact the way nurses consciously and subconsciously deliver healthcare to patients of a different gender, race, religion, culture, socioeconomic status, etc. Nurses who are more culturally competent and aware …

A Process Evaluation On The Health Care Transition Process For Youth With Osteogenesis Imperfecta With Special Health Care Needs, Janet Amirkhanian

Doctor of Nursing Practice (DNP) Manuscripts

With medical, surgical, and technological advances, children with childhood diseases with special health care needs (SHCNs) are surviving and living into adulthood (Cohen et al., 2014). Health care transition (HCT) should be a process through which youth are expected to achieve the skills necessary to manage their health as adults (Sawicki et al., 2017). HCT should be a purposeful, organized, and coordinated process involving the youth, family, and health care team (Stringer et al., 2015; Gabriel et al., 2017; White et al., 2018). Evidence supports that adolescents and young adults with SHCNs are often unprepared for transition (Jensen et al., …

Implementation Of Staff Education To Standardize Use Of Positions During The First Stage Of Labor, Kelsey Jo Koch Dec 2022

Implementation Of Staff Education To Standardize Use Of Positions During The First Stage Of Labor, Kelsey Jo Koch

Master's Projects and Capstones

The aim of this project was to assess whether supplemental education on labor positioning for Labor and Delivery nurses would increase the nurses’ confidence in utilizing patient positioning to promote fetal descent and progress labor. Previous research suggests that upright, mobile positions during the first stage of labor have various benefits, including reduced cesarean rates, shorter lengths of labor, and less epidural usage. However, a major barrier to maximizing these benefits is a gap in knowledge among Labor and Delivery nurses, resulting in inconsistent use of various positions. This improvement project addresses this problem at a large community hospital in …

Abstract

Non-pharmacological intervention alone or paired with pharmacological interventions is somewhat effective in treating and lowering rates of postpartum depression (PPD) in women. Undergraduate students reviewed studies of the use of pharmacological and non-pharmacological treatments for postpartum depression. To narrow down results, the students based the review on studies completed within the last 10 years, focusing specifically on primiparas (first births) women. Responses ranged from no notable efficacy in reducing PPD measured against the Edinburgh Scale to marginal improvements when non-pharmacological and pharmacological approaches were combined, and testing extended to other scales of motherhood and postpartum. The students discuss the …

Intermittent Auscultation Versus Continuous Fetal Monitoring In Low-Risk Pregnancies And Deliveries, Heather Wood Dec 2022

Intermittent Auscultation Versus Continuous Fetal Monitoring In Low-Risk Pregnancies And Deliveries, Heather Wood

Symposium of Student Scholars

Intermittent auscultation (IA) became the standard of practice during labor through the monitoring developments of Von Winkel. He defined fetal distress parameters that were constant until the development of electronic fetal monitoring (EFM). EFM has since eclipsed the use of IA. However, not to the benefit of the low-risk mother and child. Currently, the global caesarean rate is 21% and the US rate is 31.8%. Research indicates that the use of IA decreases the rate of caesarean section, decreases ICU admissions, and increases fetal well-being outcomes such as higher Apgar scores, lower neonatal seizures, and lower perinatal mortality. With continuous …

Reducing Diaper Dermatitis In The Nicu, Yesenia Alvira

Symposium of Student Scholars

Abstract

Purpose: This evidence-based project aims to support the acquisition of diaper dermatitis protocols that include nursing interventions, assessment tools, and educational opportunities for NICU nurses. Neonatal diaper dermatitis cases are common in the NICU due to the skin of neonates often being underdeveloped and sensitive to the environment. Diapers, especially, can irritate the perineal skin of neonates. Nurses who do not comply with standard perineal skin care can compromise the neonate's skin integrity. This can cause diaper dermatitis, which, if left untreated, can pose a risk of infection and cause pain to the neonate. Literature review: The review of …
