Nonlinear Dynamics And Interpersonal Correlates Of Verbal Turn-Taking Patterns In A Group Therapy Session, David Pincus, Stephen J. Guastello

Interpersonal processes and dynamics are ubiquitous topics in psychotherapy, yet they are difficult to study and are theoretically fragmented across therapeutic subdisciplines. The current study tests an integrative model of interpersonal dynamics in small groups using nonlinear dynamical systems theory. The conversation of one group therapy session (with six adolescent sex offenders) is analyzed using orbital decomposition, which allows for the identification of patterns in categorical time series data. The results show evidence of selforganizing social patterns, based on formal measures of turbulence (Lyapunov dimension), information novelty (Shannon's entropy), and complexity (fractal dimension). Objective: The role of group therapy in treatment of posttraumatic stress disorder (PTSD) has been traditionally restricted to issues of self-esteem and interpersonal relationships, rather than primary symptoms of the disorder. In this study, the authors examined the effectiveness of a 16-week trauma-focused, cognitive-behavioral group therapy, named Interactive Psychoeducational Group Therapy, in reducing primary symptoms of PTSD in five groups (N=29) of multiply traumatized women diagnosed with chronic PTSD. Method: The authors made assessments at baseline, at 1-month intervals during treatment, at termination, and at 6-month follow-up by using self-report and structured interview measures of PTSD and psychiatric symptoms. The present research was conducted in order to ascertain whether differences in attitudes exist between the three main professional groups which are presently the major providers of mental health care -- psychiatrists, clinical psychologists, and social workers -- with respect to six treatment modalities: Individual Psychotherapy, Individual Counseling, Family Therapy, Group Therapy, Psychiatric Medication and Psychiatric Hospitalization. Modalities evaluated include treatments appropriate to a variety of individual, social and biological concepts of mental health/pathology.
