A state budget deficit in Connecticut threatens the closing of state mental hospitals and holds the potential of displacing chronically mentally ill patients into homelessness. This article explores the historical association between deinstitutionalization of patients from mental hospitals and subsequent homelessness. One third to two thirds of homeless individuals are mentally ill and require intensive, specific, and targeted responses by the community to end their homelessness. Trends in the care of the chronically mentally ill and the conditions of those who are homeless in Connecticut are explored. Historically, people with chronic mental illnesses have been particularly at risk for homelessness. In 1984, the Connecticut Department of Mental Health (DMH) articulated policy to insure housing for mentally ill persons. One facet of that policy is to increase mental health services to homeless people. The Greater Bridgeport Community Mental Health Center has addressed this need through the formation of the Homeless Outreach Team (HOT). This article describes the development, organization, clinical work, and future of HOT. Millions of Americans suffer from physical or mental conditions that make it difficult for them to live fully independent lives. These are the frail elderly, disabled and chronically ill persons of all ages, and many mentally ill or mentally retarded persons. They need help to manage daily activities, whether they live in their own homes or in nursing homes.

Such care can be extremely expensive, since it often must be provided for many years, even a lifetime. Today, those costs are met largely by the individuals themselves or by their families and by public programs for low-income persons.

Introducing The Living With Illness Group: A Specialized Treatment For Patients With Chronic Schizophrenic Conditions, Thomas G. Plante, Stephanie L. Pinder, David Howe

This paper introduces a specialized psychotherapy/psychoeducational group treatment for patients with chronic psychotic symptoms who receive only partial benefit from psychotropic medications, psychotherapy, and milieu/activities therapy. The goal of the group is to assist patients to accurately identify the feelings, thoughts, and behaviors that are dysfunctional symptoms of their disability and then use the functional aspects of their brain as well as learned coping strategies to compensate for their disability. The group format, philosophy, procedures, and typical topics of discussion are detailed.

A Cross-Cultural Analysis Of Psychiatric Symptom Expression Using Langner's Twenty-Two Item Index, Robert Nishimoto

The Journal of Sociology & Social Welfare

An analysis of psychiatric symptom response using Langner's twentytwo item index was conducted across three cultural settings. Respondents were Anglo-American, Vietnamese Chinese, and Mexican. Some similarities did exist in response patterns and in those symptom items highly correlated with the total psychiatric screening score. Close examination revealed between-group differences indicating that not all twenty-two items were valid indicators of psychological disorder across cultures. The study highlights issues in the development and use of symptom checklists to measure and assess mental health constructs across cultures.

The paper reports on the philosophy and clinical results of an individualized but concentrated team approach to hospitalized patients undergoing a psychological crisis. The team focuses on the patient’s key vital coping technique as the source of his vulnerability and assists the patient toward constructive self-understanding. The approach favors maximum therapeutic benefit from a short hospital stay. Follow-up of the first fifteen cases managed by this method discloses a significantly low percentage of relapse and rehospitalization, and demonstrates that even such relapses can be turned to therapeutic benefit.

The Hamilton Rating Scale for Depression (HRS), a 17-item observer-rated scale, was first developed in 1960 to assess the severity of depressive symptomatology in patients diagnosed as suffering from depression. The HRS has since demonstrated high inter-rater reliability (with coefficients ranging from .87 to .94), and has proven useful in measuring changes of severity following treatment.

Since its development, the HRS has been the focus of several factor analytic investigations, four of which attempted to assess the instrument for factorial invariance. Factorial invariance refers to the generalizability or applicability of dimensions developed from one sample to another. While some consensus …

This study attempted to measure mental health status of Vietnamese refugees in Utah County, Utah. The test instrument used was the Cornell Medical Index (CMI). Two subproblems were considered: First, to see if refugee complaints on the CMI were predominantly physiological or psychological. Second, to see if there was a relationship between CMI scores and a related list of demographic variables.

Results showed the mean CMI score for the sample under study was well above the suggested score indicative of possible psychological dysfunction. For 80% of participants, psychological complaints were predominant over physical complaints. The only variables showing a relationship …

Such problems include community resistance to the mentally ill, inadequate treatment services, support systems, a lack of service utilization, and a lack of follow-up studies concerning this population, especially in rural areas. While these problems are generally characteristic of all areas of the country, whether urban or rural, they tend to have extreme manifestations in rural areas. In recent years attention has been given to the problems of the chronically mentally ill in regard to the effects of deinstitutionalization and a need for community supports. In this study, 77 service providers to the chronically mentally ill of Multnomah County (Portland), Oregon were interviewed to assess the components of the existing community support system for this population, as well as to identify the strengths and weaknesses of the system. Ten chronically mentally ill clients were interviewed in a similar manner.

The major part of this study was a treatment program designed to motivate mentally retarded adults to lose weight within a small group setting. Subjects were tested pre- and posttreatment on the Tennessee Self Concept Scale to ascertain any improvement in self-esteem. A 50-question diet quiz was constructed by the experimenters and administered pre- and posttreatment to find out if the subjects would improve their knowledge of health and nutrition through the study.

This paper discusses the use of court-imposed standards for public mental hospitals as a method of improving public mental health services. The standards set out in Wyatt v. Stickney are examined, and the author concludes that if implemented nationally such standards would transform the public hospitals. In addition, implementation would alter the power structure of mental health workers, effect the allocation of state and federal funds, and influence the larger system of mental health services. An Exploration Of The Relationship Between Loneliness And Differential Diagnosis In A Rural Mental Health Center, Kathryn Dunscomb

The review of the literature discussed loneliness in general and specified three seperate forms of loneliness; social, psychic, and existential. The relationships between loneliness, social interaction, and pathology were discussed. 139 clients were given the Belcher Extended Loneliness Scale - Revised prior to therapy. They were assigned one of six diagnostic categories to determine loneliness score (LS), diagnostic, and sex interactions. An analysis of variance found a significant difference (p <.0001) between the diagnostic categories’ LS. The schizophrenic’s and the neurotic’s LS s were significantly higher (<.05) than the remaining categories. The males had significantly (p <.036) higher LS than the females. It was also hypothesized that the degree of loneliness would increase the lower the diagnostic developmental level. The hypothesized order was not consistent with the results. Further research was recommended.

Concerned that depression might be a common malady among youth who are placed in treatment centers for delinquents, we undertook an exploratory study to determine the validity of this premise. The authors of this study have all worked with youth who, for various reasons, have been placed in residential treatment centers. The authors have observed that symptoms of depression seem to be more prevalent in these youth than in non-institutionalized adolescents, and that these symptoms are often perceived as individual pathology rather than emotional responses common to institutionalized youth.
