Transition To Home Guideline For Patients With Diabetes, Kathleen R. Burgner

Background: Diabetes is a leading contributor to preventable hospital readmissions. These hospital readmissions often result from fragmented discharge planning, poor care coordination, and limited post-discharge support. At a 915-bed academic medical center in Southern California, the diabetes-related readmission rate was 13.3%, compared with 11.4% among patients without diabetes. This project aimed to develop an evidence-based Transition-to-Home (TTH) guideline for hospitalized patients with diabetes and evaluate the strength of its recommendations through multidisciplinary stakeholder review.

This study investigated the lived experiences of individuals living with HIV (PLWH) in the Dallas-Fort Worth (DFW) area of Texas during the COVID-19 pandemic, using an Interpretative Phenomenological Analysis (IPA) framework. The research examined the intersection of health, social dynamics, and psychological resilience among PLWH, who faced compounded vulnerabilities due to systemic inequities and the dual public health crises of HIV and COVID-19. Through semi-structured interviews with ten participants from diverse backgrounds, the study captured the profound impacts of disrupted healthcare systems, heightened stigma, and social isolation on this marginalized population. Advance Care Planning (ACP) is a critical component of patient-centered care, yet its utilization remains disproportionately low among Non-White populations in the United States, particularly among individuals with chronic illnesses such as heart disease and cancer. This article explores racial disparities in ACP engagement using secondary analysis of the 2018 and 2020 waves data from the Health and Retirement Study (HRS), guided by Bandura’s self-efficacy theory (SET). The objective of this dissertation is to understand develop an understanding of experiences of online romance scam¹(ORS) victimization and to examine factors associated with ORS victimization. The dissertation samples ORS victims and uses purposive sampling strategies to recruit participants. The research used a mixed methods explanatory design approach. Inclusion criteria included ORS victims 18 and older who lived in the U.S., lost any amount of money in ORS scam, and met the scammer on any Internet platform. Substance use is one of the main problems affecting adolescents and young adults. While the consumption of alcohol has decreased, the consumption of marijuana has increased rampantly due to youth's positive perception after being legalized in 24 states with no clear message of the many adverse effects this drug causes. Different approaches to prevention have been used throughout the years: scare-led tactics, socio-emotional learning skills, and neuroscience-based curriculums. Adults with Down syndrome process and express their grief differently from the general population. There are a multitude of support groups and organizations for individuals grieving a loss and experiencing prolonged grief but there are none that exist for people with Down syndrome. This project develops a guideline for creating support groups aimed at adults with Down syndrome who experience prolonged grief. Fifty-one pieces of literature from all over the world were reviewed to formulate recommendations that were graded by seven stakeholders.
