Predicting Work Stress Burnout In Rural And Urban Emergency Medical Technicians Through The Use Of Early Recollections, Susan M. Vettor

Problem . Literature on work-stress burnout among emergency medical technicians (EMTs) suggests that they have maintained the same levels of burnout and attrition rates for the past 20 years. The purpose of this study was to investigate the relationship between early recollections and burnout in EMTs working in urban and rural locations.

Method . A demographic questionnaire, the Staff Burnout Scale for Health Professionals (SBS-HP), and two early recollections, were used to survey 120 emergency medical technicians in Toronto, Ontario and Mojave County, Arizona to assess their level of burnout and to identify various themes in early recollections.

The interrelationships of participation in counseling, level of hope, and care service utilization in a sample of individuals with HIV disease was the focus of this study. It was hypothesized that individual counseling would improve an HIV-positive person's level of hope, and would increase participation in additional care services. It was also hypothesized that individuals with more hope would seek a greater number of care services. The Health Status Questionnaire, which contains the Hope Scale, and the Social Support Questionnaire were used to collect data. One of four children in the United States lives in a family in which the child is exposed to alcoholism. Children of alcoholics (COAs) are two to ten times more likely to develop alcoholism than non-COAs. This paper reviews studies that have attempted to identify risk factors that mediate the increased vulnerability and the protective factors that moderate the risk. Factors discussed include parental antisocial personality disorder, externalizing behavior, internalizing symptoms, differential response to the effects of alcohol, and positive and negative alcohol-related expectancies. This research estimates the life expectancy of members of the Church of Jesus Christ of Latter-day Saints (LDS) in Utah. We create gender-specific life tables for four groups: total Utah, active LDS, less-active LDS, and non-LDS. Male life tables are based on data from 1991-1995; female life tables are based on data from 1994-1998. Life expectancy at birth is 75 years for all utah males, 79.8 years for active LDS Utah males, 71.6 years for less active LDS Utah males, and 71.5 years for non-LDS Utah males. Female life expectancy at birth is 80.4 years for all Utah females, 83.9 …

Increasing Readiness To Change Among Smokers In A Primary Care Setting, Sheila F. Collicott

This study compared the effectiveness of two brief interventions, direct advice and motivational interviewing, for increasing motivation to quit among male smokers in the pre-contemplation and contemplation stages of change who were primary care patients at an Eastern urban VA medical center. Contrary to expectations, participants receiving motivational interviews did not increase more in readiness to change, motivation, and actions to quit, than those receiving direct advice or a control conversation, nor did they smoke fewer cigarettes per day. As expected, contemplators reported more cutting down and quit attempts than pre-contemplators. Factors that may have limited the effectiveness of interventions …

When the cocaine epidemic began, predictions were made that the children of cocaine users would demonstrate devastating negative developmental sequelae. In infants and young children prenatally exposed to cocaine most frequently the neurobehavioral and neuromotor systems have been studied. Although clinically described as irritable, difficult to console, and jittery as infants, research findings have not been able to clearly describe a pattern of long-term developmental sequelae. The mechanisms of action of drug exposure on developmental outcome have shown to be more complex than originally suspected. Many factors, other than the drug use, can influence developmental outcome. The purpose of this exploratory and confirmatory contrasted-group classification study was to determine if specifically predicted (Hypothesis 1) and externally valid (Hypotheses 3 and 4) subtypes of childhood externalizing disorders could be identified, using previously obtained neuropsychological (NP), socio-emotional (SE), and historical variables. Cross-classification validity was evaluated to determine if subjects NP and SE subtype memberships could be predicted using either their SE and NP subtype memberships or their SE and NP variable scores, respectively (Hypothesis 2). Fifteen adults from the general population with chronic tension headaches were divided into four groups. Four were assigned to relaxation instructions, biofeedback (EMG and temperature), and home relaxation practice; four were assigned to relaxation instructions and biofeedback; four were assigned to biofeedback alone; and three were assigned to a no-treatment group. Subjects completed a ten-day baseline period followed by a three-week treatment period. Measures were taken of headache frequency, duration, intensity, frontalis EMG, and temperature recordings. Comparisons of pre- and post-treatment data indicated that not one treatment group was significantly different from any other treatment group, including the no-treatment group, …

The effectiveness of stimulation therapy for the cognitively impaired elderly living in an institution was assessed. Residents with varying degrees of cognitive impairment were treated with twenty, thirty-minute daily periods of reality orientation, reminiscence, resocialization, remotivation, and other stimulating therapies and were compared with a control group_ receiving no intervention. Treatment effects were assessed with a variety of instruments reflecting functioning across several domains. Included were ability to perform both basic self-care functions and activities of daily living, cognitive performance, affect, and satisfaction with the care received and the living environment in the nursing home. The paper reports on the philosophy and clinical results of an individualized but concentrated team approach to hospitalized patients undergoing a psychological crisis. The team focuses on the patient’s key vital coping technique as the source of his vulnerability and assists the patient toward constructive self-understanding. The approach favors maximum therapeutic benefit from a short hospital stay. Follow-up of the first fifteen cases managed by this method discloses a significantly low percentage of relapse and rehospitalization, and demonstrates that even such relapses can be turned to therapeutic benefit.

It has been assumed until recently that chronic renal failure is more prevalent among men than among women, but data now indicate that at least half of all renal patients are women. The literature continues to focus on adjustment problems of male patients, especially sexual adjustment and job-loss problems, and to assume that women can adjust more easily because of their ability to maintain the homemaker role. However, women patients whose work status is that of homemaker are found to have the highest depression scores, and job loss results in low satisfaction for those who have held meaningful outside jobs. …

An analysis of changing physician ideologies regarding the care of elderly dying patients, as expressed in technical journals read by American physicians. Markedly more titles concerning terminal care are found in Index Medicus in 1968- 78 than in 1960-67. In one journal, physicians are only after 1964 urged to tell patients openly about their condition and after 1969, to improve cooperation within professional teams. Two explanations of these data are tentatively explored: 1) a Parsonian explanation, whereby medical ideologies reflect professional autonomy and the influence of internalized moral norms; 2) a Marxist explanation, whereby medical ideologies reflect physicians' transformation from …

The purpose of this study was to determine the relationship between adjustment to life-event stressors and selected psycho-social variables. Recent research has consistently identified an etiological link between stressful life-events and physical and/or emotional disorders. It has often been noted, however, that exposure to seemingly identical stressors results in varying levels of adjustment among individuals. Lazarus (1966, 1967, 1971, 1974) has proposed that variations in stress responses are attributable to differences relative to the implicated individual's appraised threat from the stressor. Based on this conceptualization, adjustment to stressful life-events was selected as the dependent variable for the study. Five independent …

The specter of addiction eventually involves everyone of the community. It is a three-fold problem of socio-economic, psychological, and physiological factors. The least understood is the physiological or biochemical aspect of addiction.

It has been postulated that an upset in carbohydrate metabolism may be a biochemical cause of addiction. Several avenues were employed to investigate this possible cause of addiction.

1. A diet survey was conducted and revealed a greater consumption of refined carbohydrates, confections and alcoholic beverages in addicts.

2. Five-hour oral glucose tolerance tests were performed on 7 addicts and 12 non-addicts. These tests revealed that there were decreased glucose …
