There are many barriers to accessing opioid use disorder (OUD) treatment, especially for rural communities that may be far away from healthcare providers. There is a need for interventions to increase accessibility of OUD treatment. Extended release injectable buprenorphine (Sublocade) provides an option for patients to receive treatment via once a month injections. This could make OUD treatment more accessible to rural and socioeconomically disadvantaged patients by reducing the number of times they would need to travel to the clinic. Our intervention is to create a protocol for prescribing extended release buprenorphine at Family Medicine Hinesburg.

Access to medical treatment for opioid use disorder plays a significant role in the ability of a community to support a person's recovery. In Springfield, VT, community workers and providers are attempting to organize a Suboxone (buprenorphine + naloxone) clinic in order to bridge the significant geographic and socioeconomic gaps implicit to this rural community's location. In order to garner interest and increase awareness of this forthcoming project, a short, accessible educational flyer was developed with the input of the health center's staff for future use.

Each day more than 140 Americans die from drug overdoses, 91 specifically due to opioids. In Vermont, more than 50 people die each year from opioid poisoning. With insurance coverage being a critical component of Opioid Dependence Recovery, many people seeking treatment are unaware of the financial barriers to recovery- namely the out-of-pocket costs associated with treatment. This study sought to provide insight on the financial impact of Opioid Dependence Treatment on the patient, and provide financial assistance information to the Rutland County community.

Problem: Patients on Suboxone (buprenorphine-naloxone) therapy break compliance by taking higher Suboxone doses, by supplementing with Suboxone purchased illicitly (“off the street”), or by taking other opiates and illicit drugs (cocaine), excluding marijuana. Need: Physicians and Medication-Assisted Treatment (MAT) counselors would benefit from an approach to counseling and treating their patients that would better address the specific vulnerabilities that Suboxone patients face that threaten their compliance.

Opioid Use Disorder (OUD) is a major problem in the state of Maine with one person dying from overdose every day. Medication Assisted Treatment (MAT) using Suboxone has been shown to be helpful in the treatment of OUD, but there is currently a great need for providers to prescribe Suboxone in this region. We looked at barriers that providers saw for obtaining a waiver to prescribe Suboxone and created a handout to educate providers on MAT and the process for getting the waiver.

Mindfulness has been shown to aid in addiction recovery and can help prevent relapse. The Family Medicine Center & Residency Program at Eastern Maine Medical Center (EMMC) in Bangor, Maine has provided mindfulness resources to their opioid-dependent patients; however, the center has no quantitative data on the current understanding and utilization of mindfulness techniques among this patient population. A survey tool was created and distributed to opioid-dependent patients in the clinic to identify barriers to practicing mindfulness. The collected data was submitted by one of the clinic providers in a grant proposal. Grant funding could go toward addressing these barriers.

Substance use in pregnancy is not uncommon and the use of substances increases the risk of complications during pregnancy. In Vermont, opiate use in pregnancy is increasing as well as the number of people seeking treatment for opiate misuse. This project was designed to give pregnant mothers using opiates an idea of what to expect during their pregnancy as well as the resources that are available to them in the Springfield, VT area.

Opioid use disorder is a nationwide issue and a serious problem in the state of Vermont. Medication assisted treatment (MAT) is currently being used to treat individuals with opioid use disorder. In an effort to address the opioid epidemic, more and more family medicine physicians in Vermont are becoming trained medication assisted treatment providers. This project aims to recognize some of the challenges faced by physicians who are currently prescribing MAT in family medicine clinics, as well as to identify the concerns of family medicine physicians who are not currently prescribing MAT. There are several buprenorphine providers at EMMc's Center for Family Medicine serving the greater Bangor, ME region - an area of substantial opiate use. Among the patient population of outpatient buprenorphine users, both locally and nationally, there are high rates of relapse (~32%). In order to decrease relapse rates, it's first imperative to conduct a baseline review of the current buprenorphine population to identify specific types of patients who are at higher risk of relapse. By understanding the barriers to recovery, the office hopes to apply an intervention to the current program, targeting this local demographic more effectively.
