This project seeks to consolidate common immunization recommendations and preventative screening recommendations for adult women 18 years and older, based on the U.S. Preventative Services Task Force and Centers for Disease Control and Prevention recommendations. This resource should be offered to patients of the primary care office, with the goal of providing information in a concise and approachable manner that allows patients to review general recommendations on their own, generate questions or express concerns about their health, and encourage patients to advocate for their own health and well-being. There is a significant population of migrant farmworkers, especially H2A visa holders, in Windham county with limited access to health care compared to peers in Addison and Chittenden counties. Health care is delivered to migrant farmworkers across Vermont via a volunteer-based program coordinated by the University of Vermont Extension known as Bridges to Health. Given the proximity of Grace Cottage to H2A farmworkers, it is reasonable to explore ways of increasing access for this patient population, including raising awareness of Bridges to Health.

Vaccination is a very valuable tool in combating the public health crisis of COVID-19 and will be essential in reducing transmission rates and deaths due to the pandemic. Public willingness to receive the vaccination in large polls has stayed well below the ~95% of the population needed to achieve herd immunity against the virus. Here were survey Vermonters to determine their attitudes towards receiving the vaccine and what sources of information patients use to inform themselves about the different vaccines available.

Patients are frequently coming into clinics with misinformation and frequently asked questions regarding the COVID-19 vaccine. The goal of this project was to address these frequently asked questions by educating patients and sparking conversation regarding the vaccine when patients came to their clinic appoints. This was accomplished via a handout that was given to patients on arrival.

These important questions are taking valuable time during already short patient visits, and as information is constantly evolving it is difficult to give a concise answer to many questions. This project outlined the basics of the vaccine and answered many FAQs.

In December 2020, the U.S. Food and Drug Administration issued emergency use authorizations for two COVID-19 vaccines. Many patients at the Central Maine Medical Center Family Medicine Residency clinic had questions related to the vaccines and the plans for vaccine distribution in Maine. With the prevalence of misinformation related to the COVID-19 vaccines on the internet, there existed a need for accurate, easily understandable information to assist with patient education about the vaccines.

COVID-19 continues to be a significant health threat and has caused significant disease and mortality in the fall 2021 wave largely driven by the Delta Variant and complicated by unvaccinated patients. In the state of NY, approximately 40 patients died per day (7 day moving average) from serious COVID-19 illness during this time period. During this time, approximately 35% of Clinton County’s population is not vaccinated for COVID-19 and is subsequently unprotected from serious COVID-19 illness. Vermont's dairy industry relies heavily on migrant workers. These migrant farmworkers are mostly Latino and undocumented thus ineligible for health insurance. The Open Door Clinic in collaboration with UVM Extension Bridges to Health and Colchester Family Practice provided free satellite flu clinics to Northern Vermont Latino dairy farmworkers. This project served 12 dairy farms and vaccinated 48 farmworkers.

Herpes zoster (shingles) is a viral infection that results in a painful rash and has a myriad of long-term complications, including post-herpetic neuralgia- chronic pain that does not resolved despite resolution of the rash. Over the last 20 years, incidence of shingles has increased by nearly 50% and now there are 2 vaccines available to prevent shingles. Shingrix was approved by the FDA in 2017 and has garnered public attention for its efficacy. I developed a brochure that gives patients information about shingles, Shingrix, and answers some frequently asked questions.

As the immune system tends to weaken over time, patients aged 50+ begin having increased risk for certain potentially preventable diseases, especially shingles and pneumonia. Although there are vaccinations to protect against these diseases, there are many significant barriers preventing patients from receiving the vaccines that they need, including potentially confusing guidelines, concerns over risks, and recent changes to vaccine recommendations. This project aims to produce a clear and concise educational brochure to help overcome some of these barriers, and to drive patient-provider conversation about their vaccine recommendations.

Influenza contributes to millions of dollars in healthcare expenses, lost economic productivity and morbidity and mortality of the Chittenden County, VT population. Despite the dire costs of influenza illness, yearly vaccination rates remain low in Vermont and Chittenden County. This study utilized a survey that aimed to gauge the attitudes toward influenza vaccination of patients at the Hinesburg Family Practice. Results of this study suggest that patients under 35 have lower attitude scores (p= 0.035) and that patients with high school level education or bachelor’s level education are more likely to have a negative attitude towards influenza vaccination. Foreign-born individuals account for over 12% of the U.S. population, according to the most recent census data. Since many vaccine-preventable outbreaks in the U.S. have been correlated with disease importation, Congress has mandated vaccinations for numerous immigrant populations. It is essential for primary care physicians to be knowledgeable on the unique immunization-related needs of foreign-born individuals, to recognize some of the cultural and linguistic challenges that immigrants have accessing healthcare, and remember to use each medical encounter as an opportunity to provide necessary vaccinations.
