Introduction: Exfoliative dermatitis is a severe form of skin irritability that causes significant erythema and/or scaling. The United States faces a critical dermatology workforce shortage, with more than 60% of counties lacking specialist access. Primary care providers (PCPs) are often tasked with managing suspicious skin lesions but demonstrate lower diagnostic accuracy than dermatologists. Artificial intelligence (AI) tools offer potential solutions, and meta-analytic evidence demonstrates that AI assistance improves PCP sensitivity by 13 percentage points and specificity by 11 percentage points. Yet only 1 Food and Drug Administration–authorized device (January 2024) is cleared for nondermatologist use in the United States. The lines of Blaschko represent epithelial cell migration during embryonic development. Rare forms of cutaneous lupus erythematosus may manifest along these lines, and lesions are termed Blaschko-linear lupus erythematosus (BLE). The purpose of this case study is to present a rare manifestation of linear cutaneous lupus erythematosus following the lines of Blaschko in a pediatric patient. To date, there are very few reports of BLE. We present a 17-year-old female with a 2-month history of a mildly tender growing lesion along the right nasofacial sulcus. Hidradenitis suppurativa (HS) is a chronic inflammatory skin disorder characterized by recurrent deep-seated nodules and painful abscesses with scars, fistulae, and sinus tracts that predominately affect the intertriginous sites. Onset typically occurs soon after puberty. Depending on disease severity, HS can lead to varying levels of pain, disability, and disfiguration with significant impacts on quality of life. Hidradenitis suppurativa is diagnosed clinically, and no diagnostic tests are specific to the disease. Therefore, clinicians must have the knowledge and competence to identify the clinical features of HS accurately. BACKGROUND: Adalimumab is widely used for hidradenitis suppurativa (HS), but treatment durability varies and predictors of sustained benefit remain uncertain.
OBJECTIVE: To identify clinical and laboratory factors associated with adalimumab treatment durability in HS.
METHODS: We retrospectively studied 135 adults with HS who initiated adalimumab as first biologic therapy across three academic centers from 2013 to 2023. Outcomes were categorized as primary failure (12-24 weeks), secondary failure (24 weeks-2 years), or sustained response (>2 years) based on discontinuation for inefficacy. Predictors of treatment durability were evaluated using ordinal logistic regression.

Localized scleroderma, also referred to as 'morphea,' is a rare autoimmune disorder that affects the superficial layers of the skin and is characterized by both erythematous and bruise-like lesions with signs of central skin tightening or sclerosis; deeper lesions may be identified as edematous, erythematous plaques in the absence of well-defined borders. A subtype named en coup de sabre (ECDS) exists in the classification of linear morphea and presents with a linear induration on either the face or scalp. Langerhans cell histiocytosis (LCH) is a rare malignancy marked by clonal proliferation of Langerhans cells, with BRAF V600E mutations identified in over 50% of the cases. BRAF inhibitors (BRAFi), such as dabrafenib, have shown efficacy in treating BRAF V600E-mutant LCH. However, BRAFi therapy is associated with cutaneous adverse effects, including the development of new melanocytic nevi, verrucae, and keratinocyte carcinoma (KC). This case report describes a 78-year-old woman with BRAF V600E-mutant LCH who developed multiple dermatologic side effects following dabrafenib salvage therapy. Trichoblastomas are uncommon adnexal tumors that usually occur on the face. We report a case of a 62-year-old White man with a large, asymptomatic, slow-growing lesion that had been on his right upper cutaneous lip for 15 years. A 2 cm pink, pedunculated nodule with telangiectasia was seen on examination. A shave biopsy was consistent with trichoblastoma: a circumscribed-appearing dermal tumor of basaloid cells arranged in cords and islands within a cellular stroma, focal epidermal connection, and papillary mesenchymal bodies. Background: Psoriasis affects approximately 2% of the global population and is associated with comorbidities such as depression, psoriatic arthritis, and cardiometabolic syndrome. Secukinumab, a monoclonal antibody targeting interleukin-17A, is effective for moderate-to-severe plaque psoriasis. This study assessed the real-world effectiveness of secukinumab over 52 weeks.
Methods: This retrospective cohort study included 53 patients aged ≥18 years who received secukinumab and were followed for 52 weeks. Patients received weekly loading doses followed by maintenance injections every four weeks. Effectiveness was evaluated using changes in the Psoriasis Area and Severity Index (PASI).
Results: At week 16, 86.8% of patients achieved PASI <5, 79.2% PASI <3, 69.8% PASI <2, and 64.2% PASI <1, indicating substantial improvement. PASI 75 was achieved by 77.4% of patients at week 8. PASI 90 was achieved by 71.7% of all patients and by 76.9% of patients with psoriatic arthritis at week 16. No patients discontinued treatment because of adverse events.

Background: Treponema pallidum, the agent causing syphilis, is transmitted through contact with infectious lesions, bodily fluids, blood transfusions, or perinatally. Human immunodeficiency virus (HIV) infection can mask the clinical manifestations of syphilis, while syphilis may exacerbate HIV progression. Background: Lupus vulgaris (LV) is a rare, paucibacillary form of post-primary cutaneous tuberculosis, accounting for 1.5% of all extrapulmonary tuberculosis cases globally. LV may arise via hematogenous spread, lymphatic route, or contiguous dissemination. The present case highlights the importance of considering LV in patients with chronic and atypical skin lesions, particularly in tuberculosis-endemic areas.

Background: Alopecia areata (AA) is an autoimmune, non-scarring hair loss disorder commonly affecting children and often characterized by a chronic or relapsing course. Early-onset AA has traditionally been associated with a poorer prognosis, although current evidence remains inconclusive. This evidence-based case report aims to evaluate the prognostic significance of early-onset AA by synthesizing available cohort data.

Background: Medical adhesive-related skin injury (MARSI) is a common complication associated with acrylic-based adhesive removal. Topical agents such as 0.9% NaCl (normal saline), white petrolatum, and 70% alcohol are commonly used to assist adhesive removal; however, their effectiveness in reducing MARSI remains unclear.

Background: Dermatophytosis is a highly prevalent superficial fungal infection in tropical regions, often chronic due to inadequate treatment. Its recurrent nature due to misuse of topical steroids and inappropriate treatment contributes significantly to both financial burden and deterioration in quality of life. This article aimed to assess the socioeconomic burden and psychological impact of dermatophytosis, accounting for its chronicity.

Background: Cutaneous lichen planus (CLP) is a chronic inflammatory disease typically involving the flexor surfaces of the extremities. It is a CD8-positive T-cell-mediated disease of the skin in which an immune mechanism targets basal keratinocytes. Lichen planus pigmentosus (LPP), a variant of CLP, predominantly involves the extensor aspect and has recently been under the umbrella term of acquired dermal macular hyperpigmentation. The primary objective of our study was to determine the effectiveness of oral tofacitinib in treating LPP, and the secondary objective was to assess its safety.

Background: Leprosy remains a significant public health concern in endemic regions such as Indonesia. Children are particularly vulnerable due to their naive immunity and close familial contacts. This study aims to report cases of pediatric leprosy in Lembata, Indonesia, focusing on clinical characteristics and associated risk factors.
Case Illustration: A case series of eight pediatric patients (<15 >years) diagnosed with leprosy was conducted during a community-based screening program in Lembata, Indonesia, in 2022. Background: Keloid is a benign fibroproliferative skin disorder characterized by excessive collagen deposition resulting from abnormal wound healing that extends beyond the original wound margins. It is particularly challenging to treat, especially in areas with high mechanical tension and skin stiffness, such as the mandibular region, which is associated with increased risk of persistence and recurrence. Combination therapy has shown superior outcomes in reducing recurrence and improving keloids.

Background: Pemphigus vulgaris (PV) is a rare, often severe autoimmune disease affecting skin and mucous membranes. Despite the scarce literature on PV in skin of color (SOC) populations (Fitzpatrick III-VI), existing studies suggest that there are misdiagnosis, delayed treatment, and recurrence of symptoms in SOC patients with PV. Background: Climate change contributes to rising temperatures, worsening air pollution, increasing UV exposure, and more frequent extreme weather events with downstream consequences for dermatologic conditions. However, limited research has examined how these climate-related effects can impact skin of color (SOC) populations, who often face disproportionate environmental exposures and structural inequities. Objective: To summarize the dermatologic implications of climate and environmental stressors on SOC populations. Methods: A literature search of PubMed and Embase (2000-2025) identified studies assessing dermatologic outcomes related to climate change or environmental exposures. Introduction: Dermatofibrosarcoma protuberans is a locally aggressive rare dermal sarcoma with a relatively high rate of local recurrence. Incidence in the United States has been reported around 4 occurrences per million people per year, and recurrence rates have been reported between 2 to 19%. Recurrence poses a higher risk for sarcomatous transformation, increasing the risk for metastasis.
