School of Public Health Faculty Publications
Public health leaders are increasingly being asked to address adaptive challenges in the context of finite and often limited resources. Budgets and their associated resources create the financial framework within which public health agencies and organizations must operate. Yet, many public health professionals expected to undertake roles requiring this foundational knowledge and skills are not trained in the fundamentals of public finance and are ill-equipped for managing and monitoring funds. Graduate courses in schools of public health most often are focused on health care management and finance or private sector finance. To meet the needs of future public health leaders, …

Introduction
In 2020, the World Health Organization called for the expansion and greater recognition of all nursing roles, including advanced practice nurses (APNs), to better meet patient care needs. As defined by the International Council of Nurses (ICN), the two most common APN roles include nurse practitioners (NPs) and clinical nurse specialists (CNSs). They help ensure care to communities as well as patients and families with acute, chronic or complex conditions. Moreover, APNs support providers to deliver high quality care and improve access to services. Currently, there is much variability in the use of advanced practice nursing roles globally. A …

Many countries around the world strive for universal health coverage, and an essential packages of health services (EPHS) is a central policy instrument for countries to achieve this. It defines the coverage of services that are made available, as well as the proportion of the costs that are covered from different financial schemes and who can receive these services. This paper reports on the development of an analytical framework on the decision-making process of EPHS revision, and the review of practices of six countries (Afghanistan, Ethiopia, Pakistan, Somalia, Sudan and Zanzibar-Tanzania).The analytical framework distinguishes the practical organization, fairness and institutionalization …

Medical Education And Population Health—A Framework In The Design Of A New Undergraduate Program, Robert Armstrong, Michaela Mantel, Gijs Walraven, Lukoye Atwoli, Anthony Ngugi
Internal Medicine, East Africa
Health sciences curricular planners are challenged to add new content to established education programs. There is increasing pressure for content in public health, health systems, global health, and planetary health. These important areas often compete for curricular time. What is needed is a convergence model that builds a common framework within which students can integrate areas and better align this knowledge to the individual client or patient who they have responsibility to support. A population health framework is proposed for health sciences education programs that supports a common conceptual understanding of population health. The framework links five thematic areas that …

Introduction: In settings where the private sector constitutes a larger part of the health system, profit-gathering can take primacy over patients' well-being. In their interactions with pharmaceutical companies, private general practitioners (GPs) can experience the conflict of interest (COI), a situation whereby the impartiality of GPs' professional decision making may be influenced by secondary interests such as financial gains from prescribing specific pharmaceutical brands.
Methods and analysis: This study is a randomised controlled trial to assess the impact of a multifaceted intervention on GPs' medical practice. The study sample consists of 419 registered GPs who own/work in private clinics and …

Internal Medicine Faculty Publications
To provide an online service learning opportunity for medical students during the COVID-19 pandemic, medical faculty and librarians developed and implemented a “Debunking Medical Myths” module in which students learned to search for emerging medical literature, evaluate evidence, and use that evidence to create an infographics debunking a COVID-19-related myth for a non-medical audience. The resultant infographics are visually appealing and designed to make complex health information easy to understand. The module was well-received by students, who demonstrated a nuanced understanding of the use of infographics to convey health information, and students’ work was evaluated highly by community members.

Required Longitudinal Service-Learning And Its Effects On Medical Students’ Attitudes Toward The Underserved, Monica Rose Arebalos, Faun Lee Botor, Edward Simanton, Jennifer Young
School of Medicine Faculty Publications
Although medical students enter medicine with altruistic motives and seek to serve indigent populations, studies show that medical students’ attitudes towards the undeserved tend to worsen significantly as they go through their medical education. This finding emphasizes the need for medical educators to implement activities such as service-learning that may help mitigate this negative trend. All students at the University of Nevada Las Vegas (UNLV) School of Medicine are required to participate in longitudinal service-learning throughout medical school, and a majority of students interact with the underserved at their service-learning sites. Using the previously validated Medical Student Attitudes Towards the …

Introduction: Developments in technology, such as the popularity of mobile devices and social media outlets, have enhanced the ability of individuals to communicate. Currently, search engines allow for easy exploration of information related to every topic of interest. Our study purpose was to evaluate the impact of technological and social collaborations on sexual and reproductive health knowledge (SRHK).
Methods: A 50-item survey instrument, integrating factors of sociodemographics, number/type of social collaborations, technological communication use, and SRHK, was developed to assess familiarity with sexual and reproductive health perceptions. The survey was provided to consenting patients in an ambulatory, primary care setting. …

Building Connections: Skmc – Ces Medellín International Exchange Program Development, Elias Borne, Kaitlin Demarest, Rosemary Frasso, Phd, Andres Fernandez, Md, Msed
Phase 1
Introduction: To date, there has been no formal evaluation of the exchange program that sends medical students from Sydney Kimmel Medical College (SKMC) to CES University, Colombia. While many medical schools have international exchange partnerships, little is known about what makes a successful global training experience. This project aims to evaluate the SKMC-CES student exchange program with the goal of improving collaboration between the two universities and creating a logic model backbone for the program going forward.
Methods: Surveys and interviews were done with program participants from the 2018 and 2019 academic years (6 students interviewed and 4 surveyed) exploring …

Medical Student Knowledge Of Narcolepsy, Alyssa Sinko, Karl Doghramji, Md
Phase 1
Background: Narcolepsy is a sleep disorder, whose main symptoms include daytime sleepiness, cataplexy, sleep paralysis, disturbed sleep, and hypnagogic hallucinations. The disorder is poorly identified in medical settings. We hypothesized that lack of knowledge of narcolepsy and its hallmark symptoms are a cause for the low levels of identification of this disorder. This was a pilot study into medical student knowledge of narcolepsy at Sidney Kimmel Medical College of Thomas Jefferson University (SKMC).
Methods: Two groups of SKMC students, years 1 and 2 (before sleep medicine education) and years 3 and 4 (after sleep medicine education) were the subjects of …

The Impact Of Socioeconomic Factors On Food Insecurity Among Syrian Refugees In Florida, Racha Sankar
FIU Electronic Theses and Dissertations
Syrian refugees settled in the United States may experience food insecurity due to different socioeconomic factors that may include nutrition knowledge, language proficiency, women’s education, and perceived stress. The structure and the type of households may also contribute to food insecurity in this population.
The objective of this study was to measure food security among Syrian refugees residing in Florida. It also aimed to determine the socioeconomic factors that may attribute to food insecurity at household level.
A comprehensive 228-item questionnaire was administered to N=80 households (n=43 in rural areas, n=37 in urban areas). Families with and without children were …

The Impact Of Mental Health On Cancer Preventative Screenings, Chelsea Edirisuriya, Amy Leader, Phd
Phase 1
Among the mental health population of the United States, overall preventative health services, such as cancer screening rates, are remarkably low. Additionally, there is a significant 30% higher mortality rate due to cancer in psychiatric patients. This project established if there are disparities in cancer screening rates between the general population and the mental health population of Southeastern Pennsylvania. The project analyzed and compared the differences in cancer screenings for colorectal, breast, and cervical cancer among those currently diagnosed with a mental condition and the general population. Through p-value and Pearson chi-square statistical analysis of the Community Health Data Base …

Phase 1
Introduction
Opioid overdose deaths have nearly quadrupled since 1999. Nearly 2 out of 3 persons being prescribed medications are prescribed an opioid. Physicians may be prescribing opioids for more than what is required for postoperative pain control, leading to increased risk for opioid abuse and addiction. Patient perceptions of perioperative pain medications are poorly understood.
Objective
This survey aims to understand patient expectations of perioperative analgesics.
Methods
Following IRB approval, patients 18 years of age or older, were given a 13-question survey prior to their surgery at Thomas Jefferson University Hospital, to evaluate the perception of pain medications, medication efficacy, …

Mining For Voices: Reframing Hydraulic Fracturing As A Public Health Issue, Benjamin Chipkin, Ms, Pouné Saberi, Md, Mph
Phase 1
Introduction: Hydraulic fracturing extracts fossil fuels from rock formations by injecting chemicals underground. While 80% of studies demonstrate risks or actual harms to health from fracking, Pennsylvania does not require chemical disclosures. Fracking accelerates climate change, a phenomenon linked to increases in health emergencies. With 1.6 million Pennsylvanians living within 1 mile of active oil or gas development sites, there is considerable risk. In 2012, Pennsylvania banned physicians from discussing health impacts of fracking, an order struck down in 2016. Following the gag order, deficits in physician knowledge and urgency may remain around fracking. Outlining a precautionary approach to fracking …

Phase 1
Introduction: Substance use disorders, specifically opioids, are an increasing problem in the US. Screening, brief intervention, and referral to treatment (SBIRT) is a method to identify and treat patients at risk, or currently using illicit substances. Integrating SBIRT into Title X facilities, which provide reproductive healthcare to lower income populations, may allow identification and treatment of individuals that do not seek healthcare elsewhere. Objective: We aim to assess the efficacy of a longitudinal training program in Title X facilities. The program provides a structured approach to implementing new protocols, including an educational session on SBIRT and motivational interviewing (MI).
