Workload assessments help provide validation to increase staff, evaluate and ensure equal distribution of work, and assist with budget justifications. The Ontario Protocol Assessment Level (OPAL) is one of the most widely used protocol assessment tools. This study mapped an adapted OPAL score for clinical trials to actual coordinator hours from a single site to determine if the adapted OPAL score could predict coordinator hours. The purpose was to project a more accurate capacity estimate when considering new studies. This study examined the relationships between total costs of care and total readmission rates for Medicare patients undergoing major joint replacement of the lower extremity (knee/hip arthroplasty) at one of four Catholic Health Initiatives (CHI) facilities participating in both Phase 1 and Phase 2 of the BPCI program by using complete Medicare claims data for beneficiaries. Both univariate and multivariate models were utilized to examine the impact of the BPCI initiative on costs and readmissions. As the United State healthcare system continues to evolve from a reimbursement system based on volume to one based on value, understanding the relationship between physician quality metrics such as patient satisfaction and clinical quality metrics is extremely important. In order to improve value by effectuating behavior change, physician financial incentives must be designed based on desired outcomes. This study sought to answer the question, “Is there an association between quality rating performance on Nursing Home Compare (NHC) and financial performance in nursing homes?” A quantitative, non-experimental study involving archival data was conducted using data from Medicare Cost Reports and Nursing Home Compare. The sample included CMS-certified, freestanding skilled nursing facilities in the United States that participated in NHC from 2009-2011 and submitted Medicare Cost Reports from 2010-2012. There were 14,015 nursing homes in 2010; 14,139 nursing homes in 2011; and 14,265 nursing homes included in 2012. Data was analyzed with generalized linear models using the Statistical Package …

While healthcare providers have increased depression screening of adolescents during preventive care, an enormous number of adolescents do not have regular contact with healthcare providers and, therefore, do not get screened. Nationwide screenings by school nurses would be an ideal way to capture students not seen by healthcare providers; however, this screening is not required and therefore does not occur. Study Aim: To determine the relationship between hospital Magnet status designation and patient satisfaction. Data Sources: Secondary sources, the American Nurses Credentialing Center, the American Hospital Association, and Hospital Compare HCAHPS satisfaction survey data. Methods: Independent t tests were used to examine the univariate relationship between Magnet designation and 10 satisfaction survey indicators. A multivariate analysis was conducted of the 10 patient satisfaction survey indicators on Magnet designation while controlling for organization variables. It was predicted that Magnet status would be associated with 7 of the 10 measures. Results: Magnet status was found to be positively and significantly associated with …

Investigating the contents of nightstands and over-the-bed tables (OBTs) located in post-acute settings is a first step towards understanding how patients use hospital furniture to allow for user-centered design. This study provides an inventory of items located on the over-the-bed table and nightstand of patients in a rehabilitation hospital. This research agenda is to improve the usability and effectiveness of patient room furniture; this study's findings have added to the current body of knowledge and have some significant implications. Physician financial conflicts of interest exist in many forms ranging from ownership in ambulatory surgery centers, diagnostic centers and specialty hospitals. Evidence in the literature suggests growth in these types of ventures and an increase in utilization. A preponderance of research suggests that the presence of minorities on the front lines of health care can improve access, satisfaction, and quality of care for minority patients, in addition to mitigating health care disparities. Yet, there is little evidence of the utility of diversity efforts focused on the upper echelons of health care. By examining employee satisfaction scores from 58 hospitals across the United States, this study explored the relationship between the presence of women and minority managers and executives and two key indicators-representation of minority employees and employee satisfaction. The federal requirement that 51 percent of the governing boards of federally funded community health centers must be users of the health center was originally intended to insure that the community served had a voice in the services provided. This qualitative study attempted to determine the perceptions of key stakeholders of the impact of the requirement on the effectiveness of governing boards, as well as how boards develop and measure effectiveness. Open-ended, semi-structured interviews with five CEOs and five Board Chairs revealed few concerns about the challenges of complying with the requirement. HIPAA - the Health Insurance Portability and Accountability act of 1996 is the largest government action in healthcare since Medicare. Since the passage of the HIPAA legislation, estimated cost of implementation has ranged from $3.8 billion to $45 billion for the implementation of the privacy rules. This study examines the cost on implementing HIPAA in private physicians' offices in South Carolina. The study selected the medical specialties of family practice, general practice, obstetrics & gynecology, and pediatrics. The study compares and analyzes cost of rural vs. urban physician practices, by specialty, and by type of entity when implementing privacy rules.

This study examines the link between leadership behaviors and employee commitment, satisfaction and strength of culture in hospitals. The subjects were five executives in not-for-profit hospitals that had measured the culture of their hospitals with the same instrument twice over a longitudinal period of time. During the intervening period the leadership of the hospital had undertaken particular interventions in an effort to improve employee commitment, satisfaction, and strength of the culture. Measurements Of Operational Efficiency In The Outpatient Setting, Judy Grooms Burn

The results of patient flow, work sampling, and demographic analyses of the General Medicine outpatient Clinic at the Medical University of South Carolina are presented. Data collection procedures are discussed. The association between clinic efficiency and selected demographic characteristics of the patient population is examined. Inhibitors to smooth patient flow are revealed through the examination of the distributions of service and queue times. Hourly distributions of staff time expended in various activities are presented, and the work sampling data is correlated with results from the patient flow study. The usefulness of the results as indices of clinic efficiency is considered.
