Periodontal disease is the primary cause of tooth loss in the adult, and therefore, early diagnosis of periodontal disease by the general practitioner is important in terms of prevention as well as treatment. The present study was designed to develop a periodontal screening examination which would fulfill the following requirements:

  1. It should utilize precise and easily understood criteria.
  2. It should take a short period of time.
  3. It should enable the practitioner to accurately place patients into various disease categories.
  4. It should enable the practitioner to easily and quickly relate these disease categories to treatment.

Inflammation of the connective tissue adjacent to the periodontal pocket may depend on the continuous presence of bacteria. Following periodontal treatment, subgingival plaque accumulation may interfere with healing. The purpose of this study was to determine the effect of irrigation with chlorhexidine or oxytetracycline on the cultivable bacteria in standardized chronic periodontal pockets in dogs. Chronic periodontal pockets were irrigated with either 5 ml. of 2 percent chlorhexidine or 250 mg. of oxytetracycline in 5 ml. distilled water. The pockets were irrigated dally for two days and were sampled on the first, fourth, seventh, fourteeth, twenty-first, thirty-fifth and forty-second days …

Histological Comparison Of Human Pulpal Tissue Reactions After Vital Partial Pulpectomies Using N-2, Dycal And Zoe Paste, David P. Clark

A histological comparison of forty-four human teeth in which vital partial pulpectomies were performed, and the pulp stumps covered with dycal, N-2 and ZOE was carried out. The time interval at which the teeth were extracted for study was ten, twelve and fourteen weeks. These teeth were on patients twelve through seventeen years of age and were to he extracted for orthodontic purposes. The teeth were caries free, vital and asymptomatic. The percent retaining vital tissue, time intervals and type of paste were as follows:

The Effect Of Post-Operative Use Of Chlorhexidine On Periodontal Regeneration In Monkeys With Chronic Bifurcation Defects, J. Pat Bishop

Periodontal regeneration and inflammation was evaluated histometrically in 36 paired teeth in six macque(sic) monkeys with chronic bifurcation defects. Daily topical TL Chlorhexidine was applied to one-half of the teeth, saline to the other half for six weeks after the bifurcation defects were surgically debrided. Measurements were made from a section taken from the mid-bifurcation for each tooth. The results demonstrated no significant difference in the area of inflammatory response or the degree of regenerative capacity of post-operative healing between Chlorhexidine treated and paired nontreated control teeth.

The daily post-operative use of topical chlorhexidine application has been compared to daily toothbrushing. Periodontal defects were experimentally created in the bifurcations of lower premolars in dogs. The defects were treated by surgical debridement. Chlorhexidine application and toothbrushing were performed post-operatively for six weeks on contralateral pairs of teeth. The amounts of connective tissue and bone regeneration were evaluated from microscopic sections of the bifurcation areas. No difference between chlorhexidine and tooth brushing was found for the connective tissue attachment. However, significantly more bone had regenerated in the chlorhexidine treated areas than the tooth brushed area.

The effect of tooth cleaning procedures on the gingival sulcus has been investigated. Clinically healthy gingivae from thirty-two maxillary anterior teeth of eight individuals served as experimental sites. Measurements were obtained from zinc-oxide eugenol impressions of the sulcular area. Subsequent to periods of toothbrushing, the presence of a gingival sulcus could generally be demonstrated. This was observed for the Roll as well as for the Bass method of brushing. Following no-brushing periods, during which chlorhexidine plaque control was exercised, a reduction in sulcus depths and often a disappearance of the sulcus was noted. The purpose of this study was to evaluate microscopically the effect of topically applied 2% chlorhexidine gluconate on the healing of gingival wounds in dogs.

One week after the teeth were scaled and polished, seven experimental wound sites were prepared in the facial gingiva adjacent to the four premolars on each side of the maxillary arch in six young Labrador dogs. From this point the dogs were fed a soft plaque-inducing diet. The objectives of this study were to develop an animal model for testing oral bone and marrow transplantation that is adaptable to immunological studies; to determine the effect of an isogeneic graft of cancellous bone and marrow on the healing of surgically created defects; and to determine the fate of grafted isogeneic cancellous bone and marrow transplanted into defects. Fourteen CBA/H mice served as experimental animals and bilateral intraoral defects were prepared. The experimental side received isogeneic graft material while the other side served as a control. The purpose of this study was: to develop a model for testing oral bone transplantation; to study the healing of a surgically produced defect with and without the transplantation of strongly allogeneic cancellous bones and marrow; and to determine the fate of the allogenic grafts. Twenty-eight CBA/H mice served as experimental animals. Intra-oral defects were made and in 14 of the animals strongly allogenic cancellous bone and marrow were transplanted. This study examined the fluid movement in dentin associated with the exposure of the root surface for varying periods of time ranging from 75 minutes to 30 days. Eight kittens received experimental defects on the roots of the canines. Just prior to sacrifice a fluorescing dye was injected intraperitoneally to make the fluid within the dentin visible under ultraviolet light. Frozen ground sections were prepared and examined. The results showed that the exposed side of the root had a reduced amount of fluid fluorescence when compared to the opposite side of the tooth which had an intact root surface. The …

Infiltration to an injury site or area of bacterial contamination by polymorphonuclear leucocytes (or neutrophils) is an early and essential phase of the early inflammatory reaction. Their action on foreign matter is the most critical step taken by body defense mechanisms in the first 24 hours. Anything which interferes with that action, phagocytosis, greatly hampers early inflammation. This study was designed to see the effect of elevated glucose levels on neutrophil phagocytosis in a unique strain of genetically diabetic mice.

The Hydrodent toothbrush was compared to conventional toothbrush (Lactona # 19) in its effectiveness on removing the plaque, eliminating the pocket depth, and improving the gingival tissue condition.

Forty six male and female subjects with the age range of 18 to 50 years old participated in a 5 month study with 4 week intervals. Each subject received a Hydrodent brush of soft, medium, and hard textures and a conventional toothbrush. The toothbrushes were given randomly and the subject served as his own control.

The periapical dental granuloma and cyst is the most frequent sequela of pulpitis. These lesions are always intraosseous and often become quiescent after the acute phases are over, forming a space which primarily consists of chronic inflammatory tissue. This chronic inflammatory tissue usually appears as a one-centimeter area of punched out bone on the x-ray film and is referred to as a dental granuloma. The purpose of this study was to evaluate the histopathological and clinical changes that occurred when a continuous supply of exogenous acid or base, at varying pH, was delivered adjacent to the gingival crest of the monkey. A related goal was to develop a monkey model system which would deliver test solutions continuously and atraumatically, to the experimental gingival sites.

This thesis describes an in vivo perfusion apparatus which can continuously deliver controlled quantities of solutions to localized, contralateral intraoral areas in albino rats. Three animals were per fused on the experimental area (the mesial sulcular area of the maxillary first molar) on one side with tap water and Trypan blue. Localization of the solution delivered and minimal histological changes due to the presence of the apparatus was demonstrated on these animals. The purpose of this study was to evaluate pocket reduction following gingival curettage. The effects of temporary pressure packing, inflammation, oral hygiene, pocket depth, age and sex of the patient were evaluated.

Three hundred twenty-eight periodontal pockets were studied in forty-one adult patients selected with interdental lesions ranging from 3.0 - 7.5 mm. in depth. Pocket depth and gingival height were measured preoperatively, one month postoperatively and two months postoperatively. The mean change in pocket depth and gingival height was calculated and statistical significance was determined for all measurements.

The effect of a single viewing of a patient-counseling film and a single instruction by a hygienist in producing changes in the oral hygiene of a group of school children 12 to 13 years of age was investigated. The subjects were divided into three experimental groups and one control group of twenty-one each.

A different method of oral hygiene instructions was given to each of the experimental groups. One group viewed the film, the second group was instructed by a hygienist, and the third group viewed the film and was instructed by the hygienist. The control group was not given …

Two-hour postprandial blood sugar determinations were carried out on 500 patients referred for periodontal care. The mean percentage of alveolar bone loss around the mandibular bicuspid teeth was established for each individual. The subjects were then grouped according to blood sugar levels and the mean alveolar bone loss was calculated. The group was separated into male and female sub-groups and data was computed for these segments as well as the total sample. The data was evaluated in a similar manner for those subjects between 40 and 49 years of age.

Samples of blood were obtained from thirty healthy young adult males (Loma Linda University School of Dentistry), and contaminated with bacteria. Varying concentrations of pure glucose were added to this mixture to observe the effect of the glucose on in vitro neutrophilic phagocytosis. As the glucose levels increased a statistically significant suppression was noted in the ability of the neutrophilic leukocytes to engulf Staphylococcus epidermidis.

It was not possible, however, to predict the degree of suppression for any particular level of glucose added nor is it possible to state that suppression will occur in any one individual.

Presently accepted concepts of the etiology and pathogenesis of periodontal disease do not adequately explain the following consistent clinical observations: (1) equal local etiologic factors do not result in equal periodontal destruction in all patients; and, (2) equal therapy is not equally successful in all patients. The differences in patient response must be due to differences within the patient. By relating the broadened concept of disease, as described by Engel, to periodontal disease, the reasons for these individual patient differences become apparent.
