Case Description: This case report describes the successful management of a left mandibular first molar with a combined periodontic-endodontic lesion in a 35-year-old Caucasian woman with aggressive periodontitis using a concerted approach including endodontic treatment, periodontal therapy, and a periodontal regenerative procedure using an enamel matrix derivate. In spite of anticipated poor prognosis, the tooth lesion healed. This case report also discusses the rationale behind different treatment interventions. Practical Implication. Periodontic-endodontic lesions can be successfully treated if dental professionals follow a concerted treatment protocol that integrates endodontic and periodontic specialties. Apical periodontitis is the most common pathological process in the periapical region. Disciplined clinical and radiographic evaluations and appropriate diagnostic tests can detect lesions related to apical periodontitis. Aside from this, lesions mimicking pulp-related pathology but unrelated to pulpal infection and necrosis are occasionally discovered in the periapical region. As a spirochete, the genus Treponema is one of the few major bacterial groups whose natural phylogenic relationships are evident at the level of gross phenotypic characteristics such as their morphology. Treponema spp. are highly invasive due to their unique motility in dense media, and their ability to penetrate cell layers. This feature is associated with the helical cell body and the presence of flagellar filaments in the periplasm. A histological comparison of forty-four human teeth in which vital partial pulpectomies were performed, and the pulp stumps covered with dycal, N-2 and ZOE was carried out. The time interval at which the teeth were extracted for study was ten, twelve and fourteen weeks. These teeth were on patients twelve through seventeen years of age and were to he extracted for orthodontic purposes. The teeth were caries free, vital and asymptomatic. The percent retaining vital tissue, time intervals and type of paste were as follows:

The periapical dental granuloma and cyst is the most frequent sequela of pulpitis. These lesions are always intraosseous and often become quiescent after the acute phases are over, forming a space which primarily consists of chronic inflammatory tissue. This chronic inflammatory tissue usually appears as a one-centimeter area of punched out bone on the x-ray film and is referred to as a dental granuloma. Should epithelium choose to proliferate in this inflammatory area, it may undergo subsequent cystic degeneration forming the typical periapical cyst.
