Tooth transposition is a rare but disturbing problem for patients in esthetic and functional aspects. In this report, we describe a case with unilateral transposition of the maxillary canine and premolar. The patient received full mouth non-extraction orthodontic treatment with upper 1st premolar substitution for upper canine. Crown morphology modification and gingival plastic surgery for harmonious anterior esthetics and function were performed. Patient was satisfied with the treatment outcome.

This is not a perfect case. I made some mistakes during the treatment. I wish every orthodontics can learn something from the case and prevent the mistakes that I have made. This was a 11 years old girl and her chief complained is anterior teeth crowding. The findings of examination were convex facial profile, protrusive upper and lower lip, Angle Class I malocclusion, upper right and lower left canine impacted. When we notice missing maxillary incisors, how to restore the extraction site is a difficult decision. Usually, we restore this kind of tooth with conventional fixed partial denture or implant prosthesis. However, some literatures present another solution: tooth substitution with proximal tooth by orthodontic force. In this paper, we put together few cases treated in Chi-mei orthodontic department, and we also discover some problems in the process of the orthodontic treatment. Hence, if we meet a case alike to these, the experiences we had can help to improve the orthodontic results.
