A dental patient may present with a fixed partial denture such that a tooth or a fractured tooth root that was once a terminal abutment or a pier abutment of the fixed partial denture may need extraction from underneath the abutment without removing the fixed partial denture. The patient may need to retain the fixed partial denture for financial or esthetic reasons, or to use the fixed partial denture as a long-term provisional restoration until the extraction site heals, or until a new fixed partial denture is fabricated.

This two-case report presents the use of the Individual Patient Solutions (IPS) Implants® Preprosthetic system in two patients with distinct clinical needs.

Case #1 involves a 69-year-old male with significant unilateral maxillary bone loss and a residual oroantral communication (OAC) following the failure of both conventional implants and sinus lift procedures. Case #2 involves a patient with a severely atrophic, edentulous mandible. These cases present two distinct and challenging clinical scenarios where both traditional endosseous and zygomatic implants were not feasible. Both cases were successfully managed with a custom-designed IPS subperiosteal implant.

Subcutaneous Facial Emphysema And Pneumomediastinum Following A Class V Dental Restoration: A Case Report, Jonathan Tran Dds, Brian M. Will, Sidney B. Eisig D.D.S., Facs

Cervicofacial subcutaneous emphysema is an uncommon but potentially life-threatening complication of dental procedures. While classically associated with surgical extractions performed with dental high-speed handpieces instead of surgical rear-exhaust handpieces, it has been documented following other dental procedures, including restorative and endodontic treatments. As cervicofacial air emphysema spreads, it may reach the retropharyngeal space, which connects to the mediastinum, leading to pneumomediastinum and a risk for mediastinitis, which carries a high mortality rate. We present a case of cervicofacial subcutaneous emphysema leading to pneumomediastinum following a Class V restoration on tooth #28 to highlight the importance of recognizing this underdiagnosed complication.

Multiple sclerosis (MS) is a chronic central nervous system disease of inflammation and demyelination. Presentation can vary tremendously but, typically, involves neurologic deficits, including weakness, pain, numbness and motor dysfunction. MS can also affect the dentoalveolar region. We describe a patient with MS who clinically presented with “electric” and “shooting, pulling” pain in the left maxillary region. The pain subsided following a course of gabapentin, leading to the diagnosis of trigeminal neuralgia secondary to MS. This report aims to review various common dentoalveolar manifestations of MS.

Maxillofacial arteriovenous malformations (AVMs) are a rare condition in which arteries and veins are in continuity without a capillary bed in between, which poses a significant risk of severe bleeding. Intraosseous AVMs can lead to cortical bone expansion and facial deformity. Dental procedures may result in life-threatening bleeding risk if this condition is not recognized. This report describes a patient experiencing pain in the lower right side of the mouth, along with a taste of blood in the mornings and a tingling sensation in the chin and lower lip. Background: Heterotrophic ossification (HO) is defined as the formation of lamellar bone in soft tissues where normally bone does not exist. It is a rehabilitative disease which can be associated with any joint in the body; however, its pathogenesis and management in the temporomandibular joint (TMJ) is infrequently mentioned in the literature.

Types of studies reviewed: The authors searched various articles which included systematic reviews, meta-analyses, randomized clinical trials, Cohort studies, case reports and literature review on heterotopic bone formation and treatment in PubMed, Google Scholar and Scopus.

Dynamic surgical guidance (DSG) is a recent advance in implant dentistry, used in an effort to maximize the efficiency and predictability of implant surgeries. A study, conducted by Navigate Surgical Technologies using the Inliant system, was undertaken in Toronto, Canada to analyze the success and accuracy of this technology. Twenty-three implants placed in twenty-two subjects were examined. The results showed that implant placement using Inliant was extremely accurate when compared to the pre-surgical plan. Oral cancer resections cause tissue defects which result in functional complications such as masticatory dysfunction, dysphagia, articulation and phonation issues. When the defect involves the maxillary arch, obturation is needed whether it be surgical or prosthetic. In cases where surgical procedures become ineffective, prosthetic options could be challenging. This is a case of a 78-year-old male developed the diagnosis of squamous cell carcinoma of the right alveolar ridge. The aim of the present study is to determine the prevalence and demographics of temporomandibular joint (TMJ) condylar changes in the asymptomatic general population of a hospital based Oral and Maxillofacial Surgery (OMFS) practice. We retrospectively reviewed cone beam computed tomography (CBCT) scans of 272 TMJs from 136 consecutive patients to detect osteophyte, subchondral cyst, sclerosis, cortical erosions, and flattening. While gender did not appear to play a significant role in TMJ pathology, age was positively correlated with prevalence of condylar changes. Osteomas are frequently reported in the maxillofacial region with much lower incidences in the maxillary and sphenoid sinuses. An unerupted third molar within the maxillary sinus coexisting with a maxillary sinus osteoma is an extremely rare pathologic finding. Such cases can be managed endoscopically or intraorally through a Caldwell-Luc approach. The following case report deals with a patient who presents with an uncommon pathological finding: an osteoma embedding an ectopic tooth within the maxillary sinus.

The nasopalatine duct cyst (NPDC) is one of the most common non-odontogenic, non-neoplastic developmental cysts. It arises from the epithelial remnants of the nasopalatine duct within the incisive canal and crosses the midline of the anterior maxilla. Most cases are asymptomatic and either are discovered by chance on radiographs or present as a soft tissue swelling, pain, or drainage in the palate. The present study describes the diagnosis and surgical management of two NPDC cases at the Department of Oral and Maxillofacial Surgery at the Brooklyn Hospital Center.

Background: Antral foreign bodies disrupt normal sinus drainage and can lead to sinusitis and infection. These foreign bodies can arise from many sources including several unique to the dental setting. Case Description: Typical sources like displaced teeth, roots, and root canal filling materials would be iatrogenically introduced at the time of sinus violation. This is a Case Report of a 72 y/ old patient that was wearing two removable prostheses, a maxillary complete denture and a mandibular complete denture retained by two implants and locator attachments. Purpose: Assess mandibular condylar/subcondylar and angle fracture patterns in the presence or absence of third molars. Patients and Methods: Retrospective cohort study of 234 patients with mandibular fractures. Odds ratios were calculated to determine relative risk of angle and condylar/subcondylar fractures when third molar teeth were present or absent. Results: Condylar/subcondylar fractures were twice as likely to be avoided if a third molar was present; angle fractures were twice as likely to occur with retained third molars. Mucoceles are the quotidian, benign minor salivary gland lesions of the oral mucosa. They result from the accumulation of mucous secretion due to trauma or alteration of minor salivary glands. Clinically they are characterized by bluish, dome shaped, soft and fluctuant physiognomy with most frequent occurrence reported on the lower lip. Based on the histological features they are of two varieties mucous extravasation and retention cysts. Various treatment modalities including surgery, laser ablation, electrosurgery, marsupialization, cryosurgery, intra-lesional injections and corticosteroids have been reported in literature.
