Purpose: To assess the impact of dental Medicaid reform in Virginia on dental practice settings (private practice, corporate practice and safety net clinics). Methods: This retrospective cohort study of 16.2 million dental claims is from the Virginia Department of Medical Assistance Services, which included claims for providers participating in Virginia’s Medicaid program during a 10-year period (2002-2012). The dividing date for the reform was July 1, 2005. The outcome measure was mean claims per participating provider. A Poisson regression model was used to predict the mean number of claims per provider with the following predictors: reform period, practice setting, provider …

Background: Elevated blood lead levels (EBLL) are declining in the United States, although some population subgroups continue to exhibit significant health disparities. A childhood lead poisoning prevention program was recently started in Nevada, and many efforts have been made to support this program and increase the screening rates.

Methods: To expand the potential pool of children screened for EBLLs, a pilot study was performed to evaluate lead concentrations in extracted deciduous teeth using Graphic Furnace Atomic Absorption Spectrometry (GFAAS), Inductively Coupled Plasma Mass Spectrometry (ICP-MS), and X-Ray Fluorescence (XRF) analysis.

Periodontal Resident Self-Assessment Of Ergonomics Before And After Videotaped Surgeries, Corin Marantz

Objective: To examine whether self-assessment of videotaped surgeries helps improve periodontal residents’ ergonomics. Methods: Residents (n=8) provided self-assessments of their own ergonomics while performing periodontal surgery using a questionnaire with open and closed items. Results were analyzed using quantitative and qualitative means. Results: Comparison of responses resulted in a change between Pre-video Surgery 1 and PSV1 (p<0.05) and between the three occasions for flat foot and horizontal shoulder positions (p<0.05). Resident goals were most numerous for improving positions of shoulder, back and neck and most notable responses for failure to achieve goals were the need for surgical access and being too focused the procedure. Conclusions: Videotape review is a valid means of self-assessment. Intervention solely in the form of a questionnaire and videotape review was insufficient in its ability to change the residents’ ergonomics. Barriers to implementation of proper ergonomics were identified.

Objectives: This study was designed to investigate the dental health status of several communities in Honduras in terms of the DMFT index (decayed missing and filled teeth). Specifically the study focused on comparisons of the Garifuna (a maroon community in Tela) with the people of San Jose, Copan, and others who traveled from other regions to come to a medical clinic. These results were also compared to other populations of developing and developed countries. Through looking at each participant’s DMFT score, many possible correlations to oral health were explored including region, age, gender, snacking, tobacco use, and oral hygiene. …

Purpose: The aim of this study was to determine if the use of local anesthesia with general anesthesia results in less postoperative pain. The alternative hypothesis is that children will experience less postoperative discomfort when utilizing intraligamental local anesthetic during the intra-operative time period. Methods: Patients were recruited for this single blind, randomized, prospective cohort study with the following inclusion criteria: English speaking children age 3-6 years, ASA I/II requiring general anesthesia for dental treatment. Objective: Orthodontic treatment can often effect changes in the facial complex. In order to assess what those changes are, precise and reliable methods for measuring facial structures need to be used. While the techniques used for measuring have become increasingly more sophisticated they have also become more expensive and cumbersome for daily use. This study investigated the reliability of two methods, physical anthropometry and photogrammetry, that were inexpensive and relatively easy to set up.

The vast majority of periodontal pathogens are classified as gram negative bacteria. These bacteria have an outermost membrane consisting of lipopolysaccharide (LPS), a molecule that can act as an endotoxin and elicit an immune response. The lipopolysaccharide structure consists of three major regions: oligosaccharide, core, and lipid A. Oligosaccharide is exposed on the cell surface, and lipid A anchors LPS in the outer membrane. LPS has been known to hinder certain defensive mechanisms of the immune system by secreting products that interfere with signals that promote chemotaxis of leukocytes enabling the pathogens to go unrestricted. The maxillary lateral incisor is the second most frequently missing tooth in the dental arch with clinical management requiring a complex and multidisciplinary treatment approach. It has been suspected that teeth could be smaller in the maxillary or mandibular arches in patients experiencing agenesis of the maxillary lateral incisor, making ideal occlusal relationships and space creation for restoration of the lateral incisor difficult to obtain.
