Objective: (1) to compare the prophylactic effect of a single versus a multiple dose antibiotic regimens and (2) to determine factors related to surgical site infection in orthognathic surgery. Materials and methods: Medical records of patients underwent orthognathic surgery in the Faculty of Dentistry, Chulalongkorn University between 2014 and 2016 were extracted for medical data including demographic data, American Society of Anesthesiologists (ASA)-classified past medical history, orthognathic surgical procedures and complications, prophylactic antibiotic regimen, and surgical site infection. Objectives: This study aimed to i) determine the kinetic changes of immune cell phenotypes, including an exhaustion marker PD-1, in spleens of murine sepsis ii) investigate the susceptibility against secondary fungal infection after sepsis, and iii) investigate the efficacy of an anti-PD-1 treatment in post-sepsis fungal infection. Periodontium is a specialized tissue laying between tooth and surrounding bone. It functions as force cushion. In particular stimulation including mechanical force, periodontal ligament (PDL) cells which are mechanosensitive cells have the ability to secrete specific cytokines and proteins. PDL cells also have the potential to differentiate into osteoblast-like cells or fibroblast-like cells. Aim: To evaluate the effect of disking technique and resin modified glass ionomer cement (RMGI) restoration on proximal caries of primary upper incisors in tribal preschool children, aged 3-5 years, attending child development centers of Doi Tung Development Project, Chiang Rai. Methods: Twenty tribal preschool children were enrolled in this match-paired design study. Twenty-six proximal carious surfaces were randomized for RMGI restoration (control group), and 26 proximal carious surfaces were randomized for disking technique (experimental group). Previous studies suggest that periodontitis may be associated with an increasing risk of hypertension. However, data from large epidemiological studies are limited. The aim of this cross-sectional study was to examine the association between periodontitis and hypertension using data from a cohort study in a Thai population. The study population comprised of 1,378 employees of the Electricity Generating Authority of Thailand (EGAT), aged 53 to 73 years old, were categorized into no/mild, moderate, or severe periodontitis groups. This study evaluated the effect of various curing conditions in pressure cooker on flexural strength of hard chairside reline resins (UnifastTM Trad, Tokuyama® Rebase II Fast with and without Hardener®). For each material, 70 specimens were prepared and divided into 7 groups consists of control group and 6 experimental groups: cured in 2 bars air or nitrogen pressure cooker at 55ºC for 10, 15 and 20 min respectively. ผลของกาวติดฟันเทียมต่อประสิทธิภาพการบดเคี้ยวและแรงบดเคี้ยวสูงสุดในผู้ป่วยที่ใส่ฟันเทียมทั้งปาก, หยาดฝน พลศรี

วัตถุประสงค์เพื่อศึกษาผลของกาวติดฟันเทียมต่อประสิทธิภาพการบดเคี้ยว (masticatory performance) และแรงบดเคี้ยวสูงสุด (maximum occlusal force) ของผู้ป่วยฟันเทียมทั้งปาก ผู้ป่วยสันเหงือกว่างที่ใส่ฟันเทียมทั้งปากบนและล่างจำนวน 65 คน อายุเฉลี่ย 68.9±7.2 ปี เป็นเพศชาย 39 คน เพศหญิง 26 คน มีคุณภาพฟันเทียมอยู่ในระดับที่ยอมรับได้ (กลุ่ม A) และยอมรับไม่ได้ (กลุ่ม UA) จำนวน 44 และ 21 คน ตามลำดับ มีลักษณะของเนื้อเยื่อรองรับฟันเทียมตามระบบของ American College of Prosthodontists กลุ่ม I, II, III และ IV จำนวน 19 30 11 และ 5 คนตามลำดับ ผู้ป่วยเข้ารับการทดสอบประสิทธิภาพการบดเคี้ยวด้วยการเคี้ยวถั่วลิสงจำนวน 20 รอบวงเคี้ยว ถั่วลิสงที่ผ่านการเคี้ยวแล้วจะถูกร่อนผ่านเครื่องสั่น ด้วยตะแกรงทดสอบมาตรฐานจำนวน 12 ชั้น เพื่อวิเคราะห์หาขนาดอนุภาคกลางของถั่วลิสง (median particle size) และเข้ารับการทดสอบแรงบดเคี้ยวสูงสุดด้วยการกัดแผ่นฟิล์มที่อ่อนไหวต่อความดัน (pressure sensitive film) โดยวางแผ่นฟิล์มระหว่างด้านบดเคี้ยวของฟันเทียมบนและล่างในตำแหน่งสบสนิทที่สุด (maximum intercuspal position) วิเคราะห์และแปรผลด้วยโปรแกรมสถิติสำเร็จรูป เพื่อเปรียบเทียบประสิทธิภาพการบดเคี้ยวและแรงบดเคี้ยวสูงสุดระหว่างก่อน (T1) และหลังการใช้กาวติดฟันเทียมอย่างต่อเนื่องเป็นระยะเวลา 1 เดือน (T2) สถิติที่ใช้ในการทดสอบคือ paired t-test ที่ระดับความเชื่อมั่นร้อยละ 95 ผลการศึกษาพบว่าเมื่อเปรียบเทียบก่อนและหลังการใช้กาวติดฟันเทียม ประสิทธิภาพการบดเคี้ยวเพิ่มขึ้นอย่างมีนัยสำคัญในกลุ่ม A (P = 0.035) แต่ไม่มีความแตกต่างอย่างมีนัยสำคัญทางสถิตในกลุ่ม UA (P = 0.313) แรงบดเคี้ยวสูงสุดลดลงอย่างมีนัยสำคัญในกลุ่ม A (P = 0.020) แต่ไม่มีความแตกต่างอย่างมีนัยสำคัญทางสถิติในกลุ่ม UA (P …

ประสิทธิภาพของซิลเวอร์ไดเอมีนฟลูออไรด์ในการคืนแร่ธาตุในรอยผุชั้นเนื้อฟัน: การวิจัยเชิงทดลองในห้องปฏิบัติการ, กันตพร คุณพนิชกิจ

การศึกษานี้มีวัตถุประสงค์เพื่อเปรียบเทียบประสิทธิภาพของซิลเวอร์ไดเอมีนฟลูออไรด์ที่มีจำหน่ายในท้องตลาด ในการคืนแร่ธาตุในรอยผุชั้นเนื้อฟันของฟันน้ำนม การศึกษานี้เป็นการศึกษาในห้องปฏิบัติการโดยใช้ชิ้นฟันตัวอย่างจากฟันกรามน้ำนมที่มีรอยผุตามธรรมชาติชั้นเนื้อฟันจำนวน 36 ชิ้น วัดความหนาแน่นแร่ธาตุของรอยผุเริ่มต้นด้วยเครื่องถ่ายภาพรังสีคอมพิวเตอร์ระดับไมโครเมตรแล้วนำมาคำนวณความลึกรอยผุและความหนาแน่นแร่ธาตุเฉลี่ย แบ่งชิ้นฟันเป็น 3 กลุ่ม กลุ่มที่ 1 ทา Saforide® (Toyo Seiyaku Kesei, Japan) กลุ่มที่ 2 ทา Advantage ArrestTM (Elevate Oral Care, USA) และกลุ่มที่ 3 ทาน้ำปราศจากไอออน (กลุ่มควบคุม) นำไปผ่านกระบวนการสลับกรด-ด่างโดยใช้เชื้อแบคทีเรียเพื่อจำลองสภาวะในช่องปากเป็นเวลา 5 วัน จากนั้นนำมาวัดความหนาแน่นแร่ธาตุหลังการทดลองด้วยเครื่องถ่ายภาพรังสีคอมพิวเตอร์ระดับไมโครเมตร คำนวณความลึกรอยผุ ความหนาแน่นแร่ธาตุเฉลี่ยและร้อยละการเปลี่ยนแปลงความหนาแน่นแร่ธาตุหลังการทดลอง ผลการทดลองพบว่าชิ้นฟันตัวอย่างกลุ่มที่ทา Saforide® และ Advantage ArrestTM มีความลึกรอยผุหลังการทดลองลดลงอย่างมีนัยสำคัญทางสถิติ (p= 0.033 และ 0.021 ตามลำดับ) และมีความหนาแน่นแร่ธาตุเฉลี่ยหลังการทดลองเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p= 0.002 และ 0.002 ตามลำดับ) พบว่าร้อยละการเปลี่ยนแปลงความหนาแน่นแร่ธาตุระหว่างกลุ่มทดลองทั้งสองกลุ่มไม่แตกต่างกันแต่แตกต่างจากกลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p< 0.001) สรุปได้ว่า Saforide® และ Advantage ArrestTM มีประสิทธิภาพในการคืนแร่ธาตุในรอยผุชั้นเนื้อฟันของฟันน้ำนมใกล้เคียงกัน

การศึกษานี้มีวัตถุประสงค์เพื่อเปรียบเทียบผลของซีเมนต์ชนิดซิงค์ฟอสเฟตซีเมนต์ (ZC) ซิงค์โพลีคาร์บอกซิเลตซีเมนต์ (ZPC) กลาสไอโอโนเมอร์ซีเมนต์ชนิดดั้งเดิม (GI) เรซินมอดิฟายด์กลาสไอโอโนเมอร์ซีเมนต์ชนิดผงและน้ำ (RMGI-P/L) เรซินมอดิฟายด์กลาสไอโอโนเมอร์ซีเมนต์ชนิดสองหลอด (RMGI-P/P) และโพลีแอซิดมอดิฟายด์คอมโพสิตเรซิน (PMCR) ต่อการยับยั้งการสูญเสียแร่ธาตุที่ผิวเคลือบฟันบริเวณขอบของแถบรัดฟัน เป็นการศึกษาในห้องปฏิบัติการโดยใช้ฟันกรามน้อยแท้ของมนุษย์จำนวน 70 ซี่ ทำการขัดผิวเคลือบฟัน และสร้างช่องหน้าต่างทดลองบริเวณกึ่งกลางของด้านไกลกลางขนาด 1 x 2 ตารางมิลลิเมตร แบ่งฟันเป็น 7 กลุ่ม เพื่อยึดด้วย; (1) ZC (Zinc Cement Improved®), (2) ZPC (Hy-Bond Polycarboxylate Cement®), (3) GI (Hy-Bond Glasionomer CX®), (4) RMGI-P/L (GC Fuji Ortho LC®), (5) RMGI-P/P (GC Fuji Ortho Band Paste Pak®), (6) PMCR (Ultra Band-Lok®) และ (7) กลุ่มควบคุมที่ไม่ได้ยึดแถบรัดฟัน (Control) จากนั้นจำลองภาวะอุณหภูมิร้อนเย็นในช่องปาก 24 ชั่วโมง และภาวะการสูญเสียและคืนกลับแร่ธาตุ 21 วัน ทำการรื้อแถบรัดฟันและกำจัดซีเมนต์ก่อนนำชิ้นงานทั้งหมดวัดค่าร้อยละการสูญเสียฟลูออเรสเซนส์บริเวณช่องหน้าต่างทดลอง (∆F), ค่าร้อยละการสูญเสียฟลูออเรสเซนส์สูงสุดบริเวณช่องหน้าต่างทดลอง (∆Fmax) ค่าพื้นที่ที่เกิดการสูญเสียฟลูออเรสเซนส์ (Area) และปริมาตรของเคลือบฟันที่สูญเสียแร่ธาตุ (∆Q) ด้วยเทคนิควิเคราะห์การเรืองแสงเชิงปริมาณด้วยการใช้แสงกระตุ้นชนิดดิจิตอล จากการวิเคราะห์ด้วยสถิติ Kruskal-Wallis test พบความแตกต่างของค่าเฉลี่ย ∆F, ∆Fmax, Area และ ∆Q ของซีเมนต์แต่ละกลุ่มอย่างมีนัยสำคัญทางสถิติ (P<0.001) เมื่อหาความแตกต่างทางสถิติในแต่ละคู่โดยใช้สถิติ Pairwise comparisons ไม่พบความแตกต่างกันอย่างมีนัยสำคัญทางสถิติของค่าเฉลี่ย ∆F, ∆Fmax, Area และ ∆Q เมื่อจับคู่เปรียบเทียบระหว่างกลุ่ม Control:PMCR, ZP:ZPC, ZP:GI, ZP:RMGI-P/L, ZP:RMGI-P/P, ZPC:GI, ZPC:RMGI-P/L, ZPC:RMGI-P/P, GI:RMGI-P/L, GI:RMGI-P/P, RMGI-P/L:RMGI-P/P และ RMGI-P/L:PMCR แต่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติเมื่อจับคู่เปรียบเทียบระหว่างกลุ่ม Control:ZP, Control:ZPC, Control:GI, Control:RMGI-P/L และ Control: RMGI-P/P, PMCR:ZC, PMCR:ZPC, PMCR:GI และ PMCR:RMGI-P/P การศึกษาในครั้งนี้พบว่าฟันซึ่งยึดแถบรัดฟันด้วย ZC, ZPC, GI และ RMGI-P/P ไม่เกิดการสูญเสียแร่ธาตุที่ผิวเคลือบฟันบริเวณขอบของแถบรัดฟัน ส่วนกลุ่ม RMGI-P/L, PMCR และกลุ่มควบคุมเกิดการสูญเสียแร่ธาตุที่ผิวเคลือบฟัน โดยที่กลุ่ม RMGI-P/L เกิดการสูญเสียแร่ธาตุน้อยกว่ากลุ่มควบคุม แต่กลุ่ม PMCR เกิดการสูญเสียแร่ธาตุไม่แตกต่างจากกลุ่มควบคุม

บทนำ รากฟันแตกในแนวดิ่งเป็นหนึ่งในสาเหตุสำคัญของการสูญเสียฟันในฟันที่ผ่านการรักษาคลองรากฟันมาแล้ว ฟันที่มีรากฟันแตกในแนวดิ่งนั้นมักวินิจฉัยได้ยาก จึงได้นำภาพรังสีซีบีซีทีซึ่งสามารถทำให้มองเห็นคลองรากฟันได้ทั้งสามมิติมาตรวจวินิจฉัย แต่ข้อจำกัดของภาพรังสีซีบีซีทีคือเมื่อมีวัสดุทึบรังสีอยู่ในคลองรากฟันจะก่อให้เกิดสิ่งแปลกปนที่มีลักษณะคล้ายกับรอยแตก จุดประสงค์ของการศึกษานี้จึงต้องการศึกษาถึงความเที่ยงตรงของภาพรังสีซีบีซีทีในการตรวจหารอยแตกในแนวดิ่งในคลองรากฟันที่ถูกปิดกั้นด้วยวัสดุทึบรังสีชนิดต่างๆ วัสดุและวิธีการ ใช้ฟันกรามน้อยที่มีรากตรงรากเดียวจำนวนหกสิบซี่ โดยฟันสี่สิบซี่จำลองให้เกิดการแตกของรากฟันในแนวดิ่ง นำฟันทุกซี่ถ่ายภาพรังสีไมโครซีทีเพื่อยืนยันว่าฟันที่จำลองรอยแตกมีรอยแตกเกิดขึ้นจริง ฟันทุกซี่จะถูกถ่ายภาพรังสีซีบีซีที โดยไม่มีวัสดุทึบรังสีในคลองรากฟันใช้เป็นกลุ่มควบคุม และมีการใส่เดือยฟันโลหะเหวี่ยง เดือยฟันเส้นใยเรซิน และกัตตาเพอร์ชาเป็นวัสดุทึบรังสีในคลองรากฟัน ผู้แปลภาพรังสีสี่คนทำการแปลภาพรังสี และคำนวณหาค่าความไว ความจำเพาะ และความเที่ยงตรงของภาพรังสีซีบีซีทีในการตรวจหารากฟันแตกในแนวดิ่ง ผลการทดลอง ในคลองรากฟันที่ไม่มีวัสดุทึบรังสีพบว่าพบว่าซีบีซีทีมีค่าความไว ความจำเพาะ และความเที่ยงตรงอยู่ที่ ร้อยละ 77.5 ร้อยละ 88.75 และร้อยละ 81.86 ตามลำดับ หากมีวัสดุทึบรังสีปรากฏในคลองรากฟันจะทำให้ค่าความจำเพาะและความเที่ยงตรงของซีบีซีทีในการตรวจหารากฟันแตกในแนวดิ่งลดลง สรุป ภาพถ่ายรังสีซีบีซีทีมีความเที่ยงตรงสูงในการตรวจหารากฟันแตกในแนวดิ่งในคลองรากฟันที่ไม่มีวัสดุทึบรัง ซึ่งหากมีวัสดุทึบรังสีปรากฏในคลองรากฟันแล้วจะทำให้ความสามารถของซีบีซีทีในการวินิจฉัยรากฟันแตกในแนวดิ่งลดลง

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาผลของการปรับเปลี่ยนชีวภาพเนื้อฟันด้วยสารสกัดจากเมล็ดองุ่นในการลดปริมาณแอคทีฟเจลาติเนส เมื่อใช้ร่วมกับสารยึดติดยูนิเวอร์แซล เอทช์แอนด์รินส์โหมด โดยใช้ผงเนื้อฟันที่เตรียมจากฟันกรามใหญ่ซี่ที่สามของมนุษย์ในการวัดปริมาณแอคทีฟเอ็มเอ็มพี ชนิดที่ 2 และใช้ชิ้นตัวอย่างการยึดติดเรซินและเนื้อฟันเพื่อศึกษาตำแหน่งที่มีแอคทีฟเจลาติเนสในชั้นไฮบริด แบ่งกลุ่มการทดลองเป็น 6 กลุ่ม คือ เนื้อฟันที่ไม่ปรับสภาพผิวใด ๆ กลุ่มเนื้อฟันที่กัดด้วยกรดฟอสฟอริก กลุ่มเนื้อฟันที่ใช้สารยึดติดยูนิเวอร์แซล เอทช์แอนด์รินส์โหมด ซิงเกิลบอนด์ ยูนิเวอร์แซล หรือ จี พรีมิโอ บอนด์ และกลุ่มที่มีการปรับสภาพเนื้อฟันด้วยสารสกัดจากเมล็ดองุ่นนาน 1 นาที หลังกัดด้วยกรดฟอสฟอริก ร่วมกับการใช้สารยึดติดดังกล่าว วิเคราะห์ข้อมูลทางสถิติด้วยการวิเคราะห์ความแปรปรวนทางเดียวและการทดสอบบองเฟอโรนี ที่ระดับนัยสำคัญทางสถิติ 0.05 พบว่า กระบวนการยึดติดเนื้อฟันด้วยสารยึดติดยูนิเวอร์แซล เอทช์แอนด์รินส์โหมด มีผลลดปริมาณแอคทีฟเอ็มเอ็มพี ชนิดที่ 2 และการปรับเปลี่ยนชีวภาพเนื้อฟันด้วยสารสกัดจากเมล็ดองุ่นยิ่งทำให้ลดปริมาณแอคทีฟเอ็มเอ็มพี ชนิดที่ 2 โดยเฉพาะอย่างยิ่ง เมื่อใช้สารยึดติด จี พรีมิโอ บอนด์ สอดคล้องกับผลจากการศึกษาตำแหน่งที่พบแอคทีฟเจลาติเนสในชั้นไฮบริดลดลงเมื่อใช้สารสกัดจากเมล็ดองุ่นก่อนการทาสารยึดติด จี พรีมิโอ บอนด์ อาจสรุปได้ว่าสารสกัดจากเมล็ดองุ่นช่วยส่งผลดีในการลดปริมาณแอคทีฟเจลาติเนส โดยเฉพาะอย่างยิ่งเมื่อใช้ร่วมกับสารยึดติดที่มีความไม่ชอบน้ำสูง แต่อย่างไรก็ตาม ยังต้องศึกษาเพิ่มเติมเกี่ยวกับกลไกการยับยั้งการทำงานของเอนไซม์ย่อยโปรตีน ผลต่อค่ากำลังการยึดติดและประสิทธิภาพของสารสกัดจากเมล็ดองุ่นในการยืดอายุของการยึดติดเนื้อฟัน

Objectives: To investigate craniofacial and dental characteristics and identify mutations of the patients affected with osteogenesis imperfecta (OI) and dentinogenesis imperfecta (DGI). Methods: Patients included in this study were diagnosed with OI. Physical, oro-dental, and radiographic examinations were performed. Mutation was identified by whole exome sequencing. Tooth characteristics including mineral density, tooth color, surface roughness, hardness, elastic modulus, mineral compositions, and ultrastructure of DGI teeth were studied compared to the controls. Objective: To investigate oro-dental characteristics and amelogenesis imperfecta (AI)-causing genomic variants associated with AI. Methods: Three Thai families affected with autosomal dominant hypocalcified amelogenesis imperfecta (ADHCAI) were recruited. Whole exome sequencing was used to identify mutations, confirmed by sanger sequencing. Clinical and radiographic features were recorded. The purpose of this study was to investigate if the addition of biologic materials and/or oral steroids would affect horizontal bone gain, or the bone density of the grafted bone in horizontal alveolar ridge augmentations. A retrospective chart review was completed to assess the clinical and radiographic outcomes of 53 ridge augmentation patients. An average bone gain of 3.6mm of width was found in our study based on radiographic analysis. Purpose: To determine the characteristics influencing periodontal practices in Virginia and report on the anticipated changes in practice.

Methods: A REDCap survey was emailed to Virginia based American Academy of Periodontology (AAP) members assessing personal and practice demographics, trends in dental treatment and practice models, and how periodontists will adjust to account for these trends.

Results: The response rate was 31% (n=46). Respondents report greater referrals from more experienced general practitioners (GPs). Student debt was associated with age. Purpose: To assess dental students and pediatric dental residents’ knowledge and attitudes towards motivational interviewing and evaluate the efficacy of a pediatric-focused brief motivational interviewing training program.

Methods: A total of 66 participants were enrolled; 35% were third year dental students, 46% were fourth year dental students and 18% were pediatric dental residents. Participants completed three questionnaires: the first before the training, the second immediately after the training and the third approximately 3.5 months after the training.

Objective: To determine which website characteristics are preferred by orthodontists, adult patients, and parents of patients.

Materials and Methods: 1,000 active members of the American Association of Orthodontists and 750 active orthodontic patients/parents were sampled. Participants rated the importance of website characteristics, indicated presence of those characteristics on the current website, and ranked sample website images. Preferences were compared between orthodontist and the patient/parent group using t-tests and sample websites were compared using ANOVA models and Tukey’s adjusted post-hoc tests. The aim of this study was to determine the change in working length (∆WL) before and after coronal flaring and after complete rotary instrumentation as a function of canal curvature. One mesiobuccal or mesiolingual canal from each of 43 extracted molars had coronal standardization and access performed. Periodontitis is an inflammatory disease of the oral cavity induced by anaerobic bacteria, that remains to be the primary cause of tooth loss in adults worldwide. Finding an anti-microbial therapeutic to selectively target periodontal pathogens has proven to be difficult, and current treatment modalities only provide a transient benefit.
